This can delay an accurate diagnosis, which has negative outcomes for patients as well as for their families and caregivers.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Apathy and depression may look alike from the outside, but they arise from different <a href=\"https:\/\/doi.org\/10.1038\/s41398-022-02292-3\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:neurobiological pathways;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">neurobiological pathways<\/a> and have different implications for treatment, functioning and quality of life for those affected.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Understanding and identifying the differences is crucial. My recent research focused on developing a simple assessment tool to differentiate apathy from depression.<\/p>\n<p>We will see more cases as people age<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Brain disorders, particularly Alzheimer\u2019s disease and other dementias, as well as Parkinson\u2019s disease, are common in Canada.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">According to a 2017 Public Health Agency of Canada (PHAC) report, an estimated <a href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/chronic-diseases\/neurological-conditions\/fact-sheet-mapping-connections-understanding-neurological-conditions-canada.html\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:3.6 million Canadians;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">3.6 million Canadians<\/a> have been diagnosed with brain disorders. More recently, the <a href=\"https:\/\/canadianbrain.ca\/#:%7E:text=THE%20CHALLENGE,supporting%20society%20as%20a%20whole.\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Canadian Brain Research Strategy;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Canadian Brain Research Strategy<\/a> estimates that one in five Canadians is affected by these conditions, with a total estimate of more than 7.5 million people currently living with brain conditions.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">As the Canadian population ages due to demographic shifts and improved survival rates from once-fatal illnesses, the number of people living with these disorders will increase significantly.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">As of January 2025, dementia alone affected <a href=\"https:\/\/alzheimer.ca\/en\/about-dementia\/what-dementia\/dementia-numbers-canada\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:771,939 Canadians aged 65 and older;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">771,939 Canadians aged 65 and older<\/a>. The prevalence of dementia increases with age. The likelihood of a diagnosis more than <a href=\"https:\/\/www.cihi.ca\/en\/dementia-in-canada\/dementia-in-canada-summary\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:doubles every five years after age 65;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">doubles every five years after age 65<\/a>. We can therefore expect more than <a href=\"https:\/\/alzheimer.ca\/en\/about-dementia\/what-dementia\/dementia-numbers-canada\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:1.7 million Canadians;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">1.7 million Canadians<\/a> to be living with dementia by 2050.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Similarly, <a href=\"https:\/\/www.centreforbrainhealth.ca\/news\/cases-of-parkinsons-disease-in-canada-set-to-rise-69-by-2050\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Parkinson\u2019s disease;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Parkinson\u2019s disease<\/a> currently affects more than 100,000 Canadians aged 40 and older, with about 38 new diagnoses each day. Current projections predict that by 2050, the rate may rise by nearly 70 per cent.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">A <a href=\"https:\/\/globaldementia.org\/en\/resource\/canage-dementia-in-canada-cross-country-report-2022\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:growing body of evidence;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">growing body of evidence<\/a> suggests Canada is not prepared for this reality. Expert reviews and health-system assessments highlight persistent barriers to care including <a href=\"https:\/\/doi.org\/10.51731\/cjht.2022.412\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:limited specialist access;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">limited specialist access<\/a>, long diagnostic wait times, <a href=\"https:\/\/www.cihi.ca\/en\/dementia-in-canada\/dementia-in-canada-summary\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:insufficient standardized care pathways;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">insufficient standardized care pathways<\/a> and infrastructure gaps (for example, limited neuroimaging capacity) that <a href=\"https:\/\/brainwellinstitute.org\/mindthegap\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:constrain early and equitable diagnosis and treatment;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">constrain early and equitable diagnosis and treatment<\/a>.<\/p>\n<p>Why we must do better<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The lack of Canada\u2019s preparedness for the rapidly growing number of people with brain disorders is <a href=\"https:\/\/doi.org\/10.51731\/cjht.2022.412\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:harmful in many ways;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">harmful in many ways<\/a>. These include delayed diagnosis and missed opportunities for early intervention, as well as an unsustainable <a href=\"https:\/\/www.cihi.ca\/en\/dementia-in-canada\/dementia-in-canada-summary\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:reliance on emergency departments;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">reliance on emergency departments<\/a> when crises occur.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\"><a href=\"https:\/\/www.cihi.ca\/en\/dementia-in-canada\/dementia-care-across-the-health-system\/dementia-in-hospitals\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Fewer than half;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Fewer than half<\/a> of Canadians currently living with dementia receive a formal diagnosis. In many regions, <a href=\"https:\/\/brainwellinstitute.org\/mindthegap\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:wait times for specialist assessments;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">wait times for specialist assessments<\/a> exceed one to two years, further delaying care planning and symptom management. These delays cause significant stress and strain on patients and their family members, many of whom are left to provide a high level of unpaid caregiving without sufficient supports or resources.