This is the only such case that has been reported to date, and the findings are still awaiting peer-review; but for a condition where the options for patients are severely limited, the study offers some hope of new avenues to explore.<\/p>\n<p>After living with psychiatric illnesses, including depression and PTSD, for many years and experiencing his first panic attacks when he was just a kindergartner, the patient in this study had been hospitalized numerous times. The authors write that he had endured \u201cone protracted depressive episode without distinct periods of remission for 31 years.\u201d<\/p>\n<p>They describe his medical history as \u201cremarkable\u201d \u2013 he has tried at least 19 different <a href=\"https:\/\/www.iflscience.com\/antidepressants-all-you-need-to-know-from-serotonin-to-psychedelics-73941\" target=\"_blank\" rel=\"nofollow noopener\">medications<\/a> and undergone electroconvulsive therapy (ECT) three times. While this treatment <a href=\"https:\/\/www.iflscience.com\/electroconvulsive-therapy-really-works-for-depression-and-now-we-know-why-72482\" target=\"_blank\" rel=\"nofollow noopener\">can be effective<\/a> in some cases, in this patient it unfortunately left him with cognitive impairment.\u00a0<\/p>\n<p>Ultimately, the patient had experienced suicidal ideation and made attempts to take his own life. It\u2019s thought that around a third of patients with major depressive disorder will progress to TRD, as in this case, and that is a <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0165178124005341\" target=\"_blank\" rel=\"nofollow noopener\">strong risk factor<\/a> for suicidality.\u00a0<\/p>\n<p>The researchers, based primarily at the University of Minnesota, enrolled the patient in a study to evaluate, for the first time, a form of minimally invasive brain stimulation called personalized adaptive cortical electro-stimulation (PACE).\u00a0<\/p>\n<p>First, a process called precision functional mapping was carried out. This uses functional magnetic resonance imaging (fMRI) to map the patient\u2019s specific brain activity \u2013 what the authors call a \u201cbrainotype\u201d \u2013 so that the brain stimulation can be targeted most effectively.<\/p>\n<p>\u201c[The patient\u2019s] salience network was highly atypical,\u201d the authors write. This is a suite of brain regions that have been extensively studied since their discovery was reported in a <a href=\"https:\/\/www.jneurosci.org\/content\/27\/9\/2349?ijkey=6873b2121233344360e061467d4a9a7014313ce0&amp;keytype2=tf_ipsecsha\" target=\"_blank\" rel=\"nofollow noopener\">2007 paper<\/a>. It\u2019s difficult to summarize all the known <a href=\"https:\/\/www.iflscience.com\/repeated-trauma-during-adolescence-alters-connectivity-of-brains-triple-network-63713\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">functions<\/a> of the salience network; it\u2019s thought to play a vital role in our ability to <a href=\"https:\/\/journals.plos.org\/plosbiology\/article?id=10.1371\/journal.pbio.1002469\" target=\"_blank\" rel=\"nofollow noopener\">identify and pay attention to<\/a> important signals from our environment.<\/p>\n<p>\u201cIn [this patient], the salience network was expanded to cover 12.4 percent of the cortical surface area, a four-fold enlargement compared to healthy controls,\u201d the authors continued. This enlargement has previously been linked to depression, so it\u2019s not a totally surprising finding.<\/p>\n<p>Armed with their \u201cbrainotype\u201d map, the team could then perform surgery to implant four <a href=\"https:\/\/www.iflscience.com\/neuralinks-got-competition-world-record-smashed-for-electrodes-placed-on-a-human-brain-74423\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">electrodes<\/a> in the optimal places on the patient\u2019s brain, two in each hemisphere at the boundaries where the salience network was encroaching on other networks. After recovery, it was time to start stimulation.<\/p>\n<p>When the <a href=\"https:\/\/www.iflscience.com\/psilocybin-desynchronizes-important-brain-networks-and-it-could-explain-its-antidepressant-effects-70418\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">default mode network<\/a> (DMN) was stimulated, the result was instantaneous and striking. In the patient\u2019s own words, he felt overwhelming joy. \u201cIt feels nice. So weird to feel. It is so emotional,\u201d he <a href=\"https:\/\/osf.io\/nqhyg\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">reported<\/a>, breaking down in tears.<\/p>\n<p>Stimulation of other electrodes led to feelings of calm and relaxation, while another stimulation pattern actually induced anxiety in the patient, showing that not every result will be positive, and stimulation must be carefully adapted for each individual.<\/p>\n<p>This optimization happened during monthly visits with the team. Within seven weeks of treatment, the patient\u2019s suicidal ideation had completely gone away. After four months, his mood had improved by 59 percent on standardized test. The improvements were sustained for at least 30 months.\u00a0<\/p>\n<p>\u201cPACE provided [the patient] with the longest stretch of wellness he had experienced in his adult life,\u201d the authors write.\u00a0<\/p>\n<p>\u201cIn psychiatry, we don\u2019t have cures, but this is probably the closest we can get,\u201d first author Ziad Nahas told <a href=\"https:\/\/gizmodo.com\/man-experiences-joy-for-the-first-time-in-decades-after-brain-stimulation-treatment-2000646279\" target=\"_blank\" rel=\"nofollow noopener\">Gizmodo<\/a>.\u00a0<\/p>\n<p>For this individual, this is a transformative result, but it\u2019s important to remember that these are the results from just one patient. The authors have set the goal of conducting a larger double-blind clinical trial in the future. Every case of TRD is different, which is why there is so much hope resting on the <a href=\"https:\/\/www.iflscience.com\/personalized-brain-stimulation-reduces-parkinsons-symptoms-by-50-percent-in-early-study-75604\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">personalization<\/a> element of PACE treatment \u2013 something that\u2019s missing from conventional ECT.\u00a0<\/p>\n<p>The paper is awaiting peer review, and much more evidence will be needed before PACE could ever be approved for use more widely as a clinical treatment. <a href=\"https:\/\/www.iflscience.com\/fda-approves-first-ever-standalone-therapy-for-treatment-resistant-depression-77750\" target=\"_blank\" rel=\"nofollow noopener\">TRD<\/a> is a gnarly scientific problem to solve, and this approach is <a href=\"https:\/\/www.iflscience.com\/ketamines-potential-to-treat-depression-beautifully-shown-in-struggling-zebrafish-77563\" target=\"_blank\" rel=\"nofollow noopener\">one among<\/a> many <a href=\"https:\/\/www.iflscience.com\/merry-mice-human-brain-cells-transplanted-into-mice-reduce-anxiety-and-depression-80489\" target=\"_blank\" rel=\"nofollow noopener\">research avenues<\/a> being explored, but many will be watching with fascination to see if the life-changing benefits achieved here can be replicated in more patients.<\/p>\n<p>The preprint has been posted to <a href=\"https:\/\/osf.io\/preprints\/psyarxiv\/5c3ba_v1\" target=\"_blank\" rel=\"nofollow noopener\">PsyArXiv<\/a>.<\/p>\n<p>If you or someone you know is struggling, help and support are available in the US via the\u00a0<a href=\"https:\/\/988lifeline.org\/\" target=\"_blank\" rel=\"nofollow noopener\">988 Suicide &amp; Crisis Lifeline<\/a>,\u00a0which can be contacted by dialing 988.
