Summer 2024.<\/p>\n<p>\u201cHave you thought about GLPs?\u201d my primary care physician asked as she listened to my heart.<\/p>\n<p>I laughed nervously. \u201cNo, but I am now.\u201d Humor has always been my shield.<\/p>\n<p>She didn\u2019t flinch. \u201cYour BMI is over 40. You\u2019re at risk for all sorts of health conditions. Why don\u2019t you look into our weight-loss program?\u201d<\/p>\n<p>My labs were fine. But that didn\u2019t matter. My body size alone was enough to warrant a prescription. I left feeling ashamed, reminded once again that medicine sees fatness as a disease in itself \u2014 regardless of actual health indicators.<\/p>\n<p>This wasn\u2019t the first time.<\/p>\n<p>In 1995, I was 14 years old and weighed 367 pounds when an endocrinologist bluntly told my mother that I would \u201clikely be bedridden by 20.\u201d He described my legs as \u201cenormous\u201d and referred me to bariatric surgery. Soon after, I underwent a stomach stapling procedure that left an eight-inch scar down my chest. <\/p>\n<p>At the time, pediatric weight loss surgery was <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/10733812\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"quite rare\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/10733812\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"0\" rel=\"nofollow noopener\" target=\"_blank\">quite rare<\/a>. But to my doctors, my fat body made it acceptable \u2014 even necessary.<\/p>\n<p>The procedure turned eating into a cycle of pain and vomiting. Food lodged in the tiny passage created by the staples left me doubled over until I threw it back up. I became, in effect, a medically induced bulimic \u2014 praised, nonetheless, for my weight-loss \u201csuccess.\u201d <\/p>\n<p>Within a year, I had lost nearly 100 pounds. Friends, family and even acquaintances congratulated me. No one saw the violence done to my body or the damage it did to my relationship with food. They didn\u2019t know the surgery left me dependent on ice cream, crackers and mashed potatoes because I could easily chew that stuff down to a pureed consistency to avoid the pain that followed whenever food got stuck in my staples. After the surgery, and even still today, I can\u2019t tolerate most fruit \u2014 too acidic \u2014 nor many vegetables \u2014 too fibrous. Stomach stapling didn\u2019t make me \u201chealthier;\u201d it only made me thinner. <\/p>\n<p>That surgery didn\u2019t even \u201ccure\u201d me.<\/p>\n<p>More than two decades later, in 2017, weighing nearly 390 pounds, I went under the knife again, this time for a sleeve gastrectomy. By then, I had surrendered to the <a href=\"https:\/\/finance.yahoo.com\/news\/united-states-weight-loss-market-084700368.html\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"diet industrial complex, a multibillion-dollar ecosystem\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/finance.yahoo.com\/news\/united-states-weight-loss-market-084700368.html\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"1\" rel=\"nofollow noopener\" target=\"_blank\">diet industrial complex, a multibillion-dollar ecosystem<\/a> built around the idea that fat bodies are inherently flawed. The pressure was relentless \u2014 to fix, shrink and manage a body that medicine had always treated as a problem to solve.<\/p>\n<p class=\"cli-pullquote__quote accent-cli\">\u201cFrom childhood to adulthood, I\u2019ve been told that \u2018something must be done.\u2019 Rarely has anyone stopped to ask what living in this body actually means to me.\u201d<\/p>\n<p>And the scrutiny wasn\u2019t limited to exam rooms. In restaurants, on airplanes, at conferences, strangers felt entitled to comment on my body. I\u2019ve been mocked for eating a donut, told loudly in a food court that I was \u201ckilling myself,\u201d and subjected to humiliating requests to be reseated on flights. I\u2019ve even overheard cruel remarks in a language people assumed I didn\u2019t understand. Those moments left wounds far deeper than any heat rash or bruise from too-small chairs.<\/p>\n<p>Society\u2019s obsession with fatness extends well beyond personal interactions. Fat people are <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9125650\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"consistently portrayed in the media\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9125650\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"2\" rel=\"nofollow noopener\" target=\"_blank\">consistently portrayed in the media<\/a> as lazy, undisciplined or morally flawed. <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1038\/oby.2001.108\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"Research has documented\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1038\/oby.2001.108\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"3\" rel=\"nofollow noopener\" target=\"_blank\">Research has documented<\/a> how these stereotypes influence health care practices and workplace discrimination. This cultural narrative seeps into medicine, where clinicians \u2014 even those with good intentions \u2014 can unconsciously perpetuate stigma.