The cognitive domains most frequently affected in MS are speed of information processing, sustained attention, memory, and visuospatial perception [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 3\" title=\"Trenova AG, Slavov GS, Manova MG, Aksentieva JB, Miteva LD, Stanilova SA. Cognitive impairment in multiple sclerosis. Folia Med (Plovdiv). 2016;58:157\u201363.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR3\" id=\"ref-link-section-d24736918e2093\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>]. Cognitive impairment significantly affects patients\u2019 quality of life, social integration, and ability to work [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 18\" title=\"Yalachkov Y, Soyda\u015f D, Bergmann J, Frisch S, Behrens M, Foerch C, et al. Levels of anxiety and depression can have a negative impact on patients\u2019 cognitive performance, leading to poorer outcomes [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 8\" title=\"Margoni M, Preziosa P, Rocca MA, Filippi M. However, research has demonstrated that higher levels of cognitive reserve are linked to less cognitive impairment in pwMS [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 11\" title=\"Santangelo G, Altieri M, Enzinger C, Gallo A, Trojano L. Cognitive reserve and neuropsychological performance in multiple sclerosis: A meta-analysis. Neuropsychology. 2019;33:379\u201390.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR11\" id=\"ref-link-section-d24736918e2103\" rel=\"nofollow noopener\" target=\"_blank\">11<\/a>].<\/p>\n<p>The objective of this study was to investigate the relationship between depressive symptoms, CR, sustained attention, and semantic fluency in individuals with impairment. Our hypothesis was that CR predicts the relationship between depressive symptomatology and cognitive function. The main aim was to examine the role of CR as a moderator of the relationship between BDI, PASAT, and WLG.<\/p>\n<p>The findings suggest a consistent association between increased depressive symptomatology and lower performance in sustained attention, and semantic fluency.<\/p>\n<p>Depression affects over 20% of MS patients [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 19\" title=\"Marrie RA, Reingold S, Cohen J, Stuve O, Trojano M, Sorensen PS, et al. The incidence and prevalence of psychiatric disorders in multiple sclerosis: A systematic review. Mult Scler J. 2015;21:305\u201317.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR19\" id=\"ref-link-section-d24736918e2115\" rel=\"nofollow noopener\" target=\"_blank\">19<\/a>], leading to reduced quality of life, increased hospitalisation and mortality rates, and worse disability progression [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 20\" title=\"Janssens AC, van Doorn PA, de Boer JB, Kalkers NF, van der Mech\u00e9 FG, Passchier J, et al. A subsequent study revealed that symptoms of depression are more closely linked to cognitive efficiency, specifically speed, rather than memory [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 24\" title=\"Glukhovsky L, Kurz D, Brandstadter R, Leavitt VM, Krieger S, Fabian M, et al. Several studies have shown that pwMS with depression perform worse on the PASAT than non-depressed MS patients and normal control participants [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 25\" title=\"Arnett PA, Higginson CI, Voss WD, Wright B, Bender WI, Wurst JM, et al. A meta-analysis of 10 studies on MS patients with depression also found a correlation between major depression and lower PASAT scores [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 27\" title=\"Thornton AE, Raz N. Memory impairment in multiple sclerosis: A quantitative review. Neuropsychology. 1997;11:357\u201366.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR27\" id=\"ref-link-section-d24736918e2140\" rel=\"nofollow noopener\" target=\"_blank\">27<\/a>]. It has been widely demonstrated that high levels of depression are associated with worse performance on verbal semantic fluency tests [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 28\" title=\"Altieri M, Cerciello F, Gallo A, Santangelo G. The relationship between depression and cognitive performance in multiple sclerosis: A meta-analysis. Clin Neuropsychol. 2024;38:21\u201341.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR28\" id=\"ref-link-section-d24736918e2143\" rel=\"nofollow noopener\" target=\"_blank\">28<\/a>].<\/p>\n<p>However, the importance of CR as a predictor of cognitive functioning in persons with disabilities was reiterated [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 11\" title=\"Santangelo G, Altieri M, Enzinger C, Gallo A, Trojano L. Cognitive reserve and neuropsychological performance in multiple sclerosis: A meta-analysis. Neuropsychology. 