Some Doctors Think So"},"content":{"rendered":"<p id=\"mntl-sc-block_2-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> For decades, diphenhydramine\u2014best known by the brand name Benadryl\u2014has been a go-to medication for relieving allergy symptoms, such as runny noses, watery eyes, and rashes. But despite its longstanding presence in many medicine cabinets, some experts say it\u2019s time to retire the popular drug.\n<\/p>\n<p id=\"mntl-sc-block_4-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> The drug\u2019s side effects and other risks are so concerning that it should no longer be readily available over the counter or widely prescribed, several doctors argued in a paper published in the World Allergy Organization Journal earlier this year titled \u201cDiphenhydramine: It is time to say a final goodbye.\u201d\n<\/p>\n<p id=\"mntl-sc-block_6-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> \u201cIf Benadryl underwent clinical trials today, in my opinion, it would certainly not be approved for the OTC market due to adverse reactions,\u201d <a href=\"https:\/\/www.vcuhealth.org\/find-a-provider\/profile\/lawrence-schwartz\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"externalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">Lawrence B. Schwartz, MD, PhD<\/a>, an allergy and immunology specialist at VCU Health who did not work on the paper, told Health.\n<\/p>\n<p id=\"mntl-sc-block_8-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Here\u2019s why Schwartz and others say it\u2019s time to move on from Benadryl\u2014and whether that means you should, too.\n<\/p>\n<p id=\"mntl-sc-block_11-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Diphenhydramine can be taken by adults and children and is used to treat <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/seasonal-allergies-8348995\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">seasonal allergies<\/a>, hives, motion sickness, and <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/insomnia-symptoms-8404366\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"2\" rel=\"nofollow noopener\" target=\"_blank\">insomnia<\/a>, according to the American Academy of Allergy, Asthma and Immunology.\n<\/p>\n<p id=\"mntl-sc-block_13-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> The medication is what\u2019s known as a \u201cfirst-generation\u201d <a href=\"https:\/\/www.health.com\/antihistamines-8384188\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">antihistamine<\/a>, meaning that it\u2019s an older drug that works by binding to and blocking the body\u2019s histamine receptors, specifically the H1 receptors. Cells throughout the body produce histamine during allergic reactions in blood vessels, airways, and nerves, and it\u2019s also produced in the brain and plays a part in wakefulness.\n<\/p>\n<p id=\"mntl-sc-block_15-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Approved as a prescription drug in 1946 and over the counter in the 1980s, diphenhydramine is now available in approximately 300 formulations either on its own or combined with other ingredients. While there\u2019s no available data on OTC use, more than 1 million prescriptions for stronger versions of diphenhydramine are written in the United States every year.\n<\/p>\n<p id=\"mntl-sc-block_17-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Benadryl is widely used because it\u2019s affordable\u2014the average prescription costs less than $8\u2014and can effectively treat a variety of allergy-related symptoms, from watery, itchy eyes to <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/hives-overview-7109904\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">hives<\/a> and bug bite reactions, according to the recent paper.\n<\/p>\n<p id=\"mntl-sc-block_20-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Like all antihistamines, Benadryl causes drowsiness by blocking H1 receptors and crossing the blood-brain barrier.\n<\/p>\n<p id=\"mntl-sc-block_22-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> But it also does other things that can create side effects. For instance, it blocks sodium channels, helping it to act as a local anesthetic and potentially leading to dangerous cardiac effects.\n<\/p>\n<p id=\"mntl-sc-block_24-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Additionally, it can cause dry mouth, blurry vision, <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/constipation-11699854\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">constipation<\/a>, urinary retention, and an elevated heart rate because it blocks acetylcholine, a neurotransmitter in the central nervous system, <a href=\"https:\/\/www.bidmc.org\/centers-and-departments\/allergy-and-inflammation\/meet-our-team\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"externalLink\" data-ordinal=\"2\" rel=\"nofollow noopener\" target=\"_blank\">Trisha Ray, MD<\/a>, clinical director of Allergy and Immunology at Beth Israel Deaconess Medical Center, told Health.\n<\/p>\n<p id=\"mntl-sc-block_26-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Due to these significant side effects, manufacturers eventually developed <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/best-otc-allergy-medicines-8548050\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">\u201csecond-generation\u201d antihistamines<\/a>, such as loratadine (Claritin), cetirizine (Zyrtec), or fexofenadine (Allegra), to offer gentler options for the body.\n<\/p>\n<p id=\"mntl-sc-block_28-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> These newer medications bind only to H1 receptors and cross the blood-brain barrier at much lower levels than Benadryl, making them less likely to cause drowsiness and other adverse effects, said Ray.<\/p>\n<p id=\"mntl-sc-block_33-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Some experts say that second-generation antihistamines are simply safer than Benadryl.\n<\/p>\n<p id=\"mntl-sc-block_35-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Benadryl\u2019s sedating effects\u2014so effective that the pharmaceutical industry began marketing it as a sleep aid in the 1970s\u2014can affect concentration, attention, memory, and performance well into the next day, authors of the February paper noted.\n<\/p>\n<p id=\"mntl-sc-block_37-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> \u201cPatients are frequently unaware of the potential for these well-established effects,\u201d said <a href=\"https:\/\/profiles.hopkinsmedicine.org\/provider\/james-h-clark\/2700623\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"externalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">James H. Clark, MD<\/a>, the co-director of the Johns Hopkins Dysphagia and Deglutition Clinic at The Johns Hopkins School of Medicine and a co-author of the paper. \u201cThis scenario has resulted in vehicular accidents in patients who have underestimated their sedation after taking such medications.