Here&#8217;s Why. : ScienceAlert"},"content":{"rendered":"<p>For more than 20 years, hormone therapy for menopause has carried a warning label from the Food and Drug Administration describing the medication&#8217;s risk of serious harms \u2013 namely,  <a href=\"https:\/\/www.sciencealert.com\/cancer\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73077\" data-postid=\"182118\" rel=\"nofollow noopener\" target=\"_self\">cancer<\/a>, cardiovascular disease, and possibly dementia.<\/p>\n<p>On Nov. 10, 2025, the FDA announced that drugmakers should <a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/hhs-advances-womens-health-removes-misleading-fda-warnings-hormone-replacement-therapy\" rel=\"nofollow noopener\" target=\"_blank\">remove these &#8220;black box&#8221; safety warnings<\/a>.<\/p>\n<p>The Conversation US asked <a href=\"https:\/\/som.cuanschutz.edu\/Profiles\/Faculty\/Profile\/23790\" rel=\"nofollow noopener\" target=\"_blank\">Genevieve Hofmann, a women&#8217;s health nurse practitioner<\/a> at the University of Colorado Anschutz Medical Campus, to explain how the decision will affect health care for people going through menopause or postmenopause.<\/p>\n<p>Related: <a href=\"https:\/\/www.sciencealert.com\/why-stroke-risk-is-so-much-higher-in-women-according-to-experts\" rel=\"nofollow noopener\" target=\"_blank\">Why Stroke Risk Is So Much Higher in Women, According to Experts<\/a><\/p>\n<p>How did the FDA&#8217;s decision come about?<\/p>\n<p>When people think of hormone therapy for menopause, they generally think of <a href=\"https:\/\/medlineplus.gov\/druginfo\/meds\/a601041.html\" rel=\"nofollow noopener\" target=\"_blank\">systemic estrogen and progestogens<\/a> \u2013 for example, <a href=\"https:\/\/www.webmd.com\/menopause\/which-type-of-estrogen-hormone-therapy-is-right-for-you\" rel=\"nofollow noopener\" target=\"_blank\">pills or patches<\/a> that deliver hormones throughout the body.<\/p>\n<p>Health care providers prescribed hormone therapy to manage symptoms of menopause, such as hot flashes, night sweats, and brain fog, <a href=\"https:\/\/doi.org\/10.3390\/medicina55090602\" rel=\"nofollow noopener\" target=\"_blank\">much more widely in the 1980s and 1990s<\/a> than they do today.<\/p>\n<p>That&#8217;s because in the early 2000s, researchers analyzed data from a study called the Women&#8217;s Health Initiative and reported that hormone therapy increased the <a href=\"https:\/\/doi.org\/10.1001\/jama.288.3.321\" rel=\"nofollow noopener\" target=\"_blank\">risk of breast cancer, heart disease, blood clots, and stroke<\/a>, as well as <a href=\"https:\/\/doi.org\/10.1001\/jama.289.20.2651\" rel=\"nofollow noopener\" target=\"_blank\">cognitive decline after menopause<\/a>.<\/p>\n<p>After this research was first published in 2002, the use of <a href=\"https:\/\/doi.org\/10.1097\/01.AOG.0000143826.38439.af\" rel=\"nofollow noopener\" target=\"_blank\">hormone therapy fell by 46 percent within six months<\/a> \u2013 both because clinicians were reluctant to prescribe it and patients were fearful of taking it.<\/p>\n<p>In 2003, the FDA added <a href=\"https:\/\/theconversation.com\/whats-a-black-box-warning-a-pharmacologist-explains-how-these-labels-protect-patients-269470\" rel=\"nofollow noopener\" target=\"_blank\">black box warnings<\/a> \u2013 the most serious warnings, indicating a risk of serious harm or death \u2013 to all estrogen-containing hormone products for menopause.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/ca\/wp-content\/uploads\/2025\/11\/1763599162_677_0.jpg\" alt=\"YouTube Thumbnail\" tabindex=\"0\" role=\"button\" class=\"youtube-thumbnail-preview\" loading=\"lazy\"\/> frameborder=&#8221;0\u2033 allowfullscreen=&#8221;allowfullscreen&#8221;&gt;The FDA announced on Nov. 10, 2025, that it will ask drug companies to remove &#8216;black box&#8217; warnings from hormone therapy for menopause.