2019;33:379\u201390.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR11\" id=\"ref-link-section-d24736918e2150\" rel=\"nofollow noopener\" target=\"_blank\">11<\/a>]. Studies have indicated how CR predicts performance in tests of processing speed and memory [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 29\" title=\"Sumowski JF, Wylie GR, Chiaravalloti N, DeLuca J. Intellectual enrichment lessens the effect of brain atrophy on learning and memory in multiple sclerosis. Neurology. 2010;74:1942\u20135.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR29\" id=\"ref-link-section-d24736918e2153\" rel=\"nofollow noopener\" target=\"_blank\">29<\/a>]. However, it has also been shown the central role played by CR on all aspects of cognition such as executive functions, visuospatial processing and the various cognitive indices measured by the verbal fluency test [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 30\" title=\"Patel VP, Walker LA, Feinstein A. Revisiting cognitive reserve and cognition in multiple sclerosis: A closer look at depression. Mult Scler J. 2018;24:186\u201395.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR30\" id=\"ref-link-section-d24736918e2156\" rel=\"nofollow noopener\" target=\"_blank\">30<\/a>].<\/p>\n<p>To understand whether higher levels of cognitive reserve (CR) could influence the relationship between cognitive performance and depressive symptoms, we conducted a mediation analysis in our study. The analysis considered CR as a potential moderator in the model, based on the direct correlations observed. The results of the mediation analyses indicate that depressive symptoms did not have a direct negative impact on performance in working memory, sustained attention, and verbal fluency. Instead, cognitive reserve (CR) played a protective role in this relationship, resulting in better performance.<\/p>\n<p>Previous research has investigated the relationship between cognitive reserve (CR) and depression. It has been suggested that CR may have a protective effect against the negative impact of increased neurological disability on an individual\u2019s level of depression [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 31\" title=\"Cadden MH, Guty ET, Arnett PA. Cognitive reserve attenuates the effect of disability on depression in multiple sclerosis. Arch Clin Neuropsychol. 2019;34:495\u2013502.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR31\" id=\"ref-link-section-d24736918e2165\" rel=\"nofollow noopener\" target=\"_blank\">31<\/a>].<\/p>\n<p>Additionally, the correlation between the CRIq score and cognitive performance suggests that patients with higher CRIq scores exhibit better information processing speed, as well as verbal and visuospatial memory performance, compared to those with lower CRIq scores [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 10\" title=\"Santangelo G, Altieri M, Gallo A, Trojano L. Cognitive function is differentially protected by CR [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 32\" title=\"Ifantopoulou P, Artemiadis AK, Bakirtzis C, Zekiou K, Papadopoulos T-S, Diakogiannis I, et al. High CR predicts the preservation of functional brain connectivity despite grey matter atrophy and attenuates the detrimental effect of white matter alteration on information processing speed in MS patients [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 33\" title=\"Fuchs TA, Benedict RHB, Bartnik A, Choudhery S, Li X, Mallory M, et al. Preserved network functional connectivity underlies cognitive reserve in multiple sclerosis. Hum Brain Mapp. 2019;40:5231\u201341.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR33\" id=\"ref-link-section-d24736918e2177\" rel=\"nofollow noopener\" target=\"_blank\">33<\/a>]. In another MS study, high CR was found to selectively protect against the effect of peripheral grey matter atrophy on verbal memory [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 34\" title=\"Artemiadis A, Bakirtzis C, Ifantopoulou P, Zis P, Bargiotas P, Grigoriadis N, et al. The role of cognitive reserve in multiple sclerosis: A cross-sectional study in 526 patients. Mult Scler Relat Disord. 2020;41:102047.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR34\" id=\"ref-link-section-d24736918e2180\" rel=\"nofollow noopener\" target=\"_blank\">34<\/a>].