Pharmacists offer tips that could reduce your out-of-pocket drug costs"},"content":{"rendered":"<p>Even when Americans have health insurance, they can have a hard time <a href=\"https:\/\/www.healthsystemtracker.org\/chart-collection\/recent-forecasted-trends-prescription-drug-spending\/\" rel=\"nofollow noopener\" target=\"_blank\">affording the drugs they\u2019ve been prescribed<\/a>.<\/p>\n<p>About 1 in 5 U.S. adults skip filling a prescription due to its cost at least once a year, <a href=\"https:\/\/www.kff.org\/health-costs\/americans-challenges-with-health-care-costs\/\" rel=\"nofollow noopener\" target=\"_blank\">according to KFF<\/a>, a health research organization. And 1 in 3 take steps to cut their prescription drug costs, such as splitting pills when it\u2019s not medically necessary or switching to an over-the-counter drug instead of the one that their medical provider prescribed.<\/p>\n<p>As <a href=\"https:\/\/olemiss.edu\/profiles\/sramacha\" rel=\"nofollow noopener\" target=\"_blank\">pharmacy professors<\/a> who <a href=\"https:\/\/olemiss.edu\/profiles\/anpate.php\" rel=\"nofollow noopener\" target=\"_blank\">research prescription drug access<\/a>, we think it\u2019s important for Americans to know that it is possible to get prescriptions filled more affordably, as long as you know how before you go to the pharmacy.<\/p>\n<p>Cost of copays ranges widely<\/p>\n<p>When you have health insurance and have to pay for a prescription drug at the pharmacy, you\u2019re usually <a href=\"https:\/\/www.healthcare.gov\/glossary\/co-payment\/\" rel=\"nofollow noopener\" target=\"_blank\">covering the cost of your copay<\/a>. This is the amount patients or their caregivers are expected to pay after insurance covers the rest of the tab.<\/p>\n<p>If you get your <a href=\"https:\/\/theconversation.com\/a-brief-history-of-medicaid-and-americas-long-struggle-to-establish-a-health-care-safety-net-251776\" rel=\"nofollow noopener\" target=\"_blank\">health insurance through Medicaid<\/a>, the government program that covers low-income Americans and people with disabilities, you should not have to pay anything at all to obtain prescription drugs. If there is a copay, it should be low \u2013 probably less than US$5. <\/p>\n<p>And if you\u2019re <a href=\"https:\/\/theconversation.com\/medicare-advantage-is-covering-more-and-more-americans-some-because-they-dont-get-to-choose-251796\" rel=\"nofollow noopener\" target=\"_blank\">insured through Medicare<\/a>, the government program that mainly covers people who are 65 and older, or get your coverage through a private health insurance company, it\u2019s important to understand what to expect when you visit a pharmacy.<\/p>\n<p>Most <a href=\"https:\/\/insuredandmore.com\/what-is-the-average-out-of-pocket-cost-for-prescription-drugs\" rel=\"nofollow noopener\" target=\"_blank\">private insurance companies charge US$5 to $50<\/a> for prescription drug copays. The copays are tiered based on what the drug costs. Brand-name and specialty medications have higher copays; older generics have lower copays. <\/p>\n<p>Some generic drugs and vaccines may even require no copay at all. While a copay is a flat fee, it can change over the course of the year based on whether or not you have met your deductible. The deductible is the amount of money you have to pay out of pocket before your insurance starts covering your prescriptions. Before your deductible is fully paid, you may be responsible for the full cost of your medications. After you\u2019ve met your deductible for the year, you will only be required to pay the copay.<\/p>\n<p>As newer, more expensive drugs enter the market, cost-sharing at the pharmacy has increasingly <a href=\"https:\/\/www.dol.gov\/sites\/dolgov\/files\/ebsa\/laws-and-regulations\/laws\/no-surprises-act\/2024-report-to-congress-prescription-drug-spending.pdf\" rel=\"nofollow noopener\" target=\"_blank\">shifted from a copay to coinsurance<\/a>. <\/p>\n<p>In contrast with a flat copay, coinsurance means your insurance company will <a href=\"https:\/\/www.healthcare.gov\/glossary\/co-insurance\/\" rel=\"nofollow noopener\" target=\"_blank\">cover a certain percentage<\/a> of the drug\u2019s cost, and you\u2019ll pay the rest. Since the patient\u2019s share is based on a percentage of the medication\u2019s price, coinsurance often results in <a href=\"https:\/\/phrma.org\/blog\/commercial-health-plans-transition-from-copays-to-coinsurance-and-deductibles-has-increased-patient-out-of-pocket-costs-for-brand-medicines\" rel=\"nofollow noopener\" target=\"_blank\">higher out-of-pocket costs than copays do<\/a>. <\/p>\n<p>New help for patients with Medicare coverage<\/p>\n<p>Two new <a href=\"https:\/\/www.kff.org\/medicare\/explaining-the-prescription-drug-provisions-in-the-inflation-reduction-act\/\" rel=\"nofollow noopener\" target=\"_blank\">government programs<\/a> could help make prescription drugs more affordable for millions of older Americans.