\"Essentia<\/a>DEER RIVER, Min. \u2013 A staff member at Essentia Health \u2013 Deer River, 24-hour skilled nursing facility, is accused of taking \u201cnarcotic pain medications from the resident\u2019s medication supply for personal use\u201d.\u00a0 The investigation revolved around medicine intended for three patients.<\/p>\n

One patient had been receiving medication because they \u201cexperienced almost constant pain\u201d.\u00a0 The investigation report from the Minnesota Department of Health reads that patient had taken oxycodone \u201cfor a long time and knew the taste. Federal investigator notes indicated [patient R1] reported he thought on one or two occasions the medication he received from the [staff member] was not his oxycodone.\u201d<\/p>\n

The facility\u2019s records showed the staff member signed out oxycodone tabs for that resident \u201ctwenty-seven times, however, twenty-five of the doses were not documented\u201d in the residents electronic medication administration record.<\/p>\n

A second patient was taking morphine for pain management.\u00a0 The Minnesota Department of Health report says that same staff member signed morphine tabs out \u201celeven times, however, eleven of the doses were not documented in [the patient\u2019s] electronic medication administration record as administered.\u201d<\/p>\n

A third patient in the facility experienced pain \u201calmost constantly\u201d due to a broken leg.\u00a0 They were prescribed oxycodone.\u00a0 The Minnesota Department of Health report says, \u201c[staff member] signed oxycodone tabs out from [the patient\u2019s] supply fourteen times, however, thirteen of the doses were not documented [as being administered].\u201d<\/p>\n

A fourth patient was also prescribed tramadol for pain management.\u00a0 The staff member was assigned to that patient and the logs were correct for tramadol.<\/p>\n

The Minnesota Department of Health report says facility leadership met with the staff member and the staff member \u201cdisplayed concerning behaviors.\u201d<\/p>\n

The report continues to say that the staff member admitted to taking the medication but only the \u201cas needed medications\u201d and never took any scheduled medication from patients.\u00a0 The report also says the staff member would ask to work in the areas with patients that \u201chad the highest concentration of as needed oxycodone\u201d.<\/p>\n

Facility leadership stated prior to this incident they had no complaints or concerns with the staff member.<\/p>\n

The Minnesota Department of Health report says there were \u201cSubstantiated\u201d claims of financial exploitation by the staff member.\u00a0 That means someone \u201cuses, withholds, or disposes of funds or property of a vulnerable adult\u201d.<\/p>\n

The report says the staff member is no longer working at Essentia Health \u2013 Deer River.<\/p>\n

Essentia gave FOX21 a statement about the incidents,\u00a0\u201cThis employee is no longer with Essentia Health.\u00a0The conduct referenced in a recent report from the Minnesota Department of Health is inconsistent with Essentia\u2019s values and mission to provide compassionate, patient-centered care. We are reviewing our internal practices and protocols to help ensure that incidents like this do not occur in the future.\u201d<\/p>\n

The case was referred to The Office of Ombudsman for Long Term Care, The Office of Ombudsman for Mental Health and Developmental Disabilities, Itasca County Attorney, Deer River City Attorney, Deer River Police Department and Minnesota Board of Nursing.<\/p>\n
