{"id":354783,"date":"2025-12-17T22:20:09","date_gmt":"2025-12-17T22:20:09","guid":{"rendered":"https:\/\/www.newsbeep.com\/au\/354783\/"},"modified":"2025-12-17T22:20:09","modified_gmt":"2025-12-17T22:20:09","slug":"the-us-already-faces-a-health-care-workforce-shortage-immigration-policy-could-make-it-worse","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/au\/354783\/","title":{"rendered":"The US already faces a health care workforce shortage \u2013 immigration policy could make it worse"},"content":{"rendered":"<p>As Americans gather for holiday celebrations, many will quietly thank the health care workers who keep their families and friends well: the ICU nurse who stabilized a grandparent, the doctor who adjusted a tricky prescription, the home health aide who ensures an aging relative can bathe and eat safely. <\/p>\n<p>Far fewer may notice how many of these <a href=\"https:\/\/www.kff.org\/racial-equity-and-health-policy\/what-role-do-immigrants-play-in-the-hospital-workforce\" rel=\"nofollow noopener\" target=\"_blank\">professionals are foreign-born<\/a>, and how <a href=\"https:\/\/usahello.org\/2025-immigration-policies\/\" rel=\"nofollow noopener\" target=\"_blank\">immigration policies<\/a> <a href=\"https:\/\/www.reuters.com\/business\/healthcare-pharmaceuticals\/trumps-h-1b-visa-fee-increase-raises-us-doctor-shortage-concerns-2025-09-24\/\" rel=\"nofollow noopener\" target=\"_blank\">shaped <\/a>in Washington today could determine whether those same families <a href=\"https:\/\/doi.org\/10.1016\/j.jebo.2024.106682\" rel=\"nofollow noopener\" target=\"_blank\">can get care<\/a> when they need it in the future.<\/p>\n<p>As <a href=\"https:\/\/lsbe.d.umn.edu\/faculty-staff\/bedassa-tadesse-phd\" rel=\"nofollow noopener\" target=\"_blank\">an economist<\/a> who studies how immigration influences economies, including health care systems, I see a consistent picture: Immigrants are a vital part of the health care workforce, especially in roles facing staffing shortages.<\/p>\n<p>Yet current immigration policies, such as <a href=\"https:\/\/www.reuters.com\/business\/healthcare-pharmaceuticals\/trumps-h-1b-visa-fee-increase-raises-us-doctor-shortage-concerns-2025-09-24\/\" rel=\"nofollow noopener\" target=\"_blank\">increased visa fees<\/a>, <a href=\"https:\/\/kffhealthnews.org\/news\/article\/visa-public-charge-health-conditions-trump-state-department\/\" rel=\"nofollow noopener\" target=\"_blank\">stricter eligibility requirements<\/a> and enforcement actions that affect legally present workers <a href=\"https:\/\/www.vasquezlawnc.com\/blog\/latest-trends-in-u-s-immigration-law-in-2025-key-policy-shifts-enforcement-patterns-and-what-they-mean-for-you\" rel=\"nofollow noopener\" target=\"_blank\">living with undocumented family members<\/a>, risk eroding this critical workforce, threatening timely care for millions of Americans. The timing couldn\u2019t be worse.<\/p>\n<p>A perfect storm: Rising demand, looming shortages<\/p>\n<p>America\u2019s health care system is entering an unprecedented period of strain. An <a href=\"https:\/\/www.pewresearch.org\/short-reads\/2024\/01\/09\/us-centenarian-population-is-projected-to-quadruple-over-the-next-30-years\/\" rel=\"nofollow noopener\" target=\"_blank\">aging population<\/a>, coupled with rising rates of <a href=\"https:\/\/nam.edu\/product\/health-basics-chronic-disease\/\" rel=\"nofollow noopener\" target=\"_blank\">chronic conditions<\/a>, is driving demand for care to <a href=\"https:\/\/www.nature.com\/articles\/s41514-024-00148-2\" rel=\"nofollow noopener\" target=\"_blank\">new heights<\/a>.<\/p>\n<p>The workforce isn\u2019t growing fast enough to meet those needs. The U.S. faces a projected <a href=\"https:\/\/www.aamc.org\/news\/press-releases\/new-aamc-report-shows-continuing-projected-physician-shortage\" rel=\"nofollow noopener\" target=\"_blank\">shortfall of up to 86,000 physicians<\/a> by 2036. Hospitals, clinics and elder-care services are expected to add <a href=\"https:\/\/www.bls.gov\/news.release\/archives\/ecopro_09062023.pdf?\" rel=\"nofollow noopener\" target=\"_blank\">about 2.1 million jobs between 2022 and 2032<\/a>. Many of those will be <a href=\"https:\/\/www.bls.gov\/ooh\/healthcare\/home-health-aides-and-personal-care-aides.htm\" rel=\"nofollow noopener\" target=\"_blank\">front-line caregiving roles<\/a>: home health, personal care and nursing assistants.