He\u2019s cutting the programs that fund them"},"content":{"rendered":"<p>      New York<br \/>\n        \u00a0\u2014\u00a0<\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxjye00eh27p1c3go4zkm@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Last month, a homeless man with a history of serious mental illness <a href=\"https:\/\/www.cnn.com\/2025\/09\/08\/us\/iryna-zarutska-murder-ukraine-refugee\" rel=\"nofollow noopener\" target=\"_blank\">killed a woman<\/a> on a Charlotte train. The murder, shown on video, spurred calls from some for a tougher approach to dealing with the rise of homelessness and mental health issues across the United States.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00023b6n019gxssj@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A record number of Americans are living on the streets, roughly a <a href=\"https:\/\/www.kff.org\/mental-health\/a-look-at-the-new-executive-order-and-the-intersection-of-homelessness-and-mental-illness\/\" target=\"_blank\" rel=\"nofollow noopener\">quarter<\/a> with schizophrenia, bipolar disorder or other severe mental illness. The overwhelming majority <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC2686644\/\" target=\"_blank\" rel=\"nofollow noopener\">pose no public danger<\/a>, and <a href=\"https:\/\/journals.lww.com\/hrpjournal\/citation\/2021\/01000\/introduction__violence_and_mental_illness.1.aspx\" target=\"_blank\" rel=\"nofollow noopener\">research<\/a> links only 3% to 5% of violent acts to those with serious mental illness.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00033b6nbfx21ao6@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            President Donald Trump, however, frames homeless people with mental illness as a public safety threat. The solution, he says, is to commit more people into psychiatric hospitals and treatment programs against their will.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00043b6ndyaomqta@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Trump released an executive order in July encouraging states to expand involuntary commitment laws \u2014 a legal process forcing people considered a danger to themselves or others into inpatient psychiatric care.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00053b6n1uywo6iw@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cShifting homeless individuals into long-term institutional settings\u2026will restore public order,\u201d Trump\u2019s order said. He also <a href=\"https:\/\/dailycaller.com\/2025\/09\/01\/donald-trump-daily-caller-interview-insane-asylums-washington-dc-crime\/\" target=\"_blank\" rel=\"nofollow noopener\">said<\/a> in August that he would support the government reopening \u201cinsane asylums\u201d for people with serious mental illness.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00063b6ne4l2gih7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But psychiatric hospital beds, treatment slots and mental health care workers have been in short supply for years. And Trump\u2019s cuts this year to Medicaid, housing assistance and mental health programs will make it harder to add psychiatric beds, undermining his goal of increasing involuntary commitment.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00073b6ncokvddsb@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe big problem is that the mental health care system we have is grossly inadequate. It\u2019s underfunded and overburdened,\u201d said Jeffrey Swanson, a professor in psychiatry and behavioral sciences at Duke University.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00083b6nzxxsogk7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In 2022, the United States had just 18 psychiatric beds per 100,000 people, according to the Substance Abuse and Mental Health Services Administration (SAMHSA) \u2014 this was far below the roughly 60 beds experts <a href=\"https:\/\/www.tac.org\/wp-content\/uploads\/2024\/03\/TAC_ORPA_ResearchSummary1.24.pdf\" target=\"_blank\" rel=\"nofollow noopener\">estimate<\/a> are necessary so anyone who needs a bed can get one. About <a href=\"https:\/\/www.samhsa.gov\/mental-health\/what-is-mental-health\/facts#:~:text=Myth:%20I%20can&#039;t%20do,and%20services%20they%20need%20by:\" target=\"_blank\" rel=\"nofollow noopener\">20%<\/a> of the 14 million adults with serious mental illness do not receive treatment.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct00093b6nktqfjtv1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Trump\u2019s order isn\u2019t just about hospitalizing people. He also wants states to expand assisted outpatient treatment (AOT) \u2014 a court-ordered program for people who are deemed unable to live safely without supervision by a judge. AOT programs offer therapy, medication and other intensive services for people outside a hospital setting.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000a3b6n8d57w1my@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Expanding AOT alone is not a silver bullet, Swanson said. AOT\u2019s <a href=\"https:\/\/www.psychiatryonline.org\/doi\/10.1176\/appi.prcp.20240162\" target=\"_blank\" rel=\"nofollow noopener\">effectiveness<\/a> depends on pairing treatment with affordable housing, services and other support, he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000b3b6nqgdft6vu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Yet the Trump administration has not pledged any new federal money for AOT programs, psychiatric hospital beds or other treatment.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000c3b6nvblsaqq3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe administration wants to force people into treatment that may not exist. It\u2019s a bridge to nowhere,\u201d Swanson said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000d3b6nlrqlxpig@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The White House did not provide comment to CNN\u2019s questions for this article.