<\/p>\n<p>Today, at 44, I\u2019m on Wegovy. Reluctantly. When I started, I told my doctor my goal weight was 250 pounds \u2014 a number that, at 5\u201910\u201d, felt right to me.<\/p>\n<p>Now I weigh 235, down 50 pounds in eight months. My doctor tells me to stay on it. And I\u2019m not sure why I\u2019m listening.<\/p>\n<p>Initially approved to treat Type 2 diabetes, GLP-1 medications like Wegovy are now viewed as a <a href=\"https:\/\/www.news-medical.net\/health\/Wegovy-A-Breakthrough-in-Weight-Loss.aspx\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"breakthrough\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/www.news-medical.net\/health\/Wegovy-A-Breakthrough-in-Weight-Loss.aspx\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"4\" rel=\"nofollow noopener\" target=\"_blank\">breakthrough<\/a> for weight loss, but for me, they feel like another chapter in the same story: doctors offering interventions driven less by my actual health and more by the fear and stigma that surround fatness.<\/p>\n<p>Why do I take it? Because sometimes it feels easier to live with the physical and emotional side effects of <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/B9780080970868640205\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"medicalization\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/B9780080970868640205\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"5\" rel=\"nofollow noopener\" target=\"_blank\">medicalization<\/a> than with the relentless judgment of others \u2014 and, most troubling of all for me, with my own internalized fatphobia.<\/p>\n<p>Here\u2019s the truth: Fat patients aren\u2019t blank slates waiting for salvation. We know the statistics, the risks, the medical language. Weight bias, <a href=\"https:\/\/bmcmedicine.biomedcentral.com\/articles\/10.1186\/s12916-018-1116-5\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"it has been argued\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/bmcmedicine.biomedcentral.com\/articles\/10.1186\/s12916-018-1116-5\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"7\" rel=\"nofollow noopener\" target=\"_blank\">it has been argued<\/a>, can also trigger stress responses, raise blood pressure and worsen metabolic markers. In other words, the judgment itself is harming our health.<\/p>\n<p>And let us not forget that the use of GLP-1s can come with a whole new set of harms in the form of extreme side effects like <a href=\"https:\/\/www.healthline.com\/health\/drugs\/wegovy-side-effects#serious-side-effects\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"chronic diarrhea, severe nausea and pancreatitis\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/www.healthline.com\/health\/drugs\/wegovy-side-effects#serious-side-effects\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"8\" rel=\"nofollow noopener\" target=\"_blank\">chronic diarrhea, severe nausea and pancreatitis<\/a>, all <a href=\"https:\/\/asmbs.org\/resources\/bariatric-surgery-postoperative-concerns\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"familiar side effects\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/asmbs.org\/resources\/bariatric-surgery-postoperative-concerns\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"9\" rel=\"nofollow noopener\" target=\"_blank\">familiar side effects<\/a> to those, like me, who previously underwent weight loss surgery that permanently altered our digestive systems.<\/p>\n<p>While GLP-1s often lead to weight loss, <a href=\"https:\/\/bmcmedicine.biomedcentral.com\/articles\/10.1186\/s12916-025-04200-0\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"research shows\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/bmcmedicine.biomedcentral.com\/articles\/10.1186\/s12916-025-04200-0\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"10\" rel=\"nofollow noopener\" target=\"_blank\">research shows<\/a> that the weight comes back when the medication is stopped. Similarly, <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9906605\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"long-term studies\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9906605\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"11\" rel=\"nofollow noopener\" target=\"_blank\">long-term studies<\/a> on weight loss surgery reveal that many patients eventually regain a significant portion of the weight they lost. This cycle of losing and regaining weight \u2014 often called weight cycling \u2014 has been linked to negative health outcomes, including increased risk of <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6489475\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"heart disease, high blood pressure and metabolic issues\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6489475\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"12\" rel=\"nofollow noopener\" target=\"_blank\">heart disease, high blood pressure and metabolic issues<\/a>, some of the very conditions weight loss interventions are intended to prevent. But if we don\u2019t confront the bias that underpins it \u2014 the reflexive assumption that fat equals sick \u2014 we will keep reproducing the same harm.