There’s been much hand-wringing about the evils of consultants since Deloitte admitted artificial intelligence was responsible for the litany of errors and fake references in a report it recently charged the government $440,000 for.

But it turns out it’s not just AI that has professional services firms slipping up. KPMG has also handed the government a report containing a phantom reference, and, in its case, old-fashioned human error was to blame.