\u201d\n<\/p>\n<p id=\"mntl-sc-block_39-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> One study found that a single dose of diphenhydramine impaired driving more than a 0.1% blood alcohol level, but fexofenadine (Allegra) did not affect driving.\n<\/p>\n<p id=\"mntl-sc-block_41-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Experts cite other reasons Benadryl should be phased out, including its ability to cause:\n<\/p>\n<p>Additional side effects, such as heart rhythm disruptions, constipation, urinary retention, <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/condition\/dry-eye\/dry-eye-symptoms\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">dry eyes<\/a>, blurry vision, and skin flushing<br \/>\nCognitive effects, such as an increased risk of confusion and dementia, including <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/condition\/alzheimers-overview\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"2\" rel=\"nofollow noopener\" target=\"_blank\">Alzheimer\u2019s<\/a>, from cumulative use in adults ages 65 and up<br \/>\nAn increased risk of agitation and confusion in children, which can be followed by extreme sedation and coma<br \/>\nPotential for abuse, as illustrated by the TikTok \u201cBenadryl Challenge\u201d that led to hospitalizations and deaths<\/p>\n<p id=\"mntl-sc-block_45-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> While no direct head-to-head studies have compared the effectiveness of second-generation medications with that of Benadryl, numerous studies suggest that these medications relieve allergy symptoms as effectively, with fewer side effects, the paper\u2019s authors said.\n<\/p>\n<p id=\"mntl-sc-block_47-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> \u201cThe presence of effective and safer second-generation antihistamines, frequent and sometimes severe adverse reactions to first-generation agents, as well as its demonstrated abuse potential, strongly suggest it is time to remove diphenhydramine\u2019s availability from both the prescription and over-the-counter markets,\u201d they concluded.<\/p>\n<p id=\"mntl-sc-block_50-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> The study authors aren\u2019t the only ones who advise against Benadryl as a first-line defense.\u00a0\n<\/p>\n<p id=\"mntl-sc-block_52-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> In guidance from 2020, the American Academy of Allergy, Asthma, and Immunology and the American College of Asthma, Allergy, and Immunology recommends that when treating <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/allergic-rhinitis-8624580\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">rhinitis<\/a>, a collection of symptoms affecting the nose and nasal passages, clinicians prescribe second-generation oral antihistamines over first-generation options to reduce the risk of adverse effects. Many global guidelines also favor second-generation antihistamines for hives and rhinitis.\n<\/p>\n<p id=\"mntl-sc-block_54-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> For those with chronic allergies, second-generation antihistamines are superior not only because they have fewer side effects but also because they last longer, allowing for consistent use at higher doses, said Ray.\n<\/p>\n<p id=\"mntl-sc-block_56-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> But Benadryl still has its place in certain situations, <a href=\"https:\/\/www.nationaljewish.org\/doctors-departments\/jessica-hui-beckman\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"externalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">Jessica Hui, MD<\/a>, a pediatric allergist and immunologist at National Jewish Health, told Health.\n<\/p>\n<p id=\"mntl-sc-block_58-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> For example, Benadryl can be preferable if you don\u2019t want to take a medication that lasts as long in the body or in situations where drowsiness is desired, such as before a procedure, said Hui.\n<\/p>\n<p id=\"mntl-sc-block_60-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> In cases of acute allergic reactions or <a class=\"recommendation-inline-link\" href=\"https:\/\/www.health.com\/anaphylaxis-symptoms-8709151\" link-destination-recommendation=\"true\" data-component=\"link\" data-source=\"inlineLink\" data-type=\"internalLink\" data-ordinal=\"1\" rel=\"nofollow noopener\" target=\"_blank\">anaphylaxis<\/a>, Benadryl\u2019s rapid onset of action\u201415 to 30 minutes versus 20 to 60 minutes for second-generation antihistamines\u2014can also make it the top choice, explained Ray.\n<\/p>\n<p id=\"mntl-sc-block_62-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> \u201cThis may not be a major difference in most cases, but in my clinical experience, I have witnessed firsthand the rapid improvement in symptoms from administration of Benadryl in an acute setting,\u201d she said.\n<\/p>\n<p id=\"mntl-sc-block_64-0\" class=\"comp mntl-sc-block mntl-sc-block-html\"> Other than select uses, Hui said she advises allergy sufferers to turn to second-generation antihistamines for relief. \u201cIn general,\u201d she said, \u201csecond-generation antihistamines will be safer, especially in populations like the elderly.\u201d<\/p>\n","protected":false},"excerpt":{"rendered":"For decades, diphenhydramine\u2014best known by the brand name Benadryl\u2014has been a go-to medication for relieving allergy symptoms, such&hellip;\n","protected":false},"author":2,"featured_media":1308,"comment_status":"","ping_status":"","sticky":false,"template":"","format":"standard","meta":{"footnotes":""},"categories":[33],"tags":[64,63,137,490],"class_list":{"0":"post-1307","1":"post","2":"type-post","3":"status-publish","4":"format-standard","5":"has-post-thumbnail","7":"category-medication","8":"tag-au","9":"tag-australia","10":"tag-health","11":"tag-medication"},"_links":{"self":[{"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/posts\/1307","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/posts"}],"about":[{"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/types\/post"}],"author":[{"embeddable":true,"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/users\/2"}],"replies":[{"embeddable":true,"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/comments?post=1307"}],"version-history":[{"count":0,"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/posts\/1307\/revisions"}],"wp:featuredmedia":[{"embeddable":true,"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/media\/1308"}],"wp:attachment":[{"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/media?parent=1307"}],"wp:term":[{"taxonomy":"category","embeddable":true,"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/categories?post=1307"},{"taxonomy":"post_tag","embeddable":true,"href":"https:\/\/www.newsbeep.com\/au\/wp-json\/wp\/v2\/tags?post=1307"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
