Eight North Bay hospitals received an “A” for patient safety in the Leapfrog Group’s fall Hospital Safety Grades released Thursday, with three local facilities maintaining perfect scores over the past five grading periods.

Novato Community Hospital, Sutter Santa Rosa Regional Hospital and Kaiser Permanente San Rafael Medical Center earned “Straight A” recognition for the past five cycles (fall 2023–fall 2025), while Kaiser San Rafael also has posted flawless “A” grades going back as early as 2022.

Other North Bay hospitals achieving an “A” this round include Adventist Health Ukiah Valley, Providence Queen of the Valley Medical Center in Napa, David Grant USAF Medical Center at Travis Air Force Base adjoining Fairfield, Kaiser Permanente Vacaville Medical Center, Sutter Solano Medical Center in Vallejo and Petaluma Valley Hospital.

Of the 18 graded hospitals in Marin, Mendocino, Napa, Solano and Sonoma counties, the eight “A” grades represent a strong regional showing. Petaluma Valley Hospital marked one of the biggest improvements, leaping from consistent “C” and “D” grades to an “A.” Providence Queen of the Valley also climbed from a string of “C”s to an “A.”

Not all local facilities fared as well. MarinHealth Medical Center in Greenbrae dropped to a “C,” and Kaiser Permanente Santa Rosa fell from five consecutive “A”s to a “B.”

North Bay hospitals not graded were in Lake and Mendocino counties: St. Helena Hospital Clearlake, Sutter Lakeside Hospital, Adventist Health Howard Memorial in Willits and Adventist Health Mendocino Coast in Fort Bragg.

The independent Leapfrog grades, issued twice yearly, are the nation’s only ratings focused exclusively on preventable errors, accidents, injuries and infections — harm that affects one in four hospital patients and contributes to as many as 250,000 deaths annually.

Nationally, Leapfrog noted that 94% of “A” hospitals and 95% of the 358 “Straight A” hospitals belong to larger health systems. Kaiser Permanente (27 “A” hospitals), Sutter Health (15), AdventistHealth (29), and Prime Healthcare (24) all ranked among the top 10 systems for total “A” and “Straight A” facilities. Utah again led states with the highest percentage of “A” hospitals.
