BBC<\/a>.<\/p>\nOfcom said the error breached rules 5.1, which requires news to be reported accurately, and 5.5, which requires quick corrections for any errors.<\/p>\n
They said the mistake was a \u201cvery significant inaccuracy\u201d and its subject matter was of \u201cvery high interest\u201d.<\/p>\n
It happened when a maintenance staff member found the pre-recorded obituary files and played them \u201cout of curiosity\u201d but accidentally aired them and cut off the presenter.<\/p>\n
The presenter was recording remotely, as is common practice at the station, and did not realise they were off air.<\/p>\n
The station said the presenter \u201cwould not realise that they had gone \u2018off air\u2019 unless they were monitoring their output via an internet monitor\u201d.<\/p>\n
To make matters worse, the station director who usually did monitor the output was on a phone call at the time of the announcement.<\/p>\n
In the following days, station manager Peter Moore issued an apology on Facebook, explaining that a \u201ccomputer error\u201d was to blame.<\/p>\n
\u201cDue to a computer error at our main studio the Death of a Monarch procedure, which all UK stations hold in readiness while hoping not to require, was accidentally activated on Tuesday afternoon (19 May), mistakenly announcing that HM the King had passed away,\u201d he wrote.<\/p>\n
\u201cRadio Caroline then fell silent as would be required, which alerted us to restore programming and issue an on-air apology.<\/p>\n
\u201cCaroline has been pleased to broadcast Her Majesty the Queen\u2019s, and now the King\u2019s, Christmas Message and we hope to do so for many years to come.<\/p>\n
\u201cWe apologise to HM the King and to our listeners for any distress caused.\u201d<\/p>\n
The staff member involved has since been reprimanded and the digital obituary files have been removed from the desktop to avoid any repeating.<\/p>\n
Radio Caroline was founded in 1964 as an offshore pirate radio station and is the subject of the 2009 film The Boat that Rocked with Philip Seymour Hoffman.<\/p>\n
