<\/p>\nThe father of a toddler who died after not receiving a blood test at a Perth hospital says a coronial inquest has vindicated the family’s claims that their pleas were ignored, but believes there is still a lack of accountability over his death.<\/p>\n
Sandipan Dhar died on March 24, 2024 at the Joondalup Health Campus after suffering from fevers for three weeks.<\/p>\n
His parents had taken him to the hospital’s emergency department twice in three days and said their requests for a blood test had been ignored.<\/p>\n
An autopsy revealed the 21-month-old boy had undiagnosed acute lymphoblastic leukaemia.<\/p>\n
Acting State Coroner Sarah Linton found there was a “missed opportunity” to complete a blood test and that Sandipan’s death was “probably preventable”.<\/p>\n

Sandipan’s father, Sanjoy Dhar, said the clinicians involved in his son’s care needed to be penalised.<\/p>\n
“We will never get back Sandipan. That incident totally destroyed our life,” he said.<\/p>\n
“Definitely, an example should [be] set.”<\/p>\n
Sandipan Dhar’s family want toddler’s death investigated further<\/a><\/p>\n The family of a toddler who died at a Perth hospital say they will consider taking legal action if their son’s death isn’t investigated further.<\/p>\n James Cafaro, the state manager of the hospital’s operator, Ramsay Health Care, said all involved clinicians had been referred to the Australian Health Practitioner Regulation Agency (AHPRA) for review.<\/p>\n “The external regulator placed no undertakings, restrictions or conditions on the registrations of these healthcare workers and the Coroner did not recommend that any action be taken against them,” Dr Cafaro said.\u00a0<\/p>\n “Our response reflects these assessments and is in alignment with the principles of a safe and just culture.”<\/p>\n ‘Someone should be held accountable’<\/p>\n Senior emergency department consultant Yii Siow told the court she had not read the letter from the family’s GP and was not made aware of any requests for a blood test in a handover note from a more junior doctor who had seen Sandipan earlier.<\/p>\n Ms Linton found there was an opportunity for Dr Siow, as an experienced clinician, to have identified that the toddler had a serious health issue.<\/p>\n “I consider it was incumbent upon Dr Siow to inform herself personally of the contents of the GP letter, which would have alerted her to the recommendation that a blood test be performed,” Ms Linton said.<\/p>\n Sandipan Dhar was suffering from leukaemia.\u00a0 (Supplied)<\/p>\n Mr Dhar said it was unacceptable that no one had been held accountable.<\/p>\n “We do not have any personal grievance to anyone, including Dr Siow, but Dr Siow made a mistake which she never acknowledged,” he said.<\/p>\n “I never said that I would like to see her behind jail \u2026 but she has to be penalised.”<\/p>\n Dr Cafaro said significant changes had been implemented since Sandipan’s death, and the hospital would implement all recommendations made by the coroner.<\/p>\n![\"A](\"https:\/\/www.newsbeep.com\/au\/wp-content\/uploads\/2026\/04\/6ddfac8b46be5611eaa7c4246e8330cd.jpeg\")
