{"id":499540,"date":"2026-02-25T23:24:09","date_gmt":"2026-02-25T23:24:09","guid":{"rendered":"https:\/\/www.newsbeep.com\/ca\/499540\/"},"modified":"2026-02-25T23:24:09","modified_gmt":"2026-02-25T23:24:09","slug":"inside-the-race-to-find-young-onset-colorectal-cancer-patients-faster","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/ca\/499540\/","title":{"rendered":"Inside the Race to Find Young-Onset Colorectal Cancer Patients Faster"},"content":{"rendered":"<p data-journey-content=\"true\" data-node-id=\"0\" class=\"css-6wxqfj emevuu60\">Dave Runkel went to his primary care physician with a problem: the 36-year-old engineer was passing blood. He was a Navy vet living in South Carolina with a career that required frequent overseas travel. His wife also worked full-time, and together they were raising three kids between the ages of 6 and 14. Seeing blood in his stool was disturbing to Runkel, because he\u2019d intermittently had extremely loose stools and searing abdominal pain for nearly a decade\u2014but none of his doctors had ever taken it seriously. Runkel went to the doctor determined to get a colonoscopy. The doctor hesitated\u2014Runkel was younger than the screening age of 45, and colorectal cancer is still relatively rare at his age. Runkel became frustrated and decided to lie to the doctor and tell him he had a family history of cancer. The doctor ordered the test. When he finally got the colonoscopy several months later, the doctors found colorectal cancer. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"1\" class=\"css-6wxqfj emevuu60\">Living with a cancer diagnosis in his 30s was complicated. Runkel would go to work in the morning, work until four PM, and then head to the clinic to receive intra-rectal radiation. Then he started eight rounds of chemotherapy that consisted of a FOLFOX infusion\u2014a portable balloon pump containing multiple chemotherapy drugs\u2014in the middle of the week. On Friday, after the pump was removed, he would sleep for fourteen hours, feel horrible until Sunday evening, and return to work on Monday. (Since memory loss was a side effect of his chemotherapy, Runkel\u2019s coworkers supply details of events that happened during \u201cthe bad times.\u201d) <\/p>\n<p data-journey-content=\"true\" data-node-id=\"2\" class=\"css-6wxqfj emevuu60\">Then he had surgery to remove the tumor and an ileostomy procedure, where the intestine is routed through the abdomen and empties into a bag. Two months later, he underwent another surgery to reverse the ileostomy. Among all the uncomfortable drugs and medical procedures, the part that bothered Runkel the most was the extra stress cancer put on his wife and kids. And he wondered about how his life would have been different if an earlier doctor had just ordered him a colonoscopy.<\/p>\n<p data-journey-content=\"true\" data-node-id=\"4\" class=\"css-6wxqfj emevuu60\">\u201cI try not to think of what would have happened if they had found this at the beginning,\u201d says Runkel. \u201cI find that if I just dwell on that part of the story, then it&#8217;s just that you&#8217;re angry.\u201d<\/p>\n<p>Tracing Clues to Who&#8217;s At Risk<\/p>\n<p data-journey-content=\"true\" data-node-id=\"6\" class=\"css-6wxqfj emevuu60\">Scientists are racing to understand and respond to the rise in early-onset colorectal cancer, which has nearly doubled since 1995, to help find patients like Runkel sooner. According to new data from the University of Missouri, since 1990, patients aged 20 to 24\u2014that\u2019s when Runkel first had symptoms\u2014saw a 185 percent increase in cases, while patients in his age group of 35 to 39 saw a 58.1 percent increase. Even more disturbing is that people with colon cancer that develops this early often seem otherwise super healthy. Now, early onset colorectal cancer centers have opened across the United States, aimed at treating the unique needs of young patients and performing research to better understand the disease. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"7\" class=\"css-6wxqfj emevuu60\">But that\u2019s treatment. A big question that remains and gnaws at researchers is this: How can we find these younger patients before their disease spreads? Scientists\u2019 goal is to identify what is unique about patients with earlier onset, so they can increase screening efforts in the people who are likely to develop this cancer. