Superior Family Health Team is seeking a highly motivated, self-directed individual with strong interpersonal skills to join our multi-disciplinary team-based clinic
Working within the context of a multidisciplinary team, the Registered Nurse will work collaboratively with an allied health care team of professionals within the Family Health Team and with our community partners. As a vital member of the patient focused team, the RN will provide primary care services in conjunction with all health care professionals.
QUALIFICATIONS
Successful completion of an approved RN program
Current registration with the College of Nurses of Ontario
Evidence of strong clinical skill sets
Previous experience with electronic medical records (EMR) an asset
Demonstrated ability to work both independently/effectively in a multi-disciplinary team.
Excellent communication, organizational, multi-tasking and problem-solving skills.

DUTIES
Assessment of patient health care needs and treatment outcomes. Recording of vitals, patient history, biometric measurements
Patient initial intake appointments
Coordination of diagnostic, therapeutic and management plans with the physicians.
Procedures including immunizations, injections, immunotherapy, venipuncture, ear syringing, dressings, assisting in minor surgery, urinalysis, pregnancy testing
Participate in the development and implementation of all of the SFHT’s general and targeted clinical programs, as required.
Help patients to identify and use health resources, involve patients in decisions about their own health, and encourage patients to take action for their own health
Initiate and support health education activities
Liaison with community agencies, hospitals, diagnostic and therapeutic services, pharmacies
Collaboration with other team members to optimize patient health
Provide communication to hospital, community, and other interdisciplinary team members to ensure smooth transition for care
Referrals to community services, as appropriate
Vaccine pick up @ Algoma Public Health(reliable vehicle required with valid driver’s license)
Other duties may be assigned
Some evenings may be required 8pm latest time worked
Off site clinics may be required from time to time