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/ca\/wp-content\/uploads\/2025\/09\/ap25227104713672.jpg\" alt=\"Trump said homeless people in Washington, DC, should \" move=\"\" out=\"\" immediately.=\"\" class=\"image__dam-img image__dam-img--loading\" onload=\"this.classList.remove('image__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1667\" width=\"2500\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000f3b6ncw50vow0@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Forced hospitalization can be a critical tool when someone is dangerous, either to themselves or others, and is unwilling to engage in treatment, say many legal, advocacy and medical professionals.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000g3b6nlz0ekehh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But advocates for people with mental illness say involuntary commitment risks further traumatizing people and costs more. <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4780300\/\" target=\"_blank\" rel=\"nofollow noopener\">Research<\/a> shows that engaging people voluntarily with treatment and services for their mental health care is less expensive and produces better long-term outcomes.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000h3b6nikqsjs2f@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            However, the political climate has shifted in recent years as homelessness has soared, <a href=\"https:\/\/www.cnn.com\/2024\/12\/27\/business\/homelessness-highest-level-on-record\" rel=\"nofollow noopener\" target=\"_blank\">driven by a lack of affordable housing<\/a>. More than 770,000 people in the United States experienced homelessness in 2024, an 18% increase from 2023, the latest <a href=\"https:\/\/www.hudexchange.info\/news\/hud-releases-2024-ahar-report\/\" target=\"_blank\" rel=\"nofollow noopener\">data<\/a> showed. New York, California, Oregon and other Democratic-led states and cities have supported expanding involuntary commitment.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000i3b6n65obgcd4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Under New York Gov. Kathy Hochul, for example, the state has <a href=\"https:\/\/manhattan.institute\/article\/systems-under-strain-deinstitutionalization-in-new-york-state-and-city-2025-update\" target=\"_blank\" rel=\"nofollow noopener\">added hospital beds<\/a> to state psychiatric centers and loosened the state\u2019s legal standard for involuntarily commitment.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000j3b6nr5if4dch@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But even those who support increasing involuntary commitment acknowledge that several of Trump\u2019s recent policies <a href=\"https:\/\/www.kff.org\/mental-health\/a-look-at-the-new-executive-order-and-the-intersection-of-homelessness-and-mental-illness\/\" target=\"_blank\" rel=\"nofollow noopener\">work in opposition to his goals<\/a> of expanding hospitalization and treatment.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxsct000k3b6nvyij99ou@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThere are cuts that have been made that won\u2019t help with this strategy and are inconsistent with how to make it work,\u201d said Lisa Dailey, the executive director of the nonprofit Treatment Advocacy Center, which lobbies for broader involuntary commitment standards. \u201cThe cuts to Medicaid are going to be devastating.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000l3b6nuzqscfto@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Trump\u2019s broad spending and tax bill is <a href=\"https:\/\/www.kff.org\/medicaid\/allocating-cbos-estimates-of-federal-medicaid-spending-reductions-across-the-states-enacted-reconciliation-package\/\" target=\"_blank\" rel=\"nofollow noopener\">projected to slash Medicaid<\/a> \u2014 the most important source of public mental health care services \u2014 by more than $900 billion over the next decade. According to nonprofit KFF, more than one in three adult <a href=\"https:\/\/www.kff.org\/mental-health\/5-key-facts-about-medicaid-coverage-for-adults-with-mental-illness\/\" target=\"_blank\" rel=\"nofollow noopener\">Medicaid enrollees<\/a> has a mental illness, including 10% with a serious mental illness \u2014 higher than rates among adults with private or no insurance.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000m3b6ny2kmbism@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The Trump administration has also cut a slew of programs for mental health treatment and substance abuse, including<a href=\"https:\/\/www.washingtonpost.com\/health\/2025\/06\/19\/samhsa-addiction-mental-health-cuts\/\" target=\"_blank\" rel=\"nofollow noopener\"> $1 billion<\/a> from SAMHSA. It\u2019s also trying to shift funding from <a href=\"https:\/\/www.cnn.com\/2025\/08\/14\/business\/trump-homelessness-housing-first\" rel=\"nofollow noopener\" target=\"_blank\">Housing First,<\/a> a policy that moves chronically homeless people into long-term housing without preconditions and then pairs them with support services.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000n3b6nz7xr7yvf@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cWhat you see is cutting off resources for community-based services,\u201d said Luke Sikinyi, the director of public policy and public engagement for the Alliance for Rights and Recovery in New York. \u201cWe\u2019re creating a situation where more people experience homelessness and it\u2019s more difficult for providers.