Twenty new emergency beds at the Royal Children\u2019s Hospital will be left empty when a long-awaited multimillion-dollar redevelopment is completed in November, because the health service lacks the staff to use them, a damning leaked internal email reveals.<\/p>\n

In a scathing memo sent to senior staff this month, the hospital\u2019s director of emergency medicine, Dr Stuart Lewena, accused the state government of turning down a desperate funding request for more frontline doctors and nurses to be able to open the beds, despite record numbers of sick children flooding to the health service.<\/p>\n

\u201cOn the 10th of November, the builders will be gone, and the 20 additional beds will be ours,\u201d Lewena wrote in the email, sent on September 11. \u201cUnfortunately, we will not be able to open any of them due to lack of staff.\u201d<\/p>\n

The senior doctor warned that patient care was already being impacted by a severe staffing shortage and chronic under-resourcing.<\/p>\n

He alleged the government had rejected a \u201cresponsible and modest\u201d request for operational funding to allow the 20 beds to be opened and staffed.<\/p>\n

\u201cDespite our best efforts, the standard of care we would like to deliver and believe our patients deserve [is] being compromised,\u201d he wrote.<\/p>\n

The 20-bed expansion was intended to be a turning point after years of overcrowding at the hospital and was meant to increase the department\u2019s capacity by about 50 per cent.<\/p>\n

\u201cIt is not enough just to build new facilities. They must be able to operate,\u201d Lewena wrote to staff.<\/p>\n

He said the emergency department had been plagued by \u201cflawed planning\u201d since moving into its current building in 2011, which was originally built for 60,000 children a year.<\/p>\n

By 2018, it was treating more than 80,000 children every year, forcing staff to expand into nearby spaces and work in areas never designed for emergency medicine, he said.<\/p>\n<\/p>\n

\u201cSince moving into this building in 2011, our ED has been grossly undersized for the size of the work to be performed,\u201d Lewena said.<\/p>\n

The emergency department at Victoria\u2019s major specialist paediatric and flagship hospital had exceeded its capacity within the first year of relocation to its new building in Flemington Road, Parkville, he said.<\/p>\n

He warned that the department was now treating about 90,000 children a year, roughly 50 per cent more than the 60,000 the existing department was designed to accommodate.<\/p>\n

The senior doctor also lashed the government over suggestions that pressure mounting on the hospital could be alleviated by diverting children with less serious conditions away from its emergency department.<\/p>\n

He said claims that children who \u201cshould have seen their GP\u201d were clogging the department did not get to the problem.<\/p>\n

\u201cWe cannot allow thinking that blames patients for the failings within the system.\u201d<\/p>\n

Lewena said the \u201cso-called\u201d lower-acuity category 4 and 5 patients represented about a third of cases and included children who needed emergency treatment.<\/p>\n

In an audit earlier this year, he said, this group included children with conditions such as appendicitis, pneumonia, septic arthritis, new diagnosis of leukaemia and other serious conditions that could not be treated by GPs or virtually.<\/p>\n<\/p>\n

Only about 10 per cent of category 4 and 5 patients were deemed suitable for GP care following assessment, he said.<\/p>\n

The email also claimed that independent benchmarking showed the Royal Children\u2019s Hospital had the lowest nursing and medical staffing levels among children\u2019s hospital emergency departments in Australia.<\/p>\n

Lewena cited a recent Australasian College for Emergency Medicine annual site census,<\/a> which he said found an Australian emergency department that treated a comparable number of patients had, on average, 40 per cent more nurses, 50 per cent more doctors and 60 per cent greater access to emergency assessment beds than the Royal Children\u2019s.<\/p>\n

\u201cCalling us undersized and understaffed is not an opinion, but is backed up by facts,\u201d he wrote.<\/p>\n

Lewena said the hospital\u2019s leadership supported the department\u2019s staffing requirements for the 20 new beds.<\/p>\n

The hospital has previously reported that waiting times in its emergency department have blown out to 12 hours<\/a> on occasions when there had been extraordinarily high patient demand.<\/p>\n

Australian Medical Association Victoria president Dr Simon Judkins said some hospitals had been directed to freeze new hiring, preventing the health services from expanding their clinical teams without state health department authorisation.<\/p>\n

\u201cWe\u2019ve got a government that\u2019s very keen on cutting ribbons on things, but is not supporting proper staffing,\u201d Judkins, who is also an emergency doctor, said.<\/p>\n

\u201cThey\u2019re [the government] so concerned about budget overspend that any hospital that wants to expand their staffing profile has to get it signed off by the health department now,\u201d<\/p>\n

\u201cIt makes thing very, very tricky because you know clearly the health department are five times removed from the coalface, and so they actually have no idea about the pressures and the risks.\u201d<\/p>\n

A spokeswoman for the Royal Children\u2019s Hospital said it was on track to complete the redevelopment of the emergency department this year, but did not answer questions about staff or funding shortfalls.<\/p>\n

\u201cWe are continuing to work with the Department of Health to provide improved access to emergency care for children and young people as part of all urgent care services in Victoria,\u201d she said.<\/p>\n

The state government did not answer a list of questions from The Age about the rejection of the hospital\u2019s funding submission, current emergency department staffing deficits, or when the new beds would open to patients.<\/p>\n

A government spokeswoman said it was \u201cworking closely with the hospital to transition services into the expanded emergency department once works are complete\u201d.<\/p>\n

The government said it had invested more than $32.3 billion in Victoria\u2019s health system this year and that it continued to work with the hospital on operational and workforce arrangements for the expanded emergency department.<\/p>\n

The government has allocated more than $56.4 million to expand the Royal Children\u2019s Hospital\u2019s<\/a> acute paediatric beds and emergency department services.<\/p>\n

Opposition health spokeswoman Georgie Crozier said it was another example of the government\u2019s mismanagement of the healthcare system.<\/p>\n

\u201cThis is putting huge pressure on the staff, and it\u2019s actually leading to poor patient outcomes,\u201d she said.<\/p>\n
