Now it\u2019s back in Australia"},"content":{"rendered":"<p>The <a href=\"https:\/\/health.nt.gov.au\/__data\/assets\/pdf_file\/0006\/1604517\/health-alert-update-diphtheria-outbreak-in-the-nt-31-march.pdf\" rel=\"nofollow noopener\" target=\"_blank\">Northern Territory<\/a> and <a href=\"https:\/\/www.abc.net.au\/news\/2026-04-19\/kimberley-diphtheria-diagnosis-rates-raise-northern-wa\/106571924\" rel=\"nofollow noopener\" target=\"_blank\">Western Australia<\/a> are experiencing outbreaks of an almost-eradicated infection, diphtheria.<\/p>\n<p>The <a href=\"https:\/\/health.nt.gov.au\/__data\/assets\/pdf_file\/0006\/1604517\/health-alert-update-diphtheria-outbreak-in-the-nt-31-march.pdf\" rel=\"nofollow noopener\" target=\"_blank\">NT has recorded<\/a> 17 cases of respiratory diphtheria in the past month and<br \/>\n60 cases of the less serious cutaneous diphtheria, affecting the skin, in the past year.<\/p>\n<p>In the Kimberley region of WA, <a href=\"https:\/\/health.nt.gov.au\/__data\/assets\/pdf_file\/0006\/1604517\/health-alert-update-diphtheria-outbreak-in-the-nt-31-march.pdf\" rel=\"nofollow noopener\" target=\"_blank\">27 cases<\/a> have been reported in the past month alone, with one-quarter of those respiratory infections.<\/p>\n<p>Cases have also <a href=\"https:\/\/www.cdc.gov.au\/sites\/default\/files\/2026-04\/diphtheria-in-australia-epidemiological-update-14-april-2026.pdf\" rel=\"nofollow noopener\" target=\"_blank\">been found<\/a> in Queensland (two) and South Australia (one) this year.<\/p>\n<p>Historically, diphtheria was a leading cause of childhood death globally. More than 4,000 Australians died from diphtheria <a href=\"https:\/\/www1.health.gov.au\/internet\/main\/publishing.nsf\/Content\/cda-pubs-cdi-2000-cdi2406-cdi2406f.htm\" rel=\"nofollow noopener\" target=\"_blank\">between 1926 and 1935<\/a>. <\/p>\n<p>Vaccination started in Australia in the 1930s and the disease has <a href=\"https:\/\/www.aihw.gov.au\/getmedia\/f4c418f9-b4fe-4fb3-84e6-366d5098a8a0\/aihw-phe-236_diphtheria.pdf.aspx\" rel=\"nofollow noopener\" target=\"_blank\">rarely been seen<\/a> since the 1950s.<\/p>\n<p>But vaccine coverage has waned since the COVID pandemic, leading to a rising number of cases. <\/p>\n<p>How do you contract it? And how does it progress?<\/p>\n<p>Diphtheria is an infection <a href=\"https:\/\/www.cdc.gov.au\/diseases\/diphtheria#:%7E:text=It&#039;s%20caused%20by%20the%20toxin%2Dproducing%20bacteria%20Corynebacterium,lesions%20*%20Slow%20healing%20*%20Chronic%20infections\" rel=\"nofollow noopener\" target=\"_blank\">caused by<\/a> a toxin from the bacteria Corynebacterium diphtheriae or Corynebacterium ulcerans. <\/p>\n<p>Infection occurs in the nose, throat and airway (respiratory diphtheria) or the skin (cutaneous diphtheria). <\/p>\n<p>It spreads through respiratory droplets (coughs and sneezes) or direct contact with the fluid from infected skin sores. <\/p>\n<p>Respiratory diphtheria presents with early symptoms of fever, sore throat, a general sense of feeling unwell (malaise) or loss of appetite. <\/p>\n<p>Over the next few days, a greyish-white membrane may form over the throat and tonsils, making it difficult to swallow and breathe. It can progress to block the airway, causing death from asphyxia. <\/p>\n<p>Cutaneous diphtheria presents as chronic skin ulcers that won\u2019t heal. It generally isn\u2019t fatal, but these sores are a source of bacteria. They can spread to un-immunised or partially immunised children and adults, causing respiratory disease.<\/p>\n<p>The toxin in respiratory infections can also cause <a href=\"https:\/\/doi.org\/10.4103\/jfmpc.jfmpc_1396_20d\" rel=\"nofollow noopener\" target=\"_blank\">myocarditis<\/a> (inflammation of the heart) and <a href=\"https:\/\/doi.org\/10.1001\/archneur.58.9.1438\" rel=\"nofollow noopener\" target=\"_blank\">polyneuropathy<\/a>  (nerve paralysis) in the days or weeks that follow. This occurs only rarely in cutaneous infections.<\/p>\n<p>How is it treated?