A safety investigation into the death of a carnival worker in Prince Rupert last year has found that poor communication and operator distraction contributed to the incident.<\/p>\n
Technical Safety BC\u2019s Incident Investigation revealed that Robert Nickerson, who was fatally struck by Shooting Star Amusements\u2019 Zipper ride on Aug. 26, 2025, volunteered to assist the operator with the ride, which he typically did not work with.<\/p>\n
The 17-page report stated that Nickerson stepped into the ride\u2019s path to collect fallen items from underneath the ride during a typical pause in operations, while the operator who was running the ride was approached and distracted by members of the public at the same time.<\/p>\n
\u201cA few seconds later, the operator resumed ride operations in the opposite direction, unaware that the attendant was still in the path of the ride,\u201d the Incident Investigation stated. \u201cA passenger carrying unit then struck the attendant, tragically causing fatal injuries.\u201d<\/p>\n