<\/p>\n<p>But researchers soon <a href=\"https:\/\/doi.org\/10.1016\/j.fertnstert.2005.08.010\" rel=\"nofollow noopener\" target=\"_blank\">pointed out methodological flaws<\/a> in the analysis. And over the past two decades, careful reanalyses of data from that study, as well as newer studies, have shown that systemic hormone therapy <a href=\"https:\/\/doi.org\/10.1097\/AOG.0000000000005553\" rel=\"nofollow noopener\" target=\"_blank\">is very safe for most women<\/a>, though there are nuances surrounding its use.<\/p>\n<p>Meanwhile, women&#8217;s health experts have been increasingly vocal in the past five years in <a href=\"https:\/\/www.medscape.com\/viewarticle\/clinicians-call-easing-fda-warnings-low-dose-estrogen-2024a100093t\" rel=\"nofollow noopener\" target=\"_blank\">calling to remove the black box warnings<\/a> from a form of hormone menopause therapy that&#8217;s applied locally, not systemically.<\/p>\n<p><a href=\"https:\/\/my.clevelandclinic.org\/health\/drugs\/19770-estradiol-vaginal-cream\" rel=\"nofollow noopener\" target=\"_blank\">Topical localized estrogen<\/a> is applied directly to the vagina and surrounding areas, usually in the form of a cream or vaginal insert. It&#8217;s used to treat <a href=\"https:\/\/doi.org\/10.7759\/cureus.7586\" rel=\"nofollow noopener\" target=\"_blank\">the genitourinary syndrome of menopause<\/a>, which manifests as genital and urinary symptoms.<\/p>\n<p>Even though <a href=\"https:\/\/doi.org\/10.1001\/jamanetworkopen.2022.41743\" rel=\"nofollow noopener\" target=\"_blank\">topical estrogen products are extremely safe<\/a> and were not evaluated in the Women&#8217;s Health Initiative study, the FDA warnings were added to them, too.<\/p>\n<p>In July 2025, <a href=\"https:\/\/www.fda.gov\/patients\/fda-expert-panels\/fda-expert-panel-menopause-and-hormone-replacement-therapy-women-07172025\" rel=\"nofollow noopener\" target=\"_blank\">the FDA held an expert panel<\/a> to discuss <a href=\"https:\/\/doi.org\/10.1097\/AOG.0000000000005553\" rel=\"nofollow noopener\" target=\"_blank\">what&#8217;s currently known<\/a> about the risks and benefits of hormone therapy for menopause. At the meeting, most experts urged the agency to remove the warning labels on topical vaginal estrogen products.<\/p>\n<p>The <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/11\/10\/nx-s1-5604283\/fda-menopause-hormone-therapy\" rel=\"nofollow noopener\" target=\"_blank\">Nov. 10 announcement<\/a> was the outcome of that discussion, and it included both systemic and topical hormone therapy.<\/p>\n<p>Why is systemic estrogen no longer considered unsafe?<\/p>\n<p>Researchers are now finding that the balance of risks and benefits of systemic hormone therapy for menopause seems to <a href=\"https:\/\/doi.org\/10.1016\/j.metabol.2016.01.004\" rel=\"nofollow noopener\" target=\"_blank\">depend strongly on when someone starts hormones<\/a>, as well as the type, dose, and length of use.<\/p>\n<p>For women under 60 or within 10 years of their final period, <a href=\"https:\/\/theconversation.com\/menopause-treatments-can-help-with-hot-flashes-and-other-symptoms-but-many-people-arent-aware-of-the-latest-advances-230376\" rel=\"nofollow noopener\" target=\"_blank\">the therapy is much safer<\/a> than it is for older women. A 2017 follow-up of Women&#8217;s Health Initiative participants showed that <a href=\"https:\/\/doi.org\/10.1001\/jama.2017.11217\" rel=\"nofollow noopener\" target=\"_blank\">overall deaths from any causes actually decreased<\/a> in this younger cohort of menopausal women taking hormones.