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">As a result, family caregivers often experience both <a href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/publications\/diseases-conditions\/dementia-strategy.html#s6.5\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:mental and physical health issues;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">mental and physical health issues<\/a> that can profoundly <a href=\"https:\/\/doi.org\/10.1097\/WAD.0000000000000647\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:reduce their quality of life;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">reduce their quality of life<\/a> and place further strain on existing health-care systems. One of the biggest sources of stress is a lack of diagnosis or misdiagnosis.<\/p>\n<p>The key to diagnosis<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">One of the biggest barriers in accessing timely and accurate diagnoses is the fact that symptoms of brain disorders, <a href=\"https:\/\/doi.org\/10.1212\/WNL.52.7.1403\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:particularly apathy;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">particularly apathy<\/a>, can present very similarly to symptoms of depression. While depression may accompany brain disorders, focusing too narrowly on the symptoms can lead to inappropriate care plans and unsuitable medications, as depression becomes the primary diagnosis while underlying brain disorders remain undetected and undiagnosed.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\"><a href=\"https:\/\/doi.org\/10.1038\/s41398-022-02292-3\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Understanding apathy;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Understanding apathy<\/a> is central in addressing current gaps treatment and prognosis. Research shows that symptoms traditionally associated with depression, such as <a href=\"https:\/\/doi.org\/10.1177\/00048674221114597\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:reduced activity and social withdrawal;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">reduced activity and social withdrawal<\/a>, commonly manifest in people who have <a href=\"https:\/\/www.alzheimers.org.uk\/about-dementia\/stages-and-symptoms\/apathy-dementia\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:brain disorders;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">brain disorders<\/a> but it does not always involve sadness or hopelessness, which are core features of depression.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Large international studies show that apathy on its own is linked to a higher risk of cognitive decline and dementia, even when depression is not present, meaning it\u2019s an <a href=\"https:\/\/www.gbhi.org\/news-publications\/apathy-depression-and-dementia-risk-older-adults-global-collaborative-study\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:important warning sign;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">important warning sign<\/a> that can be missed if symptoms are mislabelled. When apathy is mistaken for depression, people may receive treatments that <a href=\"https:\/\/doi.org\/10.1097\/MD.0000000000021497\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:worsen their apathy;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">worsen their apathy<\/a>, leading to decreased daily functioning, higher caregiver stress, increased safety risks and poorer health over time.<\/p>\n<p>What we can do to improve?<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">To improve <a href=\"https:\/\/doi.org\/10.1002\/gps.5882\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:differentiation between apathy and depression;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">differentiation between apathy and depression<\/a> in people with brain disorders, we need targeted assessment tools that detect motivational decline separately from mood symptoms. Research shows that standard depression scales alone often fail to identify apathy because they focus on <a href=\"https:\/\/doi.org\/10.1017\/S1355617711001068\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:emotional distress rather than motivation and initiative;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">emotional distress rather than motivation and initiative<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">One solution is to adopt a brief <a href=\"https:\/\/doi.org\/10.1177\/30495334251345093\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:apathy-specific tool;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">apathy-specific tool<\/a> that is quick to administer in both care settings and at home. This tool consists of <a href=\"https:\/\/doi.org\/10.3390\/geriatrics11010001\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:three questions;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">three questions<\/a>:<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Have you dropped many of your activities and interests?<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Do you prefer to stay in your room\/home rather than going out and doing new things? and<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Do you feel full of energy?<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">If the answers are \u201cYes\u201d to question one or question two (or both) and \u201cNo\u201d to question three, apathy \u2014 not depression \u2014 should be suspected.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Another important solution is to put training and awareness at the centre of clinical practice, education and caregiver support to improve care and quality of life for people with brain disorders.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Research shows that many clinicians, caregivers and health professionals <a href=\"https:\/\/doi.org\/10.1177\/21582440241241882\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:lack specific training;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">lack specific training<\/a> on how to recognize and differentiate apathy from depression, which contributes to misdiagnosis and suboptimal care.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Increasing education about apathy including how it presents, why it differs from mood disorders and how to use appropriate assessment tools helps clinicians make more accurate diagnoses and <a href=\"https:\/\/doi.org\/10.1177\/30495334251345093\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:develop tailored care plans;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">develop tailored care plans<\/a>. Training should also extend to multidisciplinary teams and family caregivers, because apathy often goes unrecognized in routine interactions and can be <a href=\"https:\/\/doi.org\/10.1177\/21582440241241882\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:misinterpreted as laziness, resistance or depression;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">misinterpreted as laziness, resistance or depression<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">In brain disorders where cognitive changes complicate the overlap of symptoms, differentiating between apathy and depression is essential to optimal living.
