The
\ngrant will be dispersed over five years, and CMS will monitor how states
\nimplement their proposals, which means that many states may be reluctant to try
\nsomething that could be truly transformative but would take longer to show
\nresults. Cochlan-McClain said that she would like to see more states provide
\nfunds for hospitals to move away from a fee-for-service model, which is how
\nthey\u2019re reimbursed for care now, to one that provides a global, fixed fund for
\ntaking care of a patient population.<\/p>\n
Describing
\nthe latter model, she said, \u201cThey are paid more based on the outcomes of that
\npatient\u2019s health and care, and less on the number of times they are seen in the
\nclinic.\u201d Because rural health clinics have a lower volume of services, this
\nkind of payment system would allow them to focus on patients\u2019 needs, from preventative
\ncare to mental health, and would pay them for how well the patient does over
\ntime, she said. \u201cIt\u2019s a smarter way to take care of patients, and because it is
\none of the strategic goals in the applications, we are hopeful that states
\nwrote to it,\u201d she said.<\/p>\n
Pennsylvania
\nhas been testing<\/a> such a
\nmodel. But five years would be a short time to implement such a dramatic change
\nto paying for health care, she said. Rural areas already have a shortage<\/a> of
\nhealth care professionals. If doctors in rural areas can\u2019t get paid as well as
\nthey can in larger cities, no amount of workforce training<\/a>, which
