Here\u2019s what that means"},"content":{"rendered":"<p>A new medicine for attention-deficit hyperactivity disorder (ADHD) has just entered the treatment toolbox: <a href=\"https:\/\/www.otsuka-us.com\/otsuka-shares-fda-review-update-for-centanafadine\" rel=\"nofollow noopener\" target=\"_blank\">centanafadine<\/a>, a once-a-day tablet sold under the brand name Simtriyo.<\/p>\n<p>It has been approved in the United States for adults and children aged six years and older but isn\u2019t yet available in Australia. <\/p>\n<p>So how does it work? How is it different to other ADHD medications? And what are the potential side effects?<\/p>\n<p>It belongs to a new class of ADHD drugs<\/p>\n<p>Centanafadine belongs to a new <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK595006\/\" rel=\"nofollow noopener\" target=\"_blank\">class<\/a> of ADHD medicines. It\u2019s the first to <a href=\"https:\/\/www.otsuka-us.com\/otsuka-shares-fda-review-update-for-centanafadine\" rel=\"nofollow noopener\" target=\"_blank\">affect<\/a> three brain chemicals: noradrenaline (known as norepinephrine in the US), dopamine and serotonin. Others usually only target two: dopamine and noradrenaline.<\/p>\n<p>Brain cells <a href=\"https:\/\/www.betterhealth.vic.gov.au\/health\/conditionsandtreatments\/attention-deficit-hyperactivity-disorder-therapies\" rel=\"nofollow noopener\" target=\"_blank\">use chemicals<\/a>, called neurotransmitters, to send messages to each other: <\/p>\n<p>dopamine and noradrenaline are <a href=\"https:\/\/www.adhdaustralia.org.au\/potential-future-of-treatment-of-adhd-moving-beyond-stimulants\/\" rel=\"nofollow noopener\" target=\"_blank\">involved in<\/a> attention, motivation and self-control<\/p>\n<p>serotonin <a href=\"https:\/\/www.adhdaustralia.org.au\/potential-future-of-treatment-of-adhd-moving-beyond-stimulants\/\" rel=\"nofollow noopener\" target=\"_blank\">helps regulate<\/a> things such as mood, sleep and appetite.<\/p>\n<p>Normally, after a neurotransmitter sends its message, the brain cell takes it back up. This is called reuptake.<\/p>\n<p>Think of it like sending a text message and then quickly deleting it. Centanafadine <a href=\"http:\/\/doi.org\/10.18773\/austprescr.2025.043\" rel=\"nofollow noopener\" target=\"_blank\">slows down<\/a> the brain\u2019s deleting process. This leaves more of these chemicals available for longer, helping brain cells communicate.<\/p>\n<p>By increasing the activity of dopamine, noradrenaline and serotonin, centanafadine may help reduce problems with attention, hyperactivity and self-control.<\/p>\n<p>How does it differ from ADHD stimulants?<\/p>\n<p>Most ADHD medicines are stimulants. These include dexamfetamine, lisdexamfetamine and methylphenidate. <\/p>\n<p>Stimulants mainly increase the activity of two brain chemicals: dopamine and noradrenaline.  <\/p>\n<p>Centanafadine works in a similar way, but also affects serotonin. This makes centanafadine different from existing stimulant medicines. <\/p>\n<p>However there is debate about whether centanafadine is a stimulant or not. The pharmaceutical company that made the drug <a href=\"https:\/\/www.otsuka-us.com\/discover\/articles-1000\" rel=\"nofollow noopener\" target=\"_blank\">called it a non-stimulant<\/a> back in 2017. But the US Food and Drug Administration calls it a stimulant in the <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2026\/218145s000lbl.pdf\" rel=\"nofollow noopener\" target=\"_blank\">product information<\/a>. <\/p>\n<p>In any case, having another treatment option is useful because not everyone responds to ADHD medicines in the same way.<\/p>\n<p>How is it different to antidepressants?<\/p>\n<p>Centanafadine affects serotonin, but that doesn\u2019t make it an antidepressant.<\/p>\n<p>Most commonly used antidepressants <a href=\"https:\/\/www.healthdirect.gov.au\/antidepressants\" rel=\"nofollow noopener\" target=\"_blank\">mainly affect<\/a> serotonin, although some also affect noradrenaline. These are important for mood regulation<\/p>\n<p>Commonly used antidepressants generally don\u2019t increase dopamine activity enough to treat the main symptoms of ADHD. Although some of the older antidepressants do affect dopamine activity, they haven\u2019t been extensively studied to treat symptoms of ADHD.