A human infection was recorded by the Italian Ministry of Health on March 25, 2026.<\/p>\n<p>As an <a href=\"https:\/\/theconversation.com\/topics\/influenza-415\" rel=\"nofollow noopener\" target=\"_blank\">influenza<\/a> virologist, I can explain what this means and why I am not particularly worried by it \u2013 yet.<\/p>\n<p>What do we know about this case?<\/p>\n<p>The patient was infected <a href=\"https:\/\/www.salute.gov.it\/new\/it\/comunicato-stampa\/virus-influenzale-h9n2-identificato-caso-lombardia-attivate-le-ordinarie\/\" rel=\"nofollow noopener\" target=\"_blank\">outside of Europe<\/a> before travelling to the Lombardy region of northern Italy. Lombardy\u2019s welfare councillor Guido Bertolaso <a href=\"https:\/\/en.ilsole24ore.com\/art\/avian-influenza-lombardy-first-human-case-europe-returned-africa-heres-what-to-know-AIWVth9B\" rel=\"nofollow noopener\" target=\"_blank\">has reported<\/a> that the patient is a boy with underlying health conditions who was diagnosed after returning from a visit to Africa.<\/p>\n<p>Fortunately, his infection hasn\u2019t made him seriously unwell, but he has been placed in hospital isolation in the San Gerardo hospital in Monza. Italian public health authorities diagnosed H9N2 influenza virus infection using laboratory tests that detect the virus\u2019s genetic material.<\/p>\n<p>What is H9N2 influenza virus?<\/p>\n<p>H9N2 influenza viruses are influenza A viruses. This large group of viruses includes two of the viruses causing human seasonal influenza (H1N1 and H3N2) as well as many other viruses that infect birds.<\/p>\n<p>H9N2 influenza viruses are classified as \u201clow pathogenicity avian influenza viruses\u201d. \u201cLow pathogenicity\u201d refers to their ability to cause disease in poultry (avian influenza is a major threat to poultry farming), but it is unusual for H9N2 to cause anything other than mild illness in humans.<\/p>\n<p>H9N2 is not well suited to infecting humans, and when it does manage to do so it tends to be through <a href=\"https:\/\/www.bmj.com\/content\/392\/bmj.s605\" rel=\"nofollow noopener\" target=\"_blank\">direct contact with poultry in heavily contaminated environments<\/a>. Although this was the first human case in Europe, hundreds of human H9N2 cases have been recorded previously, <a href=\"https:\/\/www.cidrap.umn.edu\/avian-influenza-bird-flu\/italy-tracks-first-european-human-h9n2-avian-flu-case\" rel=\"nofollow noopener\" target=\"_blank\">mainly in China, but also in other countries across Asia and Africa<\/a>.<\/p>\n<p>What is the level of risk to humans?<\/p>\n<p>Hopefully, the infected patient will make a good recovery. At the moment, <a href=\"https:\/\/www.ecdc.europa.eu\/en\/news-events\/first-human-case-influenza-ah9n2-infection-imported-eu\" rel=\"nofollow noopener\" target=\"_blank\">the wider risk to humans is very low<\/a>.<\/p>\n<p>Why is this? Virologists look for <a href=\"https:\/\/theconversation.com\/scary-sounding-new-virus-in-the-news-here-are-the-questions-you-should-ask-232013\" rel=\"nofollow noopener\" target=\"_blank\">multiple factors<\/a> when assessing if an isolated human infection with an animal virus is likely to cause wider problems \u2013 in the worst case a pandemic, which avian influenza viruses have caused repeatedly in the past. This case of H9N2 currently shows no signs of this.<\/p>\n<p>We know that this particular strain of influenza virus would need to acquire mutations in order to become <a href=\"https:\/\/www.bmj.com\/content\/392\/bmj.s605\" rel=\"nofollow noopener\" target=\"_blank\">well adapted to growing in humans<\/a>. As a precaution, <a href=\"https:\/\/www.salute.gov.it\/new\/it\/comunicato-stampa\/virus-influenzale-h9n2-identificato-caso-lombardia-attivate-le-ordinarie\/\" rel=\"nofollow noopener\" target=\"_blank\">Italian public health authorities have traced contacts of the patient<\/a> to confirm there was no onwards transmission. At the moment, it seems very unlikely that this will go any further.<\/p>\n<p>However, there is a wider picture. There are many influenza viruses out there that are much more unpleasant than H9N2.