<\/p>\n<p>Starting in 2026, people who are insured through Medicare <a href=\"https:\/\/www.panfoundation.org\/understanding-the-medicare-part-d-cap\/\" rel=\"nofollow noopener\" target=\"_blank\">will pay no more than $2,100<\/a> out of pocket on prescription drugs over the year. That cap may be much lower than $2,100 due to a quirk in Medicare\u2019s rules. Prescriptions filled after someone has paid the maximum allowable amount will cost them nothing at all.<\/p>\n<p>In addition, the government launched the <a href=\"https:\/\/www.medicare.gov\/publications\/12211-whats-the-medicare-prescription-payment-plan.pdf\" rel=\"nofollow noopener\" target=\"_blank\">Medicare Prescription Payment Plan<\/a> in 2025. This program, which is available to people over 65, helps spread what patients spend out of pocket on prescription drugs throughout the year, making that expense more predictable and easier to budget for.<\/p>\n<p>Early data indicates that <a href=\"https:\/\/advisory.avalerehealth.com\/insights\/early-enrollment-data-indicates-more-beneficiaries-could-benefit-from-mppp\" rel=\"nofollow noopener\" target=\"_blank\">very few Americans are enrolled<\/a> in the Medicare Prescription Payment Plan. Patients insured through private companies do not have similar opportunities.<\/p>\n<\/p>\n<p>            Consumers should find out if they qualify for state or federal programs on their medications.<\/p>\n<p>Coupons and discount cards<\/p>\n<p>What if you can\u2019t afford a copay for your prescription drug?<\/p>\n<p>Before giving up on ever getting it, ask the pharmacist about your options.<\/p>\n<p>It may be worth trying to use a free online tool, <a href=\"https:\/\/www.rxassist.org\/\" rel=\"nofollow noopener\" target=\"_blank\">such as RxAssist<\/a>, sponsored by the Robert Wood Johnson Foundation, or a <a href=\"https:\/\/www.goodrx.com\/\" rel=\"nofollow noopener\" target=\"_blank\">discount card from GoodRx<\/a>, which is a publicly traded company.<\/p>\n<p>GoodRx cards are free. They help people compare local pharmacy prices and to locate coupons that <a href=\"https:\/\/www.goodrx.com\/corporate\/business\/the-goodrx-effect-how-goodrx-is-changing-the-economics-of-healthcare\" rel=\"nofollow noopener\" target=\"_blank\">make prescriptions more affordable<\/a>.<\/p>\n<p>GoodRx works by searching for the lowest available price for the prescription at various pharmacies. Other copay coupons provided by the drug manufacturer may also work similarly by lowering the cost of the medication. On some occasions, the cash price at the pharmacy may actually be cheaper than the copay, and the pharmacist should be able to help you navigate these options.<\/p>\n<p>Here\u2019s what you should know before giving GoodRx a try:<\/p>\n<p>GoodRx collects individual data on patients, <a href=\"https:\/\/www.ftc.gov\/news-events\/news\/press-releases\/2023\/02\/ftc-enforcement-action-bar-goodrx-sharing-consumers-sensitive-health-info-advertising\" rel=\"nofollow noopener\" target=\"_blank\">raising significant privacy concerns<\/a>.<\/p>\n<p>Some pharmacies <a href=\"https:\/\/www.drugchannels.net\/2020\/08\/how-goodrx-profits-from-our-broken.html\" rel=\"nofollow noopener\" target=\"_blank\">do not accept GoodRx<\/a>. You may have to visit more than one pharmacy to be able to activate its discounts.<\/p>\n<p>These cards may make the most sense for uninsured or underinsured patients, but do not always help those who have insurance because you might not get a better price. What\u2019s more, if you use a discount card, the amount you pay may not count toward your insurance deductible for the year.<\/p>\n<p>You should weigh the caveats closely depending on your circumstance.<\/p>\n<p>            <a href=\"https:\/\/images.theconversation.com\/files\/706837\/original\/file-20251207-56-1yh0ef.jpg?ixlib=rb-4.1.0&amp;q=45&amp;auto=format&amp;w=1000&amp;fit=clip\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" alt=\"A male pharmacist scanning a pharmacy product for his customer.