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"8\" class=\"css-6wxqfj emevuu60\">Recently, a fascinating new discovery has gotten researchers closer to identifying who to screen, and it\u2019s redefining the research landscape. In the past few years, some studies had already identified moderate associations between obesity, diabetes, sugar-sweetened beverages, and a sedentary lifestyle and the risk of developing colorectal cancers, but no single finding seemed to explain the increase. Researchers at the University of California San Diego (UCSD) set out to look for differences in who got this cancer and who didn\u2019t by sequencing the DNA from colorectal cancer tumors. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"9\" class=\"css-6wxqfj emevuu60\">They discovered what is called a \u201cmutational signature\u201d in earlier onset cases like Runkel\u2019s. Different carcinogens generate different patterns of DNA mutations; these allow researchers to group patients with similar exposures like cigarette smoking. The UCSD researchers, part of a large international research group called team <a href=\"https:\/\/www.mutographs.org\/\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/www.mutographs.org\/\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Mutographs\" data-node-id=\"9.1\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Mutographs<\/a>, are deciphering the unique mutational processes of different cancer types. What they found in this case is that the young patients shared a mutational signature already known to be caused by a mutation-inducing toxin, colibactin. Colibactin is produced by a specific strain of E. coli. bacteria in the colon. These E. coli harbor a polyketide synthase gene which enables the production of this colibactin toxin (the E. coli are called pks+ E. coli). Men and women with colorectal cancer under 40 were 3.3 times more likely to have this mutational signature than patients over 70. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"10\" class=\"css-6wxqfj emevuu60\">This exciting new finding was so impactful that the journal Nature designated it for accelerated article preview in April 2025. Maybe it could turn out to unlock which people are at risk for cancer. \u201cWe could try to find ways to detect this mutation, which is kind of the consequence of the infection,\u201d says Marcos D\u00edaz-Gay, PhD, the first author on the study and now a Junior Group Leader at the <a href=\"https:\/\/dashboard.eacr.org\/event\/session\/person\/968299?eid=711\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/dashboard.eacr.org\/event\/session\/person\/968299?eid=711\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Spanish National Cancer Research Center\" data-node-id=\"10.3\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Spanish National Cancer Research Center<\/a>. But even better would be actually trying to detect the infection when it\u2019s happening, he says, which the authors think could be during childhood. While colibactin testing exists, ordering it in the clinic is a few steps away. And knowing who to test for this strain of E.coli in the first place, long before they come to a clinic with cancer, could be a huge leap forward. \u201cFrom a research perspective, we would like to understand if there are certain environments, diets, lifestyle, behaviors that might be more related with this colibactin,\u201d says Dr. D\u00edaz-Gay.<\/p>\n<p>What Else Could it Be?