A woman has died following a multi-vehicle crash on Eglinton Ave. near the DVP.

A 60-year-old woman has died following a collision involving three vehicles in North York on Sunday afternoon, police say.

The crash happened at around 4:10 p.m. on the northbound off-ramp to the Don Valley Parkway (DVP) at Eglinton Avenue East.

Crash Eglinton/DVP Dec. 21 A driver has been rushed to the hospital with life-threatening injuries following a multi-vehicle collision on Eglinton Avenue East, just west of the Don Valley Parkway, on Dec. 21.

Investigators say a woman operating a a Honda Civic was exiting the northbound DVP at Eglinton Avenue East when “for unknown reasons” she did not negotiate the off-ramp to continue eastbound on Eglinton.

Instead, they said, she continued in northbound direction, crossing the eastbound lanes of Eglinton and was subsequently hit by the driver of an an eastbound vehicle.

“The female driver continued northbound crossing the Eglinton LRT tracks and then (was) struck by a westbound vehicle on Eglinton Avenue East, ultimately coming to rest at a construction fence,” Toronto Police Service said in a news release.

The two other vehicles are a Ford Bronco and a Kia Soul.

Police say the female driver was transported to a local trauma centre via emergency run in life-threatening condition. There, she was pronounced deceased.

A second person was taken to the hospital with minor injuries, they said.

Paramedics previously told CTV News Toronto that an adult female was taken to a trauma centre with “serious, life-threatening injuries.”

Three other patients were assessed on scene for minor injuries, they said.

All eastbound lanes on Eglinton remain closed in the vicinity of the collision.

The northbound DVP off-ramp to eastbound Eglinton was also blocked but has since reopened.

Westbound Eglinton, meanwhile, is fully shut down beyond the northbound on-ramp to the DVP.
