A 61-year-old man has been charged with manslaughter after his mother died at Hamilton General Hospital following a case of “suspected elder neglect,” police say.

Paramedics responded to a home in Stoney Creek for an elderly woman in medical distress in July 2025.

Police said she was taken to the hospital and admitted to the intensive care unit.

After she was admitted, hospital staff contacted police with concerns the 87-year-old woman was being neglected.

The woman, identified as Zofija Tokic, died at the hospital on July 16.

After a “lengthy” investigation, police charged Peter Tokic with manslaughter and criminal negligence causing death in connection with Zofija.

“Caregivers have a legal duty to provide the necessaries of life to those in their care,” police said in a news release on Monday.

Elder abuse can occur in various forms, police say, including neglect, physical or emotional abuse, or financial exploitation.

Investigators are asking anyone with information to contact them or Crime Stoppers anonymously.
