A woman who was flagged at two ­Limerick hospitals for sepsis, but who was not given the prescribed medication in time, died from cardiac respiratory failure a day after suffering a miscarriage, an inquest into her death heard yesterday.

Leona Cusack (33), of Ballycasey, Co Clare, might have survived if she had received a “broad spectrum” of anti­biotics. She died on February 18, 2024.
