Ms Jennifer Kearney<\/p>\nThe Chief Executive Officer (CEO) of Cork University Hospital (CUH) has called for greater clarity on the cost of implementing Sl\u00e1intecare, warning that the health service may not be able to afford to deliver the reforms at the pace needed to achieve the benefits of the strategy.<\/p>\n
Speaking at the IHCA Annual Conference in Kilkenny, Ms Jennifer Kearney said there needed to be a \u201cclear budget and a realistic timeline\u201d for the full implementation of the strategy.<\/p>\n

Ms Kearney said she first encountered Sl\u00e1intecare while working in the UK in 2019, shortly before the Covid-19 pandemic, adding: \u201cI was really impressed.\u201d<\/p>\n
She praised Sl\u00e1intecare\u2019s \u201call-party vision for the future of Irish healthcare\u201d, its commitment to reform and its ambition to ensure access to care is based on need, with services delivered \u201ccloser to home\u201d.<\/p>\n
However, Ms Kearney said the pandemic had \u201cdisrupted\u201d the reform process.<\/p>\n
She said it was now necessary \u201cto be honest about what it will take to deliver\u201d the strategy.<\/p>\n
According to Ms Kearney, implementing Sl\u00e1intecare required greater clarity on the cost of the original vision, how much had been spent on reform rather than the wider health budget, and what it would cost to achieve the strategy\u2019s aims today.<\/p>\n
\u201cMy concern is not that Sl\u00e1intecare was the wrong ambition\u2026 [but] it appears we can\u2019t afford to implement it at the pace needed to reap the benefits.\u201d<\/p>\n
The CUH CEO told the audience that she had spent 22 years working in the NHS before joining the HSE 15 months ago.<\/p>\n
\u201cI knew that systems and structures would be different; I was probably a little naive about how different,\u201d she said.<\/p>\n
\u201cIt felt in some respects like stepping back in time, maybe 10 or 15 years. The infrastructure, the constraints, the funding arrangements, the complexity of the Pay and Numbers Strategy, the limited autonomy available for those responsible for delivering services.\u201d<\/p>\n
Earlier this year, Ms Kearney was involved in a high-profile disagreement with healthcare officials over staffing levels and recruitment at CUH.<\/p>\n
In August, she wrote to HSE South West appealing for a derogation for CUH from the current employment restrictions, and the approval of a minimum of 172 priority posts to ensure a safe service entering the winter months.<\/p>\n
During her address, Ms Kearney pointed out that the \u201cquality of clinicians in Ireland is nothing short of extraordinary\u201d.<\/p>\n
However, she warned that Ireland cannot build a world-class health system \u201con the goodwill of people who are already giving all that they have\u2026 and we cannot go on confusing the extraordinary resilience of our workforce with the sustainability of the service itself\u201d.<\/p>\n

Ms Kearney also called for a national programme of genomic screening for every newborn, recommending that it be \u201cintroduced in carefully evaluated phases, with informed parental choice, robust safeguards, and a clinical service to act on the results\u201d.<\/p>\n

She added that newborn screening was \u201conly the beginning\u201d, highlighting the wider potential of genomics to transform the diagnosis and treatment of disease.<\/p>\n
\u201cGenomics can transform how we diagnose rare diseases, identify inherited risk, treat cancer, and select medicines more likely to work for individual patients,\u201d Ms Kearney told the meeting.<\/p>\n
She acknowledged the progress made through the National Genetics and Genomics Strategy, the National Genetics and Genomics Office, and the establishment of a national genomic test directory.<\/p>\n