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000p3b6n6xy2jsvt@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Trump\u2019s push represents a major policy shift. Since the 1950s, America has worked to close psychiatric institutions.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000q3b6n9ucv7ath@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In 1955, there were more than <a href=\"https:\/\/www.samhsa.gov\/sites\/default\/files\/civil-commitment-mental-health-care-continuum.pdf\" target=\"_blank\" rel=\"nofollow noopener\">550,000<\/a> state and county psychiatric beds. Today, there are an estimated <a href=\"https:\/\/www.tac.org\/wp-content\/uploads\/2024\/03\/TAC_ORPA_ResearchSummary1.24.pdf\" target=\"_blank\" rel=\"nofollow noopener\">36,000<\/a> psychiatric beds at state hospitals.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000r3b6nc7e7m23t@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            <a href=\"https:\/\/www.kff.org\/wp-content\/uploads\/2013\/01\/7684.pdf\" target=\"_blank\" rel=\"nofollow noopener\">Deinstitutionalization<\/a> was driven by many forces, including survivors\u2019 accounts of electric shock therapy and other abuses in state mental hospitals. It was also spurred by the introduction of the antipsychotic drug chlorpromazine in the 1950s; the 1963 Community Mental Health Act to fund a system of local mental health care centers; changes in Medicaid; and landmark court decisions and federal civil rights laws.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000s3b6njcgdbcqv@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But despite the move toward deinstitutionalization, a comprehensive mental health care system that supported people living outside hospitals never fully materialized.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/ca\/wp-content\/uploads\/2025\/09\/shutterstock-editorial-12119108g.jpg\" alt=\"Pilgrim State Mental Hospital in Bentwood, New York, in the late 1930s.\" class=\"image__dam-img image__dam-img--loading\" onload=\"this.classList.remove('image__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1653\" width=\"2500\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000t3b6npwxi057e@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Fewer than half of the community mental health centers proposed by the 1963 Community Mental Health Act were ever built. In 1981, President Ronald Reagan <a href=\"https:\/\/www.nytimes.com\/1981\/10\/15\/us\/concern-on-aid-for-mental-health-is-raised-in-light-of-law-s-repeal.html\" target=\"_blank\" rel=\"nofollow noopener\">cut federal funding for mental health programs by 25%<\/a>.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000u3b6nj3h402ik@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Street homelessness <a href=\"https:\/\/www.coalitionforthehomeless.org\/why-are-so-many-people-homeless\/\" target=\"_blank\" rel=\"nofollow noopener\">grew<\/a> in cities in the late 1970s, and budget-strapped states shifted the burden of mental health care to federal programs like Medicaid.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000v3b6nd50ax30b@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Today, people in the grips of psychosis are sometimes \u201cboarded\u201d in emergency departments and released back on to the street in days.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000w3b6ne477ptsa@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But more often, people with serious mental illness wind up in prisons and jails, which have become the nation\u2019s largest mental health care facilities.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000y3b6njq3kgsz4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The federal government today has limited control over mental health issues. Civil commitment laws are governed by states, and courts have established constitutional protections over decades.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu000z3b6nqdiqx3zm@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But Trump has the bully pulpit, and his executive order is a way to encourage local and state policy changes, said Stephen Eide, who studies homelessness at the conservative think tank Manhattan Institute.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu00103b6n7u57f4sw@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cI want to see untreated serious mental illness prioritized, period. This executive order is an effective way to do it,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu00113b6nrebenv9s@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Eide and other proponents of expanding hospitalization believe the administration could do even more. He advocates <a href=\"https:\/\/manhattan.institute\/article\/us-psychiatric-hospitals-under-medicaids-institutions-for-mental-diseases-exclusion\" target=\"_blank\" rel=\"nofollow noopener\">repealing Medicaid\u2019s \u201cinstitutions for mental disease\u201d (IMD) exclusion.<\/a> This obscure rule bars federal funding for mental institutions with more than 16 beds, which critics say is a major barrier to expanding capacity.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmg5gxscu00123b6nx4706mo4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The administration has approved dozens of <a href=\"https:\/\/www.kff.org\/medicaid\/medicaid-waiver-tracker-approved-and-pending-section-1115-waivers-by-state\/\" target=\"_blank\" rel=\"nofollow noopener\">state waivers<\/a> allowing Medicaid to pay for mental health services for people in institutions, but it has <a href=\"https:\/\/ritchietorres.house.gov\/posts\/congressman-ritchie-torres-introduces-the-repealing-the-imd-exclusion-act\" target=\"_blank\" rel=\"nofollow noopener\">not backed efforts<\/a> to repeal the IMD exclusion.