<\/p>\n<p><a href=\"https:\/\/www.sciencealert.com\/newsletter?utm_source=promo_health\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/ca\/wp-content\/uploads\/2025\/11\/health-newsletter-subscribe-promo-final-642x337.jpg\" alt=\"Subscribe to ScienceAlert's free fact-checked newsletter\" width=\"642\" height=\"337\" class=\"alignnone size-medium wp-image-181305\"   loading=\"lazy\"\/><\/a><\/p>\n<p>For women who are more than 10 years from their final menstrual period, starting hormone therapy may <a href=\"https:\/\/doi.org\/10.1001\/jamainternmed.2025.4510\" rel=\"nofollow noopener\" target=\"_blank\">increase their risk of cardiovascular disease<\/a>. Researchers now refer to this as <a href=\"https:\/\/theconversation.com\/menopause-start-estrogen-replacement-therapy-sooner-to-reduce-heart-disease-116890\" rel=\"nofollow noopener\" target=\"_blank\">the timing hypothesis<\/a>. Newer studies <a href=\"https:\/\/doi.org\/10.1056\/NEJMoa1505241\" rel=\"nofollow noopener\" target=\"_blank\">also support this idea<\/a>.<\/p>\n<p>Also, some ways of delivering hormones to the body turned out to be safer than others. Taking estrogen orally, as pills or tablets, <a href=\"https:\/\/doi.org\/10.1161\/CIRCULATIONAHA.106.642280\" rel=\"nofollow noopener\" target=\"_blank\">carries a higher risk of blood clots<\/a>. Those risks go away when it&#8217;s delivered through the skin using a patch, gel or spray. Many more options for hormone therapy exist today than in the early 2000s.<\/p>\n<p>Related: <a href=\"https:\/\/www.sciencealert.com\/experts-urge-caution-over-study-linking-menopause-depression-with-oral-hormones\" rel=\"nofollow noopener\" target=\"_blank\">Experts Urge Caution Over Study Linking Menopause Depression With Oral Hormones<\/a><\/p>\n<p>Additionally, it&#8217;s well established that <a href=\"https:\/\/doi.org\/10.1097\/GME.0000000000001831\" rel=\"nofollow noopener\" target=\"_blank\">hormone therapy improves bone health<\/a> by preventing bone loss. Some studies suggest that in younger menopausal women, it may actually <a href=\"https:\/\/theconversation.com\/hormone-therapy-may-cut-cardiovascular-risk-in-younger-menopausal-women-243561\" rel=\"nofollow noopener\" target=\"_blank\">protect against cardiovascular disease<\/a>, though this link is not yet proven and needs more study.<\/p>\n<p>Unfortunately, many people missed out on the timing window. In my practice, I see patients who went through menopause 10 or 15 years ago and either didn&#8217;t get hormone therapy at the time or stopped taking it when the initial Women&#8217;s Health Initiative results came out. Now, they are hearing about the benefits, and many want to try it. But their higher cardiovascular risk may overshadow the benefit.<\/p>\n<p>What about topical estrogen?<\/p>\n<p><a href=\"https:\/\/doi.org\/10.1097\/GME.0000000000001609\" rel=\"nofollow noopener\" target=\"_blank\">Genitourinary syndrome of menopause is ubiquitous<\/a> \u2013 it affects every person with ovaries who goes through menopause, and the symptoms tend to worsen with age.<\/p>\n<p>They include vaginal dryness, painful sex, and urinary issues such as an increase in urgency or frequency, along with incontinence. Urinary tract infections often tend to get <a href=\"https:\/\/www.acog.org\/womens-health\/experts-and-stories\/the-latest\/utis-after-menopause-why-theyre-common-and-what-to-do-about-them\" rel=\"nofollow noopener\" target=\"_blank\">more frequent with menopause<\/a>, particularly in older women. Treating them can require multiple courses of antibiotics.<\/p>\n<p>Tissues in the genitourinary area are <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC1888320\/\" rel=\"nofollow noopener\" target=\"_blank\">loaded with estrogen receptors<\/a> \u2013 proteins in cells that bind the hormone.<\/p>\n<p>So adding some estrogen back to these areas can help restore the quality and thickness of these tissues, and possibly even promote the growth of <a href=\"https:\/\/doi.org\/10.1016\/j.ajog.2020.08.006\" rel=\"nofollow noopener\" target=\"_blank\">healthy bacteria around the vagina<\/a> and the urinary tract. The treatment can greatly improve quality of life and promote better health and longevity.