<\/p>\n<p>Medical education is part of the problem. Physicians-in-training <a href=\"https:\/\/psycnet.apa.org\/record\/2013-23412-010\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"often harbor implicit anti-fat biases and receive minimal guidance\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/psycnet.apa.org\/record\/2013-23412-010\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"15\" rel=\"nofollow noopener\" target=\"_blank\">often harbor implicit anti-fat biases and receive minimal guidance<\/a> on treating patients with respect regardless of weight. Moreover, BMI \u2014 a tool used ubiquitously \u2014 is a <a href=\"https:\/\/www.npr.org\/2009\/07\/04\/106268439\/top-10-reasons-why-the-bmi-is-bogus\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"flawed instrument\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/www.npr.org\/2009\/07\/04\/106268439\/top-10-reasons-why-the-bmi-is-bogus\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"16\" rel=\"nofollow noopener\" target=\"_blank\">flawed instrument<\/a>. It correlates poorly with health outcomes when considered alone, and often oversimplifies complex metabolic and social factors. <\/p>\n<p>Although many people assume that a fat body is inherently unhealthy, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29998631\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"research shows\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29998631\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"17\" rel=\"nofollow noopener\" target=\"_blank\">research shows<\/a> that body size by itself isn\u2019t a reliable predictor of mortality risk. This is especially the case when other metabolic risk factors, like high blood pressure, diabetes or high cholesterol, are absent. Exercise physiologist Glenn Gaesser has long emphasized that <a href=\"https:\/\/turnerpublishing.com\/products\/big-fat-lies-the-truth-about-your-weight-and-your-health?srsltid=AfmBOor8CwVmQ-7vS8-NV6xxpgVHrs7bDRW-K4HFnv8svMTgTJlIMpbz\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"it\u2019s fitness, not fatness\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/turnerpublishing.com\/products\/big-fat-lies-the-truth-about-your-weight-and-your-health?srsltid=AfmBOor8CwVmQ-7vS8-NV6xxpgVHrs7bDRW-K4HFnv8svMTgTJlIMpbz\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"18\" rel=\"nofollow noopener\" target=\"_blank\">it\u2019s fitness, not fatness<\/a> that better predicts long-term health outcomes. And the two are not directly correlated \u2014 fat people can still be fit and healthy, just as thin people can be unhealthy.<\/p>\n<p>We must also not forget that fatphobia is rooted in racism, as sociologist Sabrina Strings outlines in \u201c<a href=\"https:\/\/nyupress.org\/9781479886753\/fearing-the-black-body\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"Fearing the Black Body: The Racial Origins of Fat Phobia\" data-vars-item-type=\"text\" data-vars-unit-name=\"68c45aece4b0b3dca95eb648\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/nyupress.org\/9781479886753\/fearing-the-black-body\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"19\" rel=\"nofollow noopener\" target=\"_blank\">Fearing the Black Body: The Racial Origins of Fat Phobia<\/a>.\u201d The earliest traces of fatphobia weren\u2019t about body size, but about white folks distancing themselves from Black people, specifically Black women. When medicine jumped onto the fatphobia bandwagon, the fear of Blackness morphed into an ideological fear of obesity, and false claims that body size and health are synonymous.<\/p>\n<p>It\u2019s time for change. Medical schools must teach that BMI is not a proxy for health. Doctors must be trained to treat fat patients as whole people, not as problems to solve. Health systems must recognize that weight stigma itself causes measurable harm, and implement policies to mitigate it. This means redesigning exam rooms with appropriate equipment, training staff on inclusive communication, and reevaluating the language used in medical records, prescriptions and treatment plans.<\/p>\n<p>From childhood to adulthood, I\u2019ve been told that \u201csomething must be done.\u201d Rarely has anyone stopped to ask what living in this body actually means to me. Rarely has anyone asked how the shame of constant surveillance affects my well-being. <\/p>\n<p>I may continue to take Wegovy, or I may not. What I do know for sure is that any decision I make about weight loss is shaped more by how society treats fat bodies than by concerns about my health. Fat people can be healthy. Fat people can be happy. But in a culture obsessed with shrinking our bodies, it\u2019s incredibly hard to believe that\u2019s true, let alone live as if it is. <\/p>\n<p>I am more than the size of my body. Every fat patient is.
