An inquest into the death of a patient who took his own life when he was being “continuously monitored” in a hospital seclusion room has heard extraordinary allegations that staff left their posts to attend a breakfast function on the morning that he died.
The allegations were raised by legal counsel representing the family of David Duncan (42), who was found unresponsive at 10.30am on January 21 last year in the seclusion room in the psychiatry department of Midland Regional Hospital, Portlaoise.
