However, the landscape is changing. With increasingly available brain MRI surveillance and growing recognition of the potential cognitive effects of cranial radiation, the role of PCI is being reconsidered.

MRI Surveillance May Offer an Alternative to PCI

In an abstract presented at the 2026 American Society for Radiation Oncology (ASTRO) annual meeting, investigators shared results of an international phase 3 trial evaluating whether regular brain MRI surveillance could be an alternative to PCI for patients with SCLC who had completed initial therapy and had no evidence of brain metastases on MRI. The trial enrolled 304 patients between January 2020 and December 2025, with 68% having limited-stage disease. Patients were randomly assigned to MRI surveillance alone or MRI surveillance plus PCI, with brain MRIs performed every 3 months during the first year and every 6 months during the second year. Cognitive function was assessed at the same intervals using standardized neurocognitive tests.

The study’s primary end point was cognitive failure–free survival (CFFS), defined as the time to cognitive failure or death. CFFS was significantly longer among patients receiving MRI surveillance alone, with a 40% reduction in the risk of cognitive failure or death compared with those who received PCI (HR, 0.60; P=0.001). The benefit was consistent regardless of whether patients had limited- or extensive-stage disease or received immunotherapy. Notably, 77% of patients who received PCI were treated with hippocampal-sparing PCI, a radiation approach designed to reduce cognitive effects, yet MRI surveillance remained associated with better CFFS.

Preliminary overall survival results showed no apparent difference between the groups (HR, 0.90; 90% CI, 0.67-1.20), although the overall survival analysis is not yet final. Patient-reported outcomes provided additional support for MRI surveillance. Although there was no statistically significant difference in patient-reported cognitive concerns between groups at the prespecified day 180 assessment, longitudinal analyses showed that patients receiving MRI surveillance reported better cognitive function over time. Quality of life was also significantly better at 90 and 180 days among patients receiving MRI surveillance alone.

Together, these findings suggest that in the setting of regular MRI surveillance, omitting PCI may preserve survival while reducing cognitive failure and improving quality of life for patients with SCLC.

Nursing Considerations

Returning to the patient in this case, the nurse can explain that choosing MRI surveillance instead of PCI does not mean leaving the patient unprotected from brain metastases. Rather, it means using regular brain MRI to detect intracranial disease early.

When MRI surveillance is used in place of PCI, nurses can help patients understand that surveillance is not simply “watching and waiting,” but an active component of the treatment plan. In this trial, patients underwent brain MRI every 3 months during the first year and every 6 months during the second year, making adherence to the surveillance schedule particularly important. Nurses can reinforce the rationale for regular imaging, help identify barriers to keeping appointments, and encourage patients to promptly report new neurologic symptoms, such as headaches, rather than waiting for the next scheduled MRI.

As the role of PCI and MRI surveillance continues to evolve, oncology nurses can help patients navigate these decisions by providing education and incorporating each patient’s treatment goals into the discussion.

For many years, PCI has been an important component of SCLC treatment following a response to initial therapy.…

[10
163,85,46
post-664973","post","type-post","status-publish","format-standard","has-post-thumbnail","category-health","tag-health","tag-il","tag-israel
["GET
