{"id":108077,"date":"2025-10-28T08:35:11","date_gmt":"2025-10-28T08:35:11","guid":{"rendered":"https:\/\/www.newsbeep.com\/ie\/108077\/"},"modified":"2025-10-28T08:35:11","modified_gmt":"2025-10-28T08:35:11","slug":"tmc-admits-hard-balancing-act-between-health-care-providers-payers","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/ie\/108077\/","title":{"rendered":"TMC Admits \u2018Hard\u2019 Balancing Act Between Health Care Providers, Payers"},"content":{"rendered":"<p>PETALING JAYA, Oct 27 \u2014 TMC Life Sciences Berhad says private hospitals are navigating a \u201chard\u201d balancing act between clinical needs and payer cost controls, as rising medical inflation and recent policy shifts reshape hospital-insurer negotiations.<\/p>\n<p>Speaking at TMC Life Sciences\u2019 annual general meeting (AGM) press conference today, Group CEO Dr Ahmad Adzuan Abdul Rahman said pressures to contain health care costs have intensified, particularly after the government moved to <a href=\"https:\/\/codeblue.galencentre.org\/2024\/12\/bank-negara-caps-medical-insurance-premium-hikes-at-10-for-most-policyholders\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">cap medical insurance premiums<\/a> last December.<\/p>\n<p>\u201cAt the end of the day, there is a need to contain costs. We are no strangers to high inflation in health care \u2013 it\u2019s not sustainable in the long term. All payers in the health care space are trying to find that equilibrium,\u201d Dr Adzuan told reporters today.<\/p>\n<p>\u201cSo when we go into uncharted territory, when the government suddenly announces a cap on insurance, it becomes a highly combustible topic.\u201d<\/p>\n<p>He said the premium cap disrupted insurers\u2019 risk and cash management models, prompting system-wide recalibration. \u201cWhen that equation is suddenly put into upheaval, it\u2019s very difficult for them to recalibrate,\u201d Dr Adzuan said.<\/p>\n<p>As insurers tighten cost controls, hospitals are seeing changes that sometimes prioritise financial considerations over clinical judgement.<\/p>\n<p>\u201cSometimes the intent isn\u2019t bad. But due to a lack of detailed understanding of the patient journey or clinical needs, some decisions become more financial-centric,\u201d he said. \u201cAt the moment, the whole industry is working through this. It\u2019s not about right or wrong \u2013 we\u2019re trying to find equilibrium, and there will be mistakes along the way.\u201d<\/p>\n<p><a href=\"https:\/\/codeblue.galencentre.org\/2025\/10\/codeblue-releases-full-responses-in-health-insurance-survey\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">CodeBlue recently published<\/a> findings from a nationwide survey of 855 specialists in private hospitals, where 99 per cent reported insurer or third-party administrator (TPA) interference in clinical decision-making.<\/p>\n<p>Specialists described interventions that follow a <a href=\"https:\/\/codeblue.galencentre.org\/2025\/10\/deny-delay-revoke-specialists-reveal-health-insurance-underbelly-in-malaysia\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">\u201cDeny, Delay, Revoke\u201d<\/a> pattern \u2014 including reclassifying inpatient cases to outpatient or daycare, delays in guarantee letter (GL) approvals, and revocation of GLs after treatment had started.<\/p>\n<p>Respondents <a href=\"https:\/\/codeblue.galencentre.org\/2025\/10\/insurance-denies-various-drug-claims-in-malaysia-including-expensive-innovators-cheap-generics\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">also reported denials<\/a> of general anaesthesia and refusals to cover standard cancer drugs, often pushing insured patients into the public system when cashless coverage was not approved.<\/p>\n<p>TMC Life Sciences\u2019 remarks today come as other private hospitals, including <a href=\"https:\/\/codeblue.galencentre.org\/2025\/10\/ihh-clinical-judgement-cannot-be-governed-by-cost\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">IHH Healthcare<\/a> and <a href=\"https:\/\/codeblue.galencentre.org\/2025\/10\/senior-oncologist-warns-of-dangerous-insurance-interference-in-cancer-care\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Beacon Hospital<\/a>, have publicly raised concerns about insurer and TPA interference in clinical decisions and access to treatment.<\/p>\n<p class=\"has-medium-font-size\">Weighing Value Against Treatment Costs<\/p>\n<p>On drug decisions, Dr Adzuan said clinicians frequently weigh clinical value against affordability, noting differences between standard generics, branded generics, and originator drugs.<\/p>\n<p>He referenced the United Kingdom\u2019s National Health Service (NHS), where he previously worked, which uses structured health economic evaluations to determine value-based prescribing.<\/p>\n<p>\u201cMost jurisdictions reference the NHS. There is a whole ecosystem that looks at the health economics of care, but that\u2019s under a nationalised system,\u201d Dr Adzuan said. He added that the marginal benefit of newer, higher-cost drugs can be limited in real-world use.<\/p>\n<p>\u201cSometimes the incremental benefit of newer drugs is very small. You spend, say, RM100 more, but you only see one in 10,000 people benefiting from it,\u201d he said. \u201cSo it\u2019s about finding that balance, but it is tough. We don\u2019t have the infrastructure to do this health economic valuation, and on the clinician side, we will always want the best for the patient.\u201d<\/p>\n<p>Thomson Medical Group Executive Director and Group CEO Dr Melvin Heng echoed the challenges of balancing competing priorities between providers and payers.<\/p>\n<p><img loading=\"lazy\" data-recalc-dims=\"1\" decoding=\"async\" width=\"660\" height=\"441\" src=\"https:\/\/www.newsbeep.com\/ie\/wp-content\/uploads\/2025\/10\/TMC-Life-Sciences_Oct-27_1.jpg\" alt=\"\" class=\"wp-image-63275\"  \/>TMC Life Sciences group chairman Mohd Mokhtar Mohd Shariff (centre) and board members at the AGM press conference at M World Hotel Petaling Jaya on October 27, 2025. Photo credit: TMC Life Sciences Bhd.<\/p>\n<p>\u201cProviders might want to give a drug that has strong value to one patient out of 100, and give it to all 100 \u2013 which is costly. In a place where a payer is strong, they might ask for the one drug that can support 100 patients, and you might lose out on benefiting the one patient,\u201d Dr Heng said. \u201cThat\u2019s the balance we are trying to find.\u201d<\/p>\n<p>For FY2025, TMC Life Sciences reported revenue of RM345.5 million, slightly lower than RM346.4 million the previous year.
