Dr. Gregory Pearl, the physician who specializes in thoracic outlet syndrome surgery and diagnosis, is reviewing imaging performed on New York Yankees outfielder Aaron Judge, according to multiple reports.

Pearl previously performed TOS surgery on major league pitchers Kyle Nelson, Merrill Kelly, and Daulton Jeffries, among others.

Judge hasn’t been formally diagnosed with thoracic outlet syndrome — only a bone bruise in his right shoulder. But the possibility of a compressed nerve in the area is perhaps weighing on the Yankees, who haven’t seen Judge since he was removed in the seventh inning of their May 31 game in West Sacramento.

Manager Aaron Boone did not have an update on Judge’s diagnosis after the Yankees’ June 4 game in New York, a 2-1 victory over the Cleveland Guardians. The reigning American League MVP isn’t even on the injured list — let alone headed for surgery — yet.

But he is clearly a step off his usual lofty pace. Judge is slashing .248/.375/.533 with 17 home runs and 38 RBIs and on pace for a 6-WAR season, excellent by most players’ standards.

Those numbers obscure a down month that perhaps reflected an underyling injury. Judge slashed .243/.368/.437 last month, with a slugging percentage closer to the MLB average (.396) than his career mark (.611).

Between the statistical toll of playing through a possible injury, and the time he might need to miss pending his diagnosis, Judge’s streak of two consecutive American League MVP awards is in jeopardy.

In 2025, Judge led the American League in batting average (.331), on-base percentage (.457) and slugging percentage (.688). Through the first two months of 2026, the Yankees are battling the Tampa Bay Rays for first place in the American League East. Judge has been integral to their offense, even if he hasn’t been his usual self.

If he goes on the injured list, Judge will join two other outfielders already sidelined by injuries: Giancarlo Stanton (calf) and Jasson Dominguez (shoulder).