<\/p>\n<p>Centanafadine affects reuptake of all three chemicals: dopamine, noradrenaline and serotonin. This is what makes it different from most common antidepressants. <\/p>\n<p>How is it different to Strattera?<\/p>\n<p>Strattera is the brand name for atomoxetine.<\/p>\n<p>Atomoxetine mainly increases the amount of noradrenaline available in the brain. It has <a href=\"http:\/\/doi.org\/10.3389\/fpsyt.2021.780921\" rel=\"nofollow noopener\" target=\"_blank\">much less effect<\/a> on dopamine and serotonin. <\/p>\n<p>Because it has less of an effect on dopamine reward pathways in the brain, it\u2019s not considered a stimulant.<\/p>\n<p>How effective is it?<\/p>\n<p>Centanafadine was <a href=\"https:\/\/www.otsuka-us.com\/otsuka-shares-fda-review-update-for-centanafadine\" rel=\"nofollow noopener\" target=\"_blank\">tested in four large phase-3 clinical trials<\/a> before it was approved in the US. These studies found people taking centanafadine had improved ADHD symptoms. The medicine was also generally well tolerated.<\/p>\n<p>But the studies compared centanafadine with a placebo, rather than directly comparing it with medicines such as methylphenidate, dexamfetamine or atomoxetine.<\/p>\n<p>This means we <a href=\"https:\/\/www.otsuka-us.com\/otsuka-shares-fda-review-update-for-centanafadine\" rel=\"nofollow noopener\" target=\"_blank\">cannot yet say<\/a> if centanafadine works better than existing ADHD medicines.<\/p>\n<p>Only some of the results from these studies have been made publicly available. For most of the trials, researchers have reported their own ratings of people\u2019s symptoms. We have less information about how participants themselves felt the medicine affected them.<\/p>\n<p>One <a href=\"https:\/\/doi.org\/10.1016\/j.jaac.2025.06.023\" rel=\"nofollow noopener\" target=\"_blank\">study in teenagers<\/a> found ADHD symptoms improved within about a week of starting centanafadine. The medicine was generally well tolerated, but some participants experienced side effects such as a reduced appetite, a headache, nausea and a rash.<\/p>\n<p>Another <a href=\"https:\/\/doi.org\/10.18553\/jmcp.2024.30.6.528\" rel=\"nofollow noopener\" target=\"_blank\">study<\/a> compared the results of centanafadine trials with previous studies of medicines such as lisdexamfetamine and atomoxetine. It suggested centanafadine may cause less insomnia, anxiety, dry mouth and loss of appetite than lisdexamfetamine.<\/p>\n<p>However, this was an indirect comparison. That means the medicines were not tested against each other in the same clinical trial. So these results should be interpreted carefully.<\/p>\n<p>What are the most common side effects?<\/p>\n<p>The <a href=\"https:\/\/otsuka-us.com\/media\/static\/Simtriyo-PI.pdf\" rel=\"nofollow noopener\" target=\"_blank\">most common side effects<\/a> reported with centanafadine include reduced appetite, insomnia, dry mouth and headache.<\/p>\n<p>These side effects can also occur with other ADHD medicines. <\/p>\n<p>The prescribing information also includes a warning about suicidal thoughts and behaviours. Similar <a href=\"https:\/\/otsuka-us.com\/media\/static\/Simtriyo-PI.pdf\" rel=\"nofollow noopener\" target=\"_blank\">warnings are made<\/a> for other medicines used to treat ADHD and depression.<\/p>\n<p>Researchers and the company that makes the drug will continue to monitor the medicine as more people use it.<\/p>\n<p>Why isn\u2019t it available in Australia?<\/p>\n<p>Centanafadine has not been approved by Australia\u2019s <a href=\"https:\/\/www.tga.gov.au\/resources\/artg\" rel=\"nofollow noopener\" target=\"_blank\">Therapeutic Goods Administration<\/a> (TGA). Nor has it been listed on the <a href=\"https:\/\/www.pbs.gov.au\/home\" rel=\"nofollow noopener\" target=\"_blank\">Pharmaceutical Benefits Scheme<\/a> (PBS). <\/p>\n<p>If it passes Australia\u2019s regulatory process, it may become available through normal treatment pathways.