<\/p>\n<p data-journey-content=\"true\" data-node-id=\"12\" class=\"css-6wxqfj emevuu60\">As exciting as these findings are, they\u2019re only a piece of the puzzle, since colibactin exposure was only associated with less than 20 percent of colorectal cancer cases in the <a href=\"https:\/\/go.redirectingat.com?id=74968X1576254&amp;url=https%3A%2F%2Fwww.nature.com%2Farticles%2Fs41586-025-09025-8%23Fig3\" data-vars-ga-outbound-link=\"https:\/\/www.nature.com\/articles\/s41586-025-09025-8#Fig3\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"study\" data-vars-ga-product-id=\"864248b6-b5c8-48f1-b1f6-63a9a4392788\" rel=\"nofollow noopener\" data-node-id=\"12.1\" data-href=\"https:\/\/www.nature.com\/articles\/s41586-025-09025-8#Fig3\" data-product-url=\"https:\/\/www.nature.com\/articles\/s41586-025-09025-8#Fig3\" data-affiliate=\"true\" data-affiliate-url=\"https:\/\/go.redirectingat.com?id=74968X1576254&amp;url=https%3A%2F%2Fwww.nature.com%2Farticles%2Fs41586-025-09025-8%23Fig3\" data-affiliate-network=\"{&quot;site_id&quot;:&quot;c7b9f75a-2f85-4251-a92e-dbc6c7213473&quot;,&quot;metadata&quot;:null,&quot;network&quot;:{&quot;name&quot;:&quot;Skimlinks&quot;},&quot;product_metadata&quot;:null,&quot;afflink_redirect&quot;:&quot;\/_p\/afflink\/12Mv8\/nature-geographic-and-age-variations-in&quot;}\" data-vars-ga-product-price=\"$0.00\" data-vars-ga-product-retailer-id=\"b698d337-2fde-4458-880e-e565478acfb0\" data-vars-ga-link-treatment=\"(not set) | (not set)\" class=\"body-link product-links css-10fnnmt e1aq0z090\" target=\"_blank\">study<\/a>. <a href=\"https:\/\/www.dana-farber.org\/find-a-doctor\/kimmie-ng\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/www.dana-farber.org\/find-a-doctor\/kimmie-ng\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Kimmie Ng, MD, MPH\" data-node-id=\"12.3\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Kimmie Ng, MD, MPH<\/a>, the founder of the Young-Onset Colorectal Cancer Center at Dana-Farber Cancer Institute, is focused on another aspect of the rise. She\u2019s studying the risks of different environmental exposures, similar to how lung cancer screening uses smoking as a risk factor to identify people who should be screened.<\/p>\n<p data-journey-content=\"true\" data-node-id=\"13\" class=\"css-6wxqfj emevuu60\">Among the possible contributors to young-onset colon cancer are the ideas that it\u2019s driven by the environment outside of you. \u201cWe think that this whole process is very complex, and related to some environmental driver,\u201d said Dr. Ng. \u201cIt does not seem to be any shift in genetics or change in family history or inflammatory bowel disease rates rising.\u201d She cites the birth cohort effect, a phenomenon where a group of people born in the same era, with the same exposures in their early life, have increased rates of disease, as evidence for an environmental change behind the increase. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"14\" class=\"css-6wxqfj emevuu60\">There\u2019s also a theory that it might have to do with changes in the environment inside of you. Cancer researchers call this the \u201cseed vs. soil\u201d hypothesis, where the cancer cell is the seed and the surrounding cells, including immune cells and other resident cells, are the soil. <a href=\"https:\/\/medschool.vanderbilt.edu\/cdb\/person\/ken-lau-ph-d\/\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/medschool.vanderbilt.edu\/cdb\/person\/ken-lau-ph-d\/\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Ken Lau, PhD\" data-node-id=\"14.1\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Ken Lau, PhD<\/a>, a Professor of Cell and Developmental Biology and Surgery at Vanderbilt University, hypothesizes that the seeds\u2014cells with specific mutations with a small chance of becoming cancer\u2014lie dormant until the soil is fertile. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"15\" class=\"css-6wxqfj emevuu60\">\u201cThe idea is that all of us probably have abnormal cells, and they lie dormant for long periods of time before they become activated to first create a polyp, then some of these polyps become cancer,\u201d said Dr. Lau. The question researchers are chasing is: \u201cWhat are the factors that shorten this dormant time and allow these abnormal cells to grow, to proliferate?\u201d he adds. <\/p>\n<p data-journey-content=\"true\" data-node-id=\"16\" class=\"css-6wxqfj emevuu60\">To help answer the question of what makes the soil fertile, and why that\u2019s happening more frequently in young people, researchers are finding ways to better understand normal colon cells in young people. <a href=\"https:\/\/www.fredhutch.org\/en\/faculty-lab-directory\/grady-william.html\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/www.fredhutch.org\/en\/faculty-lab-directory\/grady-william.html\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"William Grady, MD\" data-node-id=\"16.1\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">William Grady, MD<\/a>, the Medical Director of the Gastrointestinal Cancer Prevention Program at Fred Hutchinson Cancer Center, is interested in profiling colon cells to understand if young patients\u2019 colons are aging at a more rapid pace than in the past. They might be.