Poor communication, operator distraction caused northwest B.C. carnival death: Report

A safety investigation into the death of a carnival worker in Prince Rupert last year has found that poor communication and operator distraction contributed to the incident.
Technical Safety BC’s Incident Investigation revealed that Robert Nickerson, who was fatally struck by Shooting Star Amusements’ Zipper ride on Aug. 26, 2025, volunteered to assist the operator with the ride, which he typically did not work with.
The 17-page report stated that Nickerson stepped into the ride’s path to collect fallen items from underneath the ride during a typical pause in operations, while the operator who was running the ride was approached and distracted by members of the public at the same time.
“A few seconds later, the operator resumed ride operations in the opposite direction, unaware that the attendant was still in the path of the ride,” the Incident Investigation stated. “A passenger carrying unit then struck the attendant, tragically causing fatal injuries.”
Technical Safety BC stated the factors that led to the incident included a lack of communication between the attendant and the operator, limited physical barriers that allowed the public to interfere with the operator, and unsafe practices of working inside dangerous areas.
