A snake lying in the crop is almost impossible to see until it’s too late.

Researchers who pooled four decades of records found that snakebites aren’t scattered accidents at all. They cluster around farm work, unprotected feet, and the middle of the afternoon.

Snakebite victims are far from care

Chandrashekhar Janakiram of the Amrita JBI Centre for Evidence Synthesis and Implementation led the work.

His team pulled 142 studies published between 1987 and 2025, covering more than 214,000 recorded snakebites in Asia, Africa, the Americas, Australia and Europe.

The team did not compare bitten people with unbitten ones, so the review can’t say how much any one habit raises the odds.

Jaideep C. Menon, a co-author at the Amrita Institute of Medical Sciences in Kochi, has a name for this sort of distribution. He used it in an interview with Earth.com.

“The inverse care law is particularly relevant to snakebite; The people most exposed to snakes – because of where they live and the work they do – are also the people furthest from a hospital that can treat them,” Menon explained.

“You cannot fix that gap with antivenom alone; antivenom sits at the wrong end of the problem.”

Most snakebites occurred during farm work

Among the 47,892 records that described what a person was doing at the time of a snakebite, farming and plantation work accounted for 59.1 percent.

That covers ploughing, weeding, harvesting and irrigation, plus work on tea, coffee, rubber and palm plantations.

Another 9.1 percent happened during other work, including construction, logging, sawmill work and brickmaking. Walking accounted for 8.4 percent of instances.

Catching, killing or rescuing a snake accounted for 3.6 percent of the records, while 2.4 percent of cases occurred while a person was resting or sleeping.

Fishing and water work accounted for 0.4 percent, with most of those people bitten while handling snakes caught in their nets.

The majority of victims were bitten below the knee. Among 85,593 records that noted a place on the body, 79 percent of bites were on the lower limb.

The afternoon was the peak window

Nearly 16,000 records included a time of day, and they challenged the picture of a snake finding a sleeper at midnight.

Among records with a time window, 39 percent of bites happened between noon and 6 p.m., and another 29 percent between 6 p.m. and midnight. Only 12 percent happened between midnight and 6 a.m.

Those are working and traveling hours. Where records were sorted only into day or night, daytime came out slightly ahead, 52.6 percent to 47.4 percent.

Season matters as much as the hour. More people are bitten during the monsoon and the months after it, when flooding displaces snakes and people at once.

In Tamil Nadu, more than 70 percent of cases fell between June and September.

Vipers responsible for most snakebites

A snake species was identified in 116,759 records. Vipers and pit vipers accounted for 85.12 percent of them, cobras for 2.84 percent and kraits for 1.9 percent.

The species involved depended heavily on the country. Russell’s viper turned up in 51.3 percent of the Indian records, brown snakes in 43.1 percent of the Australian ones and Bothrops species in 90.4 percent of cases in Brazil.

An antivenom made against one group of snakes does little against another.

India’s is built for the four species known there as the Big Four, and the hump-nosed pit viper isn’t one of them. It caused 7.5 percent of the Indian bites here and a third of the Sri Lankan ones.

Menon and colleagues argued in a 2025 paper that India should look past those four.

Victims often reach hospitals too late

The World Health Organization (WHO) estimates that there are 5.4 million bites a year worldwide and 81,000 to 138,000 deaths. India accounts for close to half of them, roughly 58,000 people a year.

Mud floors, thatched roofs and open ventilation let snakes into rooms where families sleep. In one Bangladesh study, sleeping on the floor came with more than four times the risk.

“What surprised me is that the contexts and situation resulting in bite and complications were similar across geographies and regions,” Menon told Earth.com when asked what stood out in the data.

He described the typical snakebit victim as a man between 20 and 60, working outdoors, bitten on the leg or foot, poor, and slow to reach hospital care.

Due to the high costs involved, many victims choose to visit a faith healer instead of a hospital, and the subsequent care will often push the household deeper into poverty.

Menon would spend on education first

The WHO road map sets four goals for halving snakebite deaths by 2030: empower communities, secure safe treatment, strengthen health systems and build partnerships.

Asked by Earth.com which one would save lives fastest, Menon picked the first without hedging.

“Of these, I would put my money on the strategy of empowering and educating communities.”

His list starts in schools, with awareness programs and a version of snakes and ladders built to teach the same lessons. Then sessions tailored job by job, so a plantation worker and a sawmill hand each hear about their own workplace.

He recommended ordinary equipment too: a torch after dark, boots instead of sandals, a mosquito net tucked in tight. He added first aid, and what not to do after a bite.

Researchers still lack data outside India

More than 60 percent of the studies came from India, where the burden is heaviest, and one country can’t stand in for the rest. Reporting varied so much that the team couldn’t break its findings down by country.

The review also found little evidence testing whether footwear and better lighting actually reduce the incidence of bites. Venomous snakes are moving into new areas as the climate warms.

Menon said the strategies will be shared at global meetings, starting with the World Congress on Public Health in Cape Town in September. He said patients treated early rarely suffer serious harm.

“Most complications and deaths arise from delayed diagnosis and treatment,” he said.

His list is cheap, and the evidence behind it is much thinner than the evidence behind antivenom. Researchers would have to follow the same villages for years to see whether boots and torches change the count.

The full study was published in the journal PLOS Neglected Tropical Diseases.

—–

Like what you read? Subscribe to our newsletter for engaging articles, exclusive content, and the latest updates.

Check us out on EarthSnap, a free app brought to you by Eric Ralls and Earth.com.

—–

Global snakebite patterns are predictably correlated with a variety of environmental and socioeconomic factors.
