As she grew, Aisha showed delays in walking, speech, and other developmental milestones. Despite years of appointments and testing in Bahrain and abroad, no one could say with certainty what was causing Aisha\u2019s symptoms. Then, in 2015 \u2014 at a friend\u2019s suggestion \u2014 they contacted the <a href=\"https:\/\/www.childrenshospital.org\/conditions-treatments\/movement-disorders\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Movement Disorders Program<\/a> at Boston Children\u2019s Hospital.<\/p>\n<p class=\"wp-block-paragraph\">Mariam says Boston Children\u2019s stood out not only for its work in diagnosing and treating rare <a href=\"https:\/\/www.childrenshospital.org\/conditions-treatments\/movement-disorders\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">movement disorders<\/a>, but also because Aisha was connected almost immediately with specialists across the hospital to address her <a href=\"https:\/\/www.childrenshospital.org\/services\/children-medical-complexity\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">complex care needs<\/a>.<\/p>\n<p>Finding a diagnosis and understanding<\/p>\n<p class=\"wp-block-paragraph\">At Boston Children\u2019s, Aisha received <a href=\"https:\/\/www.childrenshospital.org\/services\/genetics\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">genetic testing<\/a> that unveiled she had <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25521004\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">ADCY5-related movement disorder (ADCY5-MD)<\/a>, a rare condition caused by mutations in the ADCY5 gene, which plays a key role in helping regulate signaling in the brain pathways that control movement. At the time, ADCY5-MD had only recently been recognized, and much about it \u2014 including how to treat it \u2014 was unknown.<\/p>\n<p class=\"wp-block-paragraph\">\u201cWhen Aisha was diagnosed with ADCY5, there was still very limited experience worldwide treating patients,\u201d says <a href=\"https:\/\/www.childrenshospital.org\/providers\/darius-ebrahimi-fakhari\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Dr. Darius Ebrahimi-Fakhari<\/a>, pediatric neurologist and director of the Movement Disorders Program. \u201cBut over the last decade or so, our understanding has grown considerably.\u201d<\/p>\n<p class=\"wp-block-paragraph\">For Aisha and her family, that evolution in understanding shaped the next several years of their lives.<\/p>\n<p class=\"wp-block-paragraph\">In 2021, they relocated to Boston so Aisha could receive ongoing multidisciplinary care, including a series of <a href=\"https:\/\/www.childrenshospital.org\/services\/orthopedics-and-sports-medicine\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">orthopedic procedures<\/a> on her spine and legs to improve her mobility. At the same time, the Movement Disorders Program team continued to help lead advances in understanding movement disorders such as ADCY5-MD, including whether therapies such as <a href=\"https:\/\/www.childrenshospital.org\/conditions-treatments\/deep-brain-stimulation\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">deep brain stimulation (DBS)<\/a> might be appropriate for Aisha.<\/p>\n<p class=\"wp-block-paragraph\">DBS uses implanted electrodes to deliver carefully controlled electrical stimulation to specific areas of the brain involved in movement. It has been shown to reduce abnormal movements and improve daily function in some patients with severe movement disorders, including ADCY5-MD.<\/p>\n<p class=\"wp-block-paragraph\">\u201cFor children like Aisha, the benefits of DBS can extend beyond reduced abnormal movement,\u201d Dr. Ebrahimi-Fakhari says. \u201cWe see improvements in endurance, mobility, sleep, and overall quality of life.\u201d<\/p>\n<p>A turning point for considering DBS<\/p>\n<p class=\"wp-block-paragraph\">For Mariam, DBS wasn\u2019t an easy recommendation to accept.<\/p>\n<p class=\"wp-block-paragraph\">\u201cWhen DBS was first suggested, I wasn\u2019t convinced,\u201d she admits. \u201cAisha\u2019s condition was still so new, and they were still learning about it.\u201d<\/p>\n<p class=\"wp-block-paragraph\">What helped change Mariam\u2019s perspective was speaking with another family whose child had undergone DBS for a similar movement disorder. Their conversations allowed Mariam to ask questions, hear firsthand about a similar experience, and better understand what DBS could look like for her daughter.<\/p>\n<p class=\"wp-block-paragraph\">\u201cIt gave me peace of mind,\u201d Mariam says.<\/p>\n<p class=\"wp-block-paragraph\">In 2024, Aisha underwent DBS. The results have led to meaningful improvements in her daily life. After years of struggling to maintain her weight because of constant involuntary movements, Aisha is now better able to support her nutrition. She has also gained mobility and has begun walking with assistance for the first time since she was a toddler.<\/p>\n<p class=\"wp-block-paragraph\">She continues to see the Boston Children\u2019s team and expects to return for DBS evaluations in the coming year, but Aisha\u2019s progress has allowed her family to move back home to Bahrain, where she recently completed high school and is preparing for college. She enjoys swimming, horseback riding, and going to the movies with her family and friends.<\/p>\n<p>Paying it forward<\/p>\n<p class=\"wp-block-paragraph\">Aisha\u2019s experience has inspired the clinicians who care for her. In 2024, Dr. Darius Ebrahimi-Fakhari and <a href=\"https:\/\/www.childrenshospital.org\/providers\/kathryn-yang\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Dr. Kathryn Yang<\/a> launched <a href=\"https:\/\/dbsmatchmaker.com\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">DBSMatchMaker<\/a>, a platform that connects clinicians around the world who are considering DBS for patients with rare movement disorders such as ADCY5-MD.