<\/p>\n<p>For decades, immigrant health care workers have filled gaps where U.S.-born workers are limited. They serve as <a href=\"https:\/\/www.washingtonpost.com\/health\/2025\/12\/08\/h1b-visa-fees-rural-health\/\" rel=\"nofollow noopener\" target=\"_blank\">doctors in rural clinics<\/a>, <a href=\"https:\/\/www.kff.org\/racial-equity-and-health-policy\/the-growing-role-of-foreign-educated-nurses-in-u-s-hospitals-and-implications-of-visa-restrictions\/\" rel=\"nofollow noopener\" target=\"_blank\">nurses in understaffed hospitals<\/a> and <a href=\"https:\/\/www.kff.org\/medicaid\/what-role-do-immigrants-play-in-the-direct-long-term-care-workforce\/\" rel=\"nofollow noopener\" target=\"_blank\">aides in nursing homes and home care settings<\/a>.<\/p>\n<p>Nationally, immigrants make up <a href=\"https:\/\/kffhealthnews.org\/news\/article\/health-care-employment-growth-immigration-crackdown-medicaid-cuts\/\" rel=\"nofollow noopener\" target=\"_blank\">about 18% of the health care workforce<\/a>, and they\u2019re even more concentrated in critical roles. Roughly 1 in 4 physicians, 1 in 5 registered nurses and <a href=\"https:\/\/www.kff.org\/medicaid\/what-role-do-immigrants-play-in-the-direct-long-term-care-workforce\/\" rel=\"nofollow noopener\" target=\"_blank\">1 in 3 home health aides<\/a> are foreign-born.<\/p>\n<p>State-level data reveals just how deeply immigrants are embedded in the health care system. <a href=\"https:\/\/www.ppic.org\/blog\/immigrants-are-key-to-californias-health-workforce\" rel=\"nofollow noopener\" target=\"_blank\">Consider California<\/a>, where immigrants account for 1 in 3 physicians, 36% of registered nurses and 42% of health aides. On the other side of the country, immigrants make up 35% of hospital staff in New York state. In New York City, they are the majority of health care workers, representing <a href=\"https:\/\/www.steptwopolicy.org\/post\/the-importance-of-immigrants-in-new-york-s-healthcare-workforce\" rel=\"nofollow noopener\" target=\"_blank\">57% of the health care workforce<\/a>.<\/p>\n<p>Even in states with smaller immigrant populations, their impact is outsized.<\/p>\n<p>In Minnesota, immigrants are nearly <a href=\"https:\/\/www.minnpost.com\/greater-minnesota\/2025\/01\/staff-strapped-nursing-homes-look-to-new-americans-for-help\/\" rel=\"nofollow noopener\" target=\"_blank\">1 in 3 nursing assistants in nursing homes and home care agencies<\/a>, despite being just 12% of the overall workforce. Iowa, where immigrants are <a href=\"https:\/\/data.census.gov\/profile\/Iowa?g=040XX00US19\" rel=\"nofollow noopener\" target=\"_blank\">just 6.3% of the population<\/a>, <a href=\"https:\/\/www.wsj.com\/articles\/SB10001424053111903327904576526294052507136\" rel=\"nofollow noopener\" target=\"_blank\">relies on them<\/a> for a disproportionate share of rural physicians. <\/p>\n<\/p>\n<p>These patterns transcend geography and partisan divides. From urban hospitals to rural clinics, immigrants keep facilities operational. Policies that reduce their numbers \u2013 through higher visa fees, stricter eligibility requirements or increased deportations \u2013 have <a href=\"https:\/\/www.kff.org\/racial-equity-and-health-policy\/what-role-do-immigrants-play-in-the-hospital-workforce\/\" title=\"): [vacant shifts](https:\/\/www.axios.com\/2025\/06\/18\/trump-immigration-nursing-homes \" rel=\"nofollow noopener\" target=\"_blank\">ripple effects<\/a>, <a href=\"https:\/\/www.reuters.com\/business\/healthcare-pharmaceuticals\/trumps-h-1b-visa-fee-increase-raises-us-doctor-shortage-concerns-2025-09-24\/\" title=\") and [longer waits for routine care](https:\/\/doi.org\/10.1056\/NEJMp2504949 \" rel=\"nofollow noopener\" target=\"_blank\">closed hospital beds<\/a>.<\/p>\n<p>While health care demand soars, the pipeline for new health care workers could struggle to keep pace under current rules. Medical schools and nursing programs <a href=\"https:\/\/www.healthaffairs.org\/content\/forefront\/graduate-loan-caps-put-advanced-nursing-education-and-access-care-risk\" rel=\"nofollow noopener\" target=\"_blank\">face capacity limits<\/a>, and the time required to train new professionals \u2013 often <a href=\"https:\/\/www.aamc.org\/media\/75231\/download?attachment\" rel=\"nofollow noopener\" target=\"_blank\">a decade for doctors<\/a> \u2013 means that <a href=\"https:\/\/www.amga.org\/getmedia\/e195c030-f2a4-4919-8912-92329b007cc1\/2025-strengthening-gme-issue-brief.pdf\" rel=\"nofollow noopener\" target=\"_blank\">there aren\u2019t any quick fixes<\/a>.