<\/p>\n<p data-journey-content=\"true\" data-node-id=\"17\" class=\"css-6wxqfj emevuu60\">Dr. Grady is looking at epigenetic modifications to figure this out. Epigenetic modification is a process of reversible chemical modifications to the DNA which alter gene expression. What that means: They act like an on-off switch. For example, when a methyl group is added to DNA, that gene will be turned off. These changes can be correlated with biological age through grip strength, lung function, and other tests. So far, Dr. Grady\u2019s team found that in people with early onset colorectal cancer, \u201ctheir epigenetic age is actually even higher than people who have late onset colorectal cancer. It suggests that something&#8217;s causing an accelerated biological aging in those younger individuals.\u201d <\/p>\n<p data-journey-content=\"true\" data-node-id=\"18\" class=\"css-6wxqfj emevuu60\">It\u2019s possible, he thinks, that biological aging is associated with metabolic syndrome\u2014that\u2019s when people have three or more of these symptoms at the same time: high blood pressure, high blood sugar, high triglycerides, low HDL cholesterol, and abdominal obesity. Metabolic syndrome incidence is significantly increasing in people aged 20 to 39. \u201cWe&#8217;re wondering if something&#8217;s happening that&#8217;s also just accelerating the biological age in general, which then would increase the cancer risk,\u201d says Dr. Grady.  <\/p>\n<p>What to Do Now<\/p>\n<p data-journey-content=\"true\" data-node-id=\"20\" class=\"css-6wxqfj emevuu60\">Despite the increase in risk, young-onset colorectal cancer is still relatively rare. People who are experiencing symptoms like blood in their stool, a change in bowel habits, abdominal pain or unexplained weight loss should see a doctor \u2013 and you may need to seek a second opinion if you feel dismissed. \u201cWe know colorectal cancer, when caught early, has a good survival rate, and that\u2019s true even for younger patients,\u201d says <a href=\"https:\/\/www.cancer.columbia.edu\/profile\/joel-t-gabre-md\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/www.cancer.columbia.edu\/profile\/joel-t-gabre-md\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Joel Gabre, MD\" data-node-id=\"20.1\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Joel Gabre, MD<\/a>, one of the directors of Columbia University\u2019s <a href=\"https:\/\/www.cancer.columbia.edu\/early-onset-colorectal-cancer-program\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/www.cancer.columbia.edu\/early-onset-colorectal-cancer-program\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Early Onset Colorectal Cancer Program\" data-node-id=\"20.3\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Early Onset Colorectal Cancer Program<\/a>.<\/p>\n<p data-journey-content=\"true\" data-node-id=\"21\" class=\"css-6wxqfj emevuu60\">But for young people with no symptoms, Dr. Grady recommends putting your energy into lowering your cancer risk until we know more about whom to test for what. \u201cFocus on the things you can do to live a healthy lifestyle: exercise regularly; maintain a normal body weight; eat a healthy diet, which is mainly avoiding red meat and processed foods and eating more whole foods; and avoid tobacco exposure.\u201d <\/p>\n<p data-journey-content=\"true\" data-node-id=\"22\" class=\"css-6wxqfj emevuu60\">\u201cThe research that\u2019s needed most is the risk stratification research\u2014who do we screen,\u201d says Dr. Gabre. By getting to the root of who\u2019s at risk, we\u2019ll know whom to screen, and ideally, find people like Runkel faster. In the meantime, don\u2019t ignore new symptoms. It could save your life. <\/p>\n<p>New Tech, New Hope<\/p>\n<p data-journey-content=\"true\" data-node-id=\"25\" class=\"css-6wxqfj emevuu60\">These technologies are moving science closer to knowing why colon cancer is rising in young people.