<\/p>\n<p>Despite topical estrogen&#8217;s safety and effectiveness, the FDA did not distinguish between it and systemic estrogen when adding the black box warnings in 2003.<\/p>\n<p>For this reason, many providers whose patients have symptoms relating to the genitourinary syndrome of menopause <a href=\"https:\/\/doi.org\/10.1016\/j.jogc.2025.103129\" rel=\"nofollow noopener\" target=\"_blank\">have been reluctant to prescribe it<\/a>. Often, providers simply don&#8217;t know that it has a different safety profile than systemic estrogen.<\/p>\n<p>How will removing the black box warnings affect patients?<\/p>\n<p>Overall, I see this as a big win for women and their ability to <a href=\"https:\/\/doi.org\/10.1097\/GME.0000000000002028\" rel=\"nofollow noopener\" target=\"_blank\">manage the symptoms of menopause<\/a>. I think this will make clinicians and patients far less anxious about prescribing and taking this medication.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/ca\/wp-content\/uploads\/2025\/11\/woman_at_doctor_642.jpg\" alt=\"woman consulting doctor\" width=\"642\" height=\"350\" class=\"wp-image-182122 size-full\"   loading=\"lazy\"\/>Removing black box safety warnings from HRT could make patients less anxious. (<a href=\"https:\/\/www.canva.com\/photos\/MAGz-Mqik6Y\/\" rel=\"nofollow noopener\" target=\"_blank\">Alena Shekhovtsova\/Canva Creative Studio<\/a>)<\/p>\n<p>Clinicians like me who specialize in women&#8217;s health and menopause \u2013 and who have been following the research \u2013 have been safely prescribing hormone therapy all along. But many general practitioners who often lacked either menopause-specific training or the time and resources to stay on top of the latest findings have been more reluctant to do so.<\/p>\n<p>Safety concerns that led to the black box warnings, especially in regard to local vaginal estrogen, have turned out to be overblown. While clinicians still need to consider who is a good candidate for systemic hormone use, the evidence shows that for most people, it is a safe option.<\/p>\n<p>Even more important, patients who were previously convinced that hormone therapy was unsafe may feel more comfortable discussing it with their provider and considering it.<\/p>\n<p>And if they do receive a prescription for hormone therapy, I hope that the likelihood of them starting this effective treatment is no longer <a href=\"https:\/\/doi.org\/10.1007\/s00192-025-06246-3\" rel=\"nofollow noopener\" target=\"_blank\">hindered by reading a scary package insert<\/a> that was based on outdated evidence.<\/p>\n<p>While this medication is not a silver bullet that reverses aging, starting hormones at the right time can safely improve symptoms that diminish people&#8217;s quality of life.<\/p>\n<p>So if you&#8217;re having symptoms that are bothersome, consider asking your provider about menopause hormone therapy to help manage them.<img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/ca\/wp-content\/uploads\/2025\/11\/1763599164_622_count.gif\" alt=\"The Conversation\" width=\"1\" height=\"1\" referrerpolicy=\"no-referrer-when-downgrade\" loading=\"lazy\"\/>\n<\/p>\n<p><a href=\"https:\/\/theconversation.com\/profiles\/genevieve-hofmann-2527256\" rel=\"nofollow noopener\" target=\"_blank\">Genevieve Hofmann<\/a>, Assistant Professor of Nursing and Women&#8217;s Health, <a href=\"https:\/\/theconversation.com\/institutions\/university-of-colorado-anschutz-medical-campus-4838\" rel=\"nofollow noopener\" target=\"_blank\">University of Colorado Anschutz Medical Campus<\/a><\/p>\n<p>This article is republished from <a href=\"https:\/\/theconversation.com\" rel=\"nofollow noopener\" target=\"_blank\">The Conversation<\/a> under a Creative Commons license.