<\/p>\n<p>Both respiratory and cutaneous diphtheria require prompt <a href=\"https:\/\/www.cdc.gov.au\/diseases\/diphtheria\" rel=\"nofollow noopener\" target=\"_blank\">treatment with antibiotics<\/a> to clear the organism and limit transmission. <\/p>\n<p>Respiratory diphtheria treatment may also require diphtheria <a href=\"https:\/\/immunisationhandbook.health.gov.au\/contents\/vaccine-preventable-diseases\/diphtheria\" rel=\"nofollow noopener\" target=\"_blank\">antitoxin<\/a>, which must be administered early to neutralise circulating toxin. Once the toxin has entered host cells, its effects are no longer reversible. <\/p>\n<p>But the diphtheria antitoxin isn\u2019t readily available. A global decline in cases and decreased production <a href=\"https:\/\/terrance.who.int\/mediacentre\/data\/sage\/SAGE_Docs_Ppt_Apr2017\/10_session_diptheria\/Apr2017_session10_diphtheria_antitoxin_supply.pdf\" rel=\"nofollow noopener\" target=\"_blank\">means<\/a> there are few stockpiles and minimal manufacturing capacity. <\/p>\n<p>Even with treatment, up to one in ten people with respiratory diphtheria <a href=\"https:\/\/www.cdc.gov.au\/diseases\/diphtheria#:%7E:text=It&#039;s%20caused%20by%20the%20toxin%2Dproducing%20bacteria%20Corynebacterium,lesions%20*%20Slow%20healing%20*%20Chronic%20infections\" rel=\"nofollow noopener\" target=\"_blank\">will die<\/a>.<\/p>\n<p>What can be done to prevent it?<\/p>\n<p>Diphtheria can be prevented by vaccination. Diphtheria vaccines are given as part of a combined vaccine that also protects against whooping cough and tetanus (DTP). It stimulates the production of antibodies which protect against diphtheria toxin. <\/p>\n<p>These are routinely given to children in Australia at two months, four months, six months and 18 months, then at four years and again in early adolescence. It\u2019s also given during each pregnancy and a booster is recommended for adults around age 50.<\/p>\n<p>In outbreaks, vaccines can also be used in targeted campaigns to reduce transmission and disease.<\/p>\n<p>One month after completing the three-dose course in infancy, <a href=\"https:\/\/immunisationhandbook.health.gov.au\/contents\/vaccine-preventable-diseases\/diphtheria#immunogenicity\" rel=\"nofollow noopener\" target=\"_blank\">more than 99% of babies<\/a> achieve protective antibody levels that last through childhood. <\/p>\n<p>By middle age, however at least half of people will have <a href=\"https:\/\/immunisationhandbook.health.gov.au\/contents\/vaccine-preventable-diseases\/diphtheria#immunogenicity\" rel=\"nofollow noopener\" target=\"_blank\">low antibody levels<\/a> which may not protect against disease. A booster dose restores this immunity. <\/p>\n<p>The vaccine is <a href=\"https:\/\/www.ausvaxsafety.org.au\/national-immunisation-program-schedule-vaccines\/6-months-schedule-point\" rel=\"nofollow noopener\" target=\"_blank\">safe and generally well-tolerated<\/a>. Mild pain at the injection site and mild fevers are the most common side effects.<\/p>\n<p>How a near-eradicated disease came back<\/p>\n<p>Diphtheria has traditionally resurged in settings with fractured access to vaccination, such as <a href=\"https:\/\/doi.org\/10.1186\/s44263-025-00156-8\" rel=\"nofollow noopener\" target=\"_blank\">in areas of conflict<\/a>. <\/p>\n<p>But even modest declines in vaccine coverage <a href=\"https:\/\/theconversation.com\/measles-cases-are-surging-globally-should-children-be-vaccinated-earlier-257942\" rel=\"nofollow noopener\" target=\"_blank\">create pockets of susceptibility<\/a>, leaving children and vulnerable adults at risk. <\/p>\n<p>In 2025, routine childhood immunisation coverage <a href=\"https:\/\/ncirs.org.au\/tens-thousands-young-children-unprotected-immunisation-coverage-hits-five-year-low-australia\" rel=\"nofollow noopener\" target=\"_blank\">fell to its lowest level in five years<\/a>. Among 12 month olds, vaccination coverage dropped from 94.8% in 2020 to 90.5% in 2025. For two year olds, it fell from 92.1% to 88.4%.