<\/p>\n<p>Previous studies have examined the protective role of CR on cognitive function in patients without multiple sclerosis, but none have specifically focused on the moderating role of CR in the relationship between cognition and depression. Wight and colleagues [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 35\" title=\"Wight RG, Aneshensel CS, Seeman TE. Educational attainment, continued learning experience, and cognitive function among older men. J Aging Health. 2002;14:211\u201336.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR35\" id=\"ref-link-section-d24736918e2186\" rel=\"nofollow noopener\" target=\"_blank\">35<\/a>] investigated the impact of education, post-school training, and occupation on cognitive function in a sample of older men living in the community. The study found that a higher level of education moderated the effect of depressive symptoms on cognition, but only when the symptoms were more severe. Bhalla et al. [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 36\" title=\"Bhalla RK, Butters MA, Zmuda MD, Seligman K, Mulsant BH, Pollock BG, et al. Does education moderate neuropsychological impairment in late-life depression? Int J Geriatr Psychiatry. 2005;20:413\u20137.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR36\" id=\"ref-link-section-d24736918e2189\" rel=\"nofollow noopener\" target=\"_blank\">36<\/a>] found no moderating effect of education level on neuropsychological performance in a sample of depressed patients. In a study designed to examine the mediation of cognitive reserve in the relationship between cognitive performance and late-life depression, subjects with higher levels of education and\/or higher reading scores showed greater declines in cognitive performance with increasing depressive symptoms than those with lower levels of education or reading scores in the reference sample [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 37\" title=\"O\u2019Shea DM, Fieo RA, Hamilton JL, Zahodne LB, Manly JJ, Stern Y. Examining the association between late-life depressive symptoms, cognitive function, and brain volumes in the context of cognitive reserve. Int J Geriatr Psychiatry. 2015;30:614\u201322.\" href=\"http:\/\/bmcpsychology.biomedcentral.com\/articles\/10.1186\/s40359-025-03162-5#ref-CR37\" id=\"ref-link-section-d24736918e2192\" rel=\"nofollow noopener\" target=\"_blank\">37<\/a>]. In agreement with the literature, we also found better performance on cognitive tasks in our study in the presence of higher levels of cognitive reserve, which acts as a protective factor when higher levels of depressive symptoms are registered. While the effect size of -0.044 might be considered weak according to Cohen\u2019s standards, it is important to note that even small effects can have substantial clinical implications for MS patients. The attenuation of cognitive reserve benefits by depressive symptoms highlights the need for integrated therapeutic approaches that address both cognitive and emotional health in MS care.<\/p>\n<p>A few limitations must be considered when interpreting our results. The cross-sectional design, utilizing self-reported measures of CR and neuropsychiatric symptoms, was performed at a single center with a relatively small patient sample and a limited neuropsychological battery. It would be interesting to observe changes in cognitive profiles in larger cohorts over long-term follow-up with a more comprehensive neuropsychological evaluation (including the SDMT and other relevant tests), which could serve as a future goal for this preliminary cross-sectional study. Our findings are specific to the RR-MS population and may not be generalizable to patients with secondary progressive MS (SP-MS). The relatively low percentage of patients on DMTs highlights the variability in treatment practices and patient circumstances. Further research could explore the impact of cognitive reserve and depressive symptoms in other MS subtypes, including SPMS.<\/p>\n<p>Additionally, recognizing the potential limitation of including only newly diagnosed PwMS, who may exhibit higher levels of depression or increased depressive symptoms, future research should compare this group with patients diagnosed for at least two years. This comparison will help clarify the impact of disease duration on depression and cognitive functions.<\/p>\n<p>Finally, another limitation of this study is the absence of specific inclusion criteria for anxiety, fatigue, and sleep quality, despite their known effects on depression in people with disabilities. The sole reliance on the BDI is an additional limitation, as depression can be a secondary symptom in individuals with multiple sclerosis, complicating the relationship between depression and cognitive performance.