<\/p>\n<p>Immigrants have long bridged this gap \u2013 not just in clinical roles but in research and innovation. International students, who often pursue STEM and health-related fields at U.S. universities, are a key part of this pipeline. Yet recent surveys from the <a href=\"https:\/\/cgsnet.org\/data-infographics\/status-of-2025-international-student-admissions\" rel=\"nofollow noopener\" target=\"_blank\">Council of Graduate Schools show a sharp decline<\/a> in new international student enrollment for the 2025-26 academic year, driven partly by <a href=\"https:\/\/www.highereddive.com\/news\/new-international-enrollment-decline-fall-nafsa\/806482\/\" rel=\"nofollow noopener\" target=\"_blank\">visa uncertainties<\/a> and global talent competition.<\/p>\n<p>If this trend holds, the smaller cohorts arriving today will mean fewer physicians, nurses, biostatisticians and medical researchers in the coming decade \u2013 precisely when demand peaks. Although no major research organization has yet modeled the full impact that <a href=\"https:\/\/www.niskanencenter.org\/immigration-as-a-solution-to-healthcare-workforce-shortages\" rel=\"nofollow noopener\" target=\"_blank\">stricter immigration policies<\/a> could have on the health care workforce, <a href=\"https:\/\/doi.org\/10.1056\/NEJMp2504949\" rel=\"nofollow noopener\" target=\"_blank\">experts warn<\/a> that tighter visa rules, higher application fees and stepped-up enforcement are likely to intensify shortages, not ease them.<\/p>\n<p>These policies <a href=\"https:\/\/www.reuters.com\/business\/healthcare-pharmaceuticals\/trumps-h-1b-visa-fee-increase-raises-us-doctor-shortage-concerns-2025-09-24\/\" rel=\"nofollow noopener\" target=\"_blank\">make it harder to hire foreign-born workers<\/a> and create uncertainty for those already here. In turn, that complicates efforts to staff hospitals, clinics and long-term care facilities at a moment when the system can least afford additional strain.<\/p>\n<p>The hidden toll: Delayed care, rising risks<\/p>\n<p>Patients don\u2019t feel staffing gaps as statistics \u2013 they feel them physically.<\/p>\n<p>A specialist appointment delayed by months can mean worsening pain. Older adults without home care aides face higher risks of falls, malnutrition and medication errors. An understaffed nursing home turning away patients leaves families scrambling. These aren\u2019t hypotheticals \u2013 they\u2019re <a href=\"https:\/\/www.theguardian.com\/us-news\/2023\/nov\/07\/us-nursing-home-workers-face-catastrophic-crisis-understaffing\" rel=\"nofollow noopener\" target=\"_blank\">already happening<\/a> in <a href=\"https:\/\/www.statnews.com\/2023\/05\/02\/doctor-appointment-wait-times-solutions\/\" rel=\"nofollow noopener\" target=\"_blank\">pockets of the country<\/a> where <a href=\"https:\/\/www.ahcancal.org\/News-and-Communications\/Press-Releases\/Pages\/State-Of-The-Sector-Nursing-Home-Staffing-Shortages-Persist-Despite-Unprecedented-Efforts-To-Attract-More-Staff-.aspx\" rel=\"nofollow noopener\" target=\"_blank\">shortages are acute<\/a>.<\/p>\n<p>The costs of restrictive immigration policies won\u2019t appear in federal budgets but in human tolls: months spent with untreated depression, discomfort awaiting procedures, or preventable hospitalizations. Rural communities, often served by immigrant physicians, and urban nursing homes, reliant on immigrant aides, <a href=\"https:\/\/today.ucsd.edu\/story\/foreign-born-doctors-help-serve-rural-and-low-income-communities\" rel=\"nofollow noopener\" target=\"_blank\">will feel this most acutely<\/a>.<\/p>\n<p>Most Americans won\u2019t read a visa bulletin or a labor market forecast over holiday dinners. But they will notice when it becomes harder to get care for a child, a partner or an aging parent.<\/p>\n<p>Aligning immigration policy with the realities of the health care system will not, by itself, fix every problem in U.S. health care. But tightening the rules in the face of rising demand and known shortages almost guarantees more disruption.