<\/p>\n<p>In 2022, an unvaccinated toddler from the Far North Coast of New South Wales was <a href=\"https:\/\/www1.health.gov.au\/internet\/main\/publishing.nsf\/Content\/CA1DBF11D71F5F3ECA258ADE0019B036\/$File\/cdi-2024-48-41.pdf\" rel=\"nofollow noopener\" target=\"_blank\">diagnosed<\/a> in respiratory diphtheria, requiring intensive care treatment. This was the first respiratory diphtheria case in Australia since 1992. <\/p>\n<p>Outbreaks have since been reported in <a href=\"https:\/\/www.populationmedicine.eu\/Toxigenic-diphtheria-cases-in-North-Queensland-Australia,165756,0,2.html\" rel=\"nofollow noopener\" target=\"_blank\">North Queensland<\/a>, the <a href=\"https:\/\/news.wapha.org.au\/outbreak-of-diphtheria-in-the-kimberley\/\" rel=\"nofollow noopener\" target=\"_blank\">Kimberley region<\/a> and the <a href=\"https:\/\/www1.health.gov.au\/internet\/main\/publishing.nsf\/Content\/cda-pubs-cdi-2000-cdi2406-cdi2406f.htm\" rel=\"nofollow noopener\" target=\"_blank\">NT<\/a>. <\/p>\n<p>Only 91.9% of five year olds in the NT have received all the recommended doses of diphtheria vaccine, the <a href=\"https:\/\/www.health.gov.au\/topics\/immunisation\/immunisation-data\/childhood-immunisation-coverage\/current-coverage-data-tables-for-all-children\" rel=\"nofollow noopener\" target=\"_blank\">lowest of all states and territories<\/a>. <\/p>\n<p>However Aboriginal and Torres Strait Islander children in the NT have <a href=\"https:\/\/www.health.gov.au\/topics\/immunisation\/immunisation-data\/childhood-immunisation-coverage\/current-coverage-data-tables-for-aboriginal-and-torres-strait-islander-children\" rel=\"nofollow noopener\" target=\"_blank\">higher vaccine coverage<\/a>, with 95.35% of five year olds vaccinated against diphtheria.<\/p>\n<p>In WA, 92.2% of five year olds olds \u2013 and 92.8% of First Nations five year olds \u2013 are vaccinated against diphtheria.<\/p>\n<p>Why is coverage declining?<\/p>\n<p>The drop in immunisation coverage is due to a mix of <a href=\"https:\/\/ncirs.org.au\/childhood-vaccination-insights\/2025-national-survey-childhood-vaccination-barriers-australia\" rel=\"nofollow noopener\" target=\"_blank\">barriers<\/a>:<\/p>\n<p>practical challenges such as being able to attend a vaccine appointment due to time or travel issues<\/p>\n<p>concerns, beliefs and social influences affecting vaccine acceptance. <\/p>\n<p>Concerns and hesitations about vaccines <a href=\"https:\/\/ncirs.org.au\/our-work\/national-vaccination-insights-project\" rel=\"nofollow noopener\" target=\"_blank\">increased in the last year<\/a> and were often driven by a lack of trust in information from health care providers.<\/p>\n<p>How can we reverse the decline?<\/p>\n<p>Ensuring easy appointments and having supportive conversations about vaccines with GPs, pharmacists or nurses can help parents feel more confidence about vaccines.  <\/p>\n<p>But parents <a href=\"https:\/\/ncirs.org.au\/our-work\/national-vaccination-insights-project\" rel=\"nofollow noopener\" target=\"_blank\">report limited opportunities<\/a> for these conversations. Consultations are often short and out-of-pocket costs can be high.<\/p>\n<p>Reversing this will require investment in primary care \u2013 including funding models that remunerate providers for longer, dedicated vaccine discussions \u2013 and support for training in effective, empathetic communication.<\/p>\n<p>What to do now<\/p>\n<p>To check if your diphtheria vaccination is up to date, ask your health provider to access your Australian Immunisation Records or you can do it <a href=\"https:\/\/my.gov.au\/en\/services\/health-and-disability\/seeking-medical-help\/accessing-your-medical-information\/proof-of-vaccinations\" rel=\"nofollow noopener\" target=\"_blank\">via the myGov app<\/a>. <\/p>\n<p>See your GP, pharmacist or nurse at your community health clinic or Aboriginal medical centre for <a href=\"https:\/\/www.health.gov.au\/topics\/immunisation\/immunisation-information-for-health-professionals\/catch-up-immunisations\" rel=\"nofollow noopener\" target=\"_blank\">catch-up vaccinations<\/a>. These are free for children and adolescents under 20.<\/p>\n<p>If you\u2019re concerned about potential diphtheria exposure and have upper respiratory or chronic skin sores, contact your health provider immediately, as early treatment is essential.<\/p>\n","protected":false},"excerpt":{"rendered":"The Northern Territory and Western Australia are experiencing outbreaks of an almost-eradicated infection, diphtheria.
