Explore highlights from nine studies:

JACC: Cardiovascular Interventions

Staged vs. Concomitant PCI and TAVR on AKI

A systematic review and meta-analysis conducted by Veda Chanda, BA; Philippe Garot, MD, MSc, FACC; and colleagues of 14 studies comprising 20,576 patients found no difference between staged and concomitant PCI and TAVR in terms of overall acute kidney injury (AKI; relative risk [RR], 1.02; p=0.92) or in-hospital AKI (RR, 1.16; p=0.63). Staged PCI was associated with lower risks of 30-day AKI (RR, 0.64; p<0.01) and stage 3 AKI (RR, 0.59; p<0.01), although the latter association was no longer statistically significant after exclusion of one large cohort.

RV Function Following TTVR

A cardiac computed tomography angiography (CTA) analysis by Marcos Ferrandez-Escarabajal, MD; Gilbert H.L. Tang, MD, MSc, MBA, FACC; and colleagues of 35 patients (mean age, 80 years; 71% women) with severe or greater tricuspid regurgitation found that conventional measures of right ventricular (RV) systolic function initially worsened after transcatheter tricuspid valve replacement (TTVR) with the Evoque system but recovered toward baseline by one year. Among 10 patients with one-year imaging, RVEF changed from 44.3% at baseline to 34.7% at 30 days and to 44.8% at one year. Right ventricular end-diastolic volume index progressively decreased from 156.15 to 122.49 to 107.47 mL/m2, while effective RVEF improved from 26% to 33% to 40%.

Early Safety Outcomes of ScienCrown Valve in AR

A multicenter analysis by Yang Chen, MD, and colleagues of 111 patients (mean age, 73 years; 37% women) with severe pure aortic regurgitation (AR) undergoing TAVR with the fully retrievable, bottle-plug anchor ScienCrown valve reported favorable early procedural and safety results. Valve deployment required no retrieval in 48 patients, one retrieval in 33, two in 17, and three or more in 13 patients. Outcomes included one valve-in-valve TAVR, one complete atrioventricular block and three left bundle branch blocks, with no deaths before discharge.

TRIPLACE: Elevated dTVG After TTVR Identifies Risk

A discharge landmark analysis by Matteo Biroli, MD, and colleagues of 474 patients (mean age, 76 years; 64% women) from the international, multicenter TRIPLACE registry found that an elevated discharge tricuspid valve gradient (dTVG) identified patients at increased risk of adverse outcomes. A gradient ≥3.5 mm Hg was associated with three-year mortality (adjusted hazard ratio [aHR], 1.84; p=0.022) and a composite of death or heart failure (HF) hospitalization at one (aHR, 1.77; p=0.009) and three years (aHR, 1.77; p=0.004). Larger indexed inner valve diameter was associated with lower odds of an elevated gradient and was the only identified factor that could potentially be modified.

Supraflex Cruz SES vs. SYNERGY EES

In a randomized comparison of 1,548 patients (median age, 69 years; 24% women) with de novo three-vessel coronary artery disease undergoing contemporary PCI, Patrick W. Serruys, MD, FACC, and colleagues found the Supraflex Cruz sirolimus-eluting stent (SES) (n=774) was noninferior to the SYNERGY everolimus-eluting stent (EES) (n=774) for a one-year composite of all-cause death, any stroke, any myocardial infarction or any revascularization (15.23% vs. 14.51%; risk difference, 0.72 percentage points; p for noninferiority=0.025).

TRIVIR: TViV/TViR in High-Risk Patients

An analysis by Razvan Berghi, MD, and colleagues of 75 high-risk patients (mean age, 62 years; 69% women) in the international, multicenter TRIVIR registry evaluated tricuspid valve-in-valve (TViV; n=62) and tricuspid valve-in-ring (TViR; n=13) procedures using Sapien or Myval valves. No deaths occurred within 30 days, while periprocedural complications occurred in 24% of patients, including minor complications in 17%. At three years, Kaplan-Meier estimates of all-cause mortality and a composite of death, HF hospitalization and tricuspid bioprosthesis degeneration were 35.6% and 42.9%, respectively, with patients who died more likely to have markers of advanced disease.

JACC: Clinical Electrophysiology

Amulet IDE: Determinants of Major Bleeding After LAAO

An analysis by Jens Erik Nielsen-Kudsk, MD, DMSc, and colleagues of 1,833 patients from the Amulet IDE trial found that increasing age (HR, 1.04; p<0.001), prior bleeding (HR, 1.98; p<0.001), renal or urinary disease (HR, 1.98; p=0.003) and anemia (HR, 2.12; p<0.001) were independently associated with major bleeding through five years after LAAO. Five-year cumulative major bleeding was 31% among patients with baseline anemia vs. 14% in those without anemia. Of patients who experienced major bleeding, 22% had a preceding minor bleed.

JACC: Advances

CULPRIT-STRAIN: RWSmax in Culprit vs. Nonculprit Vessels

In the multicenter CULPRIT-STRAIN study, Ioannis Skalidis, MD, PhD, and colleagues analyzed earlier angiograms from 101 patients (median age, 69 years; 23% women) who subsequently developed acute coronary syndrome (ACS). Maximum radial wall strain (RWSmax) was higher in future culprit vessels than in nonculprit vessels (20% vs. 11%; p<0.001), and the culprit vessel had the highest RWSmax in 95% of patients (p<0.001). Higher culprit-vessel RWSmax was associated with a shorter interval to ACS, although 60.4% of patients had elevated RWSmax in at least two vessels.

Japanese Structural Heart Diseases Registry: Tracking BPA-Related Complications

An analysis by Tadao Aikawa, MD, PhD, FACC, et al., of 5,403 balloon pulmonary angioplasty (BPA) sessions (mean patient age, 67 years; 68% women) found BPA-related complications in 751 sessions (14%). Severe events were uncommon, but included three periprocedural deaths, six sessions requiring invasive mechanical ventilation and seven sessions requiring extracorporeal membrane oxygenation. A hospital volume ≥150 BPA sessions over three years was associated with higher odds of BPA-related complications (adjusted odds ratio, 1.60; p=0.019), whereas operator volume was not independently associated. When treated separately as continuous variables, both hospital and operator volume were associated with complications, but neither remained independently associated after mutual adjustment.

Read all the simultaneously published research at the JACC Journals’ TCT 2026 Event Page.
