Efforts to improve outcomes have increasingly focused on understanding its biological heterogeneity and\u00a0identifying\u00a0patient subgroups that may respond differently to therapy.\u00a0<\/p>\n<p>Acute Respiratory Distress Syndrome Subtypes Identified Rapidly\u00a0<\/p>\n<p>In this large multicentre study, researchers prospectively enrolled patients with acute respiratory distress syndrome or acute\u00a0hypoxaemic\u00a0respiratory failure in intensive care units. Using a near-patient immunoassay, they measured key inflammatory biomarkers, including\u00a0IL-6 and\u00a0TNF\u00a0receptor-1, alongside blood bicarbonate levels.\u00a0<\/p>\n<p>These measurements enabled classification into two acute respiratory distress syndrome subtypes,\u00a0hyperinflammatory and\u00a0hypoinflammatory,\u00a0within approximately one hour,\u00a0demonstrating\u00a0feasibility for real-time clinical use.\u00a0<\/p>\n<p>Higher Mortality in Hyperinflammatory Subtype\u00a0<\/p>\n<p>Among patients successfully\u00a0subphenotyped, 18% were classified as hyperinflammatory. This group experienced markedly worse outcomes, with a 60-day mortality rate of 51%, compared with 28% in the\u00a0hypoinflammatory\u00a0group.\u00a0<\/p>\n<p>Even after adjustment for confounding factors, the hyperinflammatory subtype remained strongly associated with increased mortality risk, reinforcing the clinical significance of these biological differences.\u00a0<\/p>\n<p>From Retrospective Evidence to Real-Time Practice\u00a0<\/p>\n<p>Previous\u00a0research had consistently\u00a0identified\u00a0acute respiratory distress syndrome subtypes and linked them to prognosis and treatment response. However, these findings were based on retrospective analyses, limiting their immediate clinical applicability.\u00a0<\/p>\n<p>This study provides the first prospective evidence that\u00a0subphenotypes\u00a0can be reliably\u00a0identified\u00a0in real time at the bedside. The use of a near-patient testing platform across multiple centres also\u00a0demonstrates\u00a0the practicality of implementing this approach in routine care.\u00a0<\/p>\n<p>Toward Precision Medicine in Acute Respiratory Distress Syndrome\u00a0<\/p>\n<p>The findings support a shift toward\u00a0<a href=\"https:\/\/www.emjreviews.com\/respiratory\/article\/precision-medicine-for-airway-diseases-interview-with-kian-fan-chung\/\" rel=\"nofollow noopener\" target=\"_blank\">precision medicine<\/a>\u00a0in managing acute respiratory distress syndrome subtypes. Real-time identification opens the door to more targeted treatment strategies and improved patient stratification in clinical trials.\u00a0<\/p>\n<p>Taken together with\u00a0previous\u00a0evidence, the results confirm that inflammatory\u00a0subphenotypes\u00a0are clinically meaningful and actionable.
