Leo is a 62-year-old aeroplane engineer who presents with a three-month history of progressive exertional dyspnoea, chest tightness and exertional dry cough.

He reports increasing lethargy over the same period but denies recent fevers, chest pain, haemoptysis, night sweats or unintentional weight loss.
