The Public Health Nurse (PHN) is a member of multiple public health program teams responsible for the delivery of the requirements mandated by the Ontario Public Health Program Standards.<\/p>\n
The PHN works in collaboration with internal and external partners to improve the health and well-being of the population (i.e. children, families, youth, and adults) in our communities. The PHN and the teams use knowledge of healthy growth and development, school health, healthy behaviours, community wellness, infectious diseases, evidence informed practice and the assessment of the community to plan health promotion and prevention initiatives. Programs and services will be delivered using relevant guidelines and protocols and the PHN will work in their full scope of practice.<\/p>\n
Activities may include home visits, school and child care visits, clinical services, population health approach, community assessment, collaboration and meaningful community engagement.<\/p>\n
Qualifications, Experience, and Public Health Nurse Skills for this position include but not limited to:<\/p>\n
\u2022 Bachelor of Science in Nursing from a recognized university.
\u2022 Current certificate of registration from College of Nurses of Ontario.
\u2022 Current CPR certificate.
\u2022 Post graduate education or certificate in public health nursing considered an asset.
\u2022 Ability to apply health promotion, disease and injury prevention and health protection strategies.
\u2022 Ability to facilitate planned change through the application of the Population Health Model.
\u2022 Experience with program planning, development, implementation and evaluation based on health equity and effective public health practice including public policy, community mobilization, health education and skill building.
\u2022 Ability to interact effectively with diverse individuals, families, groups and communities.
\u2022 Ability to effectively work independently or as part of a multidisciplinary team.
\u2022 Ability to administer immunizations and manage any post immunization adverse effects safely, effectively, and efficiently to a high volume of clients in a fast-paced clinic environment.
\u2022 Proficient at administering intramuscular, subcutaneous, and intradermal injections to clients of all ages in an empathetic and caring manner.
\u2022 Maintain client confidentiality and ensure that all information is collected in accordance with APH policy and privacy legislation.
\u2022 Utilize evidence-informed practice.
\u2022 Transport, lift and carry equipment and supplies up to 20 lbs.
\u2022 Comply with workplace safety policies and procedures including PPE requirements.
\u2022 Ability to navigate and promote linkages to community programs and services.
\u2022 Ability to build effective relationships with community partners, agencies and\/or regional\/provincial committees.
\u2022 Ability to use reflective practice to continually assess and improve practice.
\u2022 Excellent interviewing, counselling and teaching\/facilitating skills.
\u2022 Ability to work in a self-directed, and highly responsive manner, to establish priorities and balance multiple projects.
\u2022 Experience with electronic health records and electronic data systems would be an asset.
\u2022 Strong interpersonal, organizational and problem-solving skills.
\u2022 Demonstrated proficiency using Microsoft office programs including Word, Excel and Outlook.
\u2022 Must be reliable and have a good attendance record.
\u2022 Advanced oral and written proficiency in English is essential. Proficiency in both official languages, as well as knowledge of additional languages relevant to our diverse communities, will be considered an asset.
\u2022 As a condition of employment, employees must meet immunization requirements in accordance with the APH Immunization policy.
\u2022 Requires a valid \u201cG\u201d Ontario Driver\u2019s Licence and use of a reliable vehicle.
\u2022 Ability to travel to various locations throughout Algoma to deliver services.
\u2022 Ability to work daytime, evenings and weekends, as required for service delivery.