<\/p>\n<p>Exposomics<\/p>\n<p data-journey-content=\"true\" data-node-id=\"27\" class=\"css-6wxqfj emevuu60\">To accelerate discovery, scientists are determined to gather as much data as possible about people with young-onset colorectal cancer. One study, the <a href=\"https:\/\/healthcommcore.org\/portfolio-items\/consortium-for-childhood-cancer-prevention-c3p\/\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/healthcommcore.org\/portfolio-items\/consortium-for-childhood-cancer-prevention-c3p\/\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Beyond CRC Project from Dana-Farber\" data-node-id=\"27.1\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Beyond CRC Project from Dana-Farber<\/a>, will collect samples of blood, stool, and tumor from all new young-onset patients. These will be paired with exposome data: validated diet, lifestyle, and environmental exposure questionnaires. They will also collect data from healthy controls and older colorectal cancer patients to make clear comparisons about what is unique in younger patients. To understand the increase globally, they\u2019re enrolling patients from Spain, Brazil, and around the world. <\/p>\n<p>Spatial transcriptomics<\/p>\n<p data-journey-content=\"true\" data-node-id=\"29\" class=\"css-6wxqfj emevuu60\">Scientists are using this technology to help build a comprehensive map of what\u2019s happening in the colon. The milieu around tumor cells\u2014called the tumor microenvironment\u2014is made up of many other types of normal, native cells, as well as blood vessels and structural scaffolding. Cancer cells, determined to grow bigger, send signals to these other cells. These shut down opposing immune cells, build new blood vessels, and gain fuel and space so they can keep growing. Spatial transcriptomics shows us where all those players are at a specific time, almost like a freeze-frame instant replay. Tracking the complex interactions of all these components could reveal dynamics that could be game changers. For instance, some tumors have very active immune cells while others don\u2019t. Understanding why some patients have \u201chotter\u201d tumors than others could be a key to improving treatment.<\/p>\n<p>AI<\/p>\n<p data-journey-content=\"true\" data-node-id=\"31\" class=\"css-6wxqfj emevuu60\">Since the locations of cancer cells and the tumor microenvironment are so important, scientists want to see these interactions in three dimensions. Traditional spatial transcriptomics looks at only one slice of tumor in two dimensions, but Dr. Lau\u2019s research team at Vanderbilt University aims to look at multiple levels of tissue. To splice them together into a three-dimensional reconstruction, the team will use AI. This 3D data will be used in a large study called the <a href=\"https:\/\/humantumoratlas.org\/\" target=\"_blank\" data-vars-ga-outbound-link=\"https:\/\/humantumoratlas.org\/\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Human Tumor Atlas Network (HTAN)\" data-node-id=\"31.1\" class=\"body-link css-10fnnmt emevuu60\" rel=\"nofollow noopener\">Human Tumor Atlas Network (HTAN)<\/a>. As in other studies, this team is focused on comparing young-onset patients to healthy controls. Another clue in why tumors develop in young patients could be found in pre-cancerous tumors, which they plan to study using the 3D reconstructions. The HTAN hopes to understand how the organization of cells in 3D space could be key to why some patients develop cancer and others don\u2019t. <\/p>\n<p>Metagenomics<\/p>\n<p data-journey-content=\"true\" data-node-id=\"33\" class=\"css-6wxqfj emevuu60\">This way of studying the microbiome allows researchers to get a read on how the microbiome is different between younger and older patients. In Dr. Gabre\u2019s Columbia lab, they are using metagenomics to study not just one type of microbe, but the whole population at once. Think of this like fishing: instead of just fishing for one type of fish with a specific lure, a net will catch all the fish in an area at once. Similarly, metagenomics will find and identify all the diverse bacteria species at once using sequencing. This will help doctors understand how specific microbes in the colon affect nearby colon cells.
