A team of researchers from China conducted the study and published their findings in Clinics and Research in Hepatology and Gastroenterology. They explained that their goal was to determine a \u201cfixed inflammatory-response score in ulcerative colitis biologic studies.\u201d<\/p>\n
Using \u201cplatform-specific probe mapping and accession-level baseline standardization,\u201d the study authors calculated a \u201cfixed 200-gene score.\u201d In addition, the study team used three randomized datasets, including \u201cactive-versus-placebo\u201d data to estimate \u201ctreatment-by-score interactions.\u201d<\/p>\n
To analyze the prognostic associations of baseline mucosal inflammation, the investigators used data from 7 pre-treated patient cohorts, while they determined longitudinal change and cellular localization using information from 6 paired cohorts and \u201cdonor-aware single-cell summaries,\u201d respectively.<\/p>\n
The investigators reported that the \u201crandomized branch\u201d of the study included 681 patients. They reported odds ratios for the treatment-by-score interactions from the 3 randomized datasets of 0.64, 0.46, and 0.76. However, the 95% confidence intervals (CI) of the datasets \u201cwere wide and crossed one.\u201d<\/p>\n
Out of 681 total participants, 541 individuals were receiving active treatment, and among those patients, higher baseline mucosal inflammation scores \u201cwere associated with poorer outcome (pooled odds ratio 0.44, 0.33-0.58).\u201d<\/p>\n
In contrast, among the 154 patients in the six paired cohorts, respondents showed a lower follow-up score after adjusting for baseline mucosal inflammation (-1.07 standardized units, -1.61 to -0.53).<\/p>\n
\u201cAnalysis of 212,623 cells localized the programme across epithelial, myeloid, and fibroblast\/pericyte compartments,\u201d the study authors wrote. \u201cBaseline compartment analyses included only 18-19 donors and did not establish a remission association.\u201d<\/p>\n
The investigators reflected on the findings and highlighted that baseline mucosal inflammation in patients with UC was consistently shown to correlate with inferior clinical outcomes, as well as a \u201cresponse-associated decline\u201d in patients across the evaluated datasets, although \u201ctreatment-effect modification\u201d was not established by the study evidence.<\/p>\n