\" src=\"https:\/\/www.newsbeep.com\/au\/wp-content\/uploads\/2025\/12\/file-20251207-56-1yh0ef.jpg\" class=\"native-lazy\" loading=\"lazy\"  \/><\/a><\/p>\n<p>              Your pharmacist can help you navigate the various discount offerings.<br \/>\n              <a class=\"source\" href=\"https:\/\/www.gettyimages.com\/detail\/photo\/an-asian-chinese-male-pharmacist-scanning-pharmacy-royalty-free-image\/1214910646?phrase=prescription%20drug%20costs&amp;searchscope=image%2Cfilm&amp;adppopup=true\" rel=\"nofollow noopener\" target=\"_blank\">CG Tan\/E+ via Getty Images<\/a><\/p>\n<p>Prescription assistance programs<\/p>\n<p><a href=\"https:\/\/www.novaregion.org\/844\/PAPs-Prescription-Assistance-Programs\" rel=\"nofollow noopener\" target=\"_blank\">Prescription assistance programs<\/a> provide another cost-saving tool for Americans.<\/p>\n<p>Drugmakers, nonprofits and government agencies sponsor those programs, which help patients who are uninsured or underinsured \u2013 even if they are on Medicare \u2013 fill prescriptions either at a discount or for free.<\/p>\n<p>These programs include manufacturer-specific programs as well as charitable pharmacies like <a href=\"https:\/\/www.dispensaryofhope.org\/\" rel=\"nofollow noopener\" target=\"_blank\">Dispensary of Hope<\/a>, <a href=\"https:\/\/novascriptscentral.org\/medhelp\" rel=\"nofollow noopener\" target=\"_blank\">NOVA Scripts Central<\/a> and the <a href=\"https:\/\/www.patientadvocate.org\/connect-with-services\/\" rel=\"nofollow noopener\" target=\"_blank\">Patient Advocate Foundation<\/a>. Qualifying criteria vary for these programs, but typically you must have a low income and be a citizen or a legal U.S. resident.<\/p>\n<p>The <a href=\"https:\/\/www.panfoundation.org\/\" rel=\"nofollow noopener\" target=\"_blank\">Patient Access Network Foundation<\/a> and <a href=\"https:\/\/www.rxassist.org\/\" rel=\"nofollow noopener\" target=\"_blank\">RxAssist<\/a>, two nonprofits that help Americans pay their medical bills, also offer helpful tools to identify programs that could work for you.<\/p>\n<p>Assistance from these programs could cut your copay or even provide a prescription drug at no cost.<\/p>\n<p>Separately, the Trump administration announced in November 2025 that a new White House prescription drug pricing program will soon begin to connect consumers to companies that have agreed to sell certain prescription drugs at a big discount.<\/p>\n<p>Many experts don\u2019t expect the program, <a href=\"https:\/\/www.whitehouse.gov\/fact-sheets\/2025\/11\/fact-sheet-president-donald-j-trump-announces-major-developments-in-bringing-most-favored-nation-pricing-to-american-patients\/\" rel=\"nofollow noopener\" target=\"_blank\">known as TrumpRx<\/a>, to help people who have health insurance. Instead, it could be most likely to <a href=\"https:\/\/www.healthline.com\/health-news\/trump-rx-lower-costs-glp-1s-prescription-drugs\" rel=\"nofollow noopener\" target=\"_blank\">help those with no insurance at all<\/a>. The new government program is slated to begin to roll out in 2026.<\/p>\n<p>Direct-to-consumer models<\/p>\n<p>Beyond coupons and assistance programs, a more radical shift is in the works: <a href=\"https:\/\/www.statnews.com\/2025\/08\/19\/direct-to-consumer-drug-costs-pharma-analysis\/\" rel=\"nofollow noopener\" target=\"_blank\">direct-to-consumer platforms<\/a> and cash-payment models.<\/p>\n<p>In 2025, several manufacturers offered to sell medications directly to patients on websites and patient portals at cash prices. For example, the <a href=\"https:\/\/www.cnbc.com\/2025\/12\/01\/eli-lilly-prices-zepbound-weight-loss-drug-vials.html\" rel=\"nofollow noopener\" target=\"_blank\">drug manufacturer Eli Lilly is offering<\/a> its popular weight-loss medication, Zepbound, <a href=\"https:\/\/www.lilly.com\/lillydirect\/medicines\/zepbound\" rel=\"nofollow noopener\" target=\"_blank\">on its website<\/a>.<\/p>\n<p>These websites have out-of-pocket costs that can run upward of <a href=\"https:\/\/www.cnbc.com\/2025\/12\/01\/eli-lilly-prices-zepbound-weight-loss-drug-vials.html\" rel=\"nofollow noopener\" target=\"_blank\">$300 a month<\/a>, making them too high for many, if not most, Americans to afford. And insurance companies have so far refused to cover them.<\/p>\n<p>To be sure, the systems underlying these programs are still being built.
