Healthy versions of low fat and low carb diets that prioritized plant-based foods, whole grains, and unsaturated fats were associated with a lower risk of coronary heart disease. Unhealthy versions of the same diets high in refined carbohydrates, animal fats, and proteins were associated with a higher risk of coronary heart disease.<\/p>\n<p>The quality and content of low-carbohydrate (carb) and low fat diets may be more important in reducing heart disease risk than the amount of fat or carbs consumed.<\/p>\n<p>New research shows that healthy versions of <a href=\"https:\/\/www.healthline.com\/nutrition\/low-carb-diet-meal-plan-and-menu\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">low carb<\/a> and low fat diets that emphasized plant-based foods, unsaturated fats, and whole grains were associated with a significantly lower risk of <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/health\/heart-health\/coronary-heart-disease-vs-coronary-artery-disease\/\" rel=\"nofollow noopener\" target=\"_blank\">coronary heart disease (CHD)<\/a>.<\/p>\n<p>\u201cOur findings highlighted that it\u2019s not simply about cutting carbs or fat, but it\u2019s about the quality of foods people choose to construct those diets,\u201d <a href=\"https:\/\/hsph.harvard.edu\/profile\/zhiyuan-wu\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">Zhiyuan Wu<\/a>, a postdoctoral research fellow at the Harvard T.H. Chan School of Public Health and lead author of the study, said in a <a href=\"https:\/\/www.eurekalert.org\/news-releases\/1115327\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">press statement<\/a>.<\/p>\n<p>\u201cThe efficacy of low carb and <a href=\"https:\/\/www.healthline.com\/nutrition\/healthy-low-fat-foods\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">low fat <\/a>diets in reducing CHD risk is a topic of ongoing debate, and past studies showed mixed findings. Focusing only on nutrient compositions but not food quality may not lead to health benefits,\u201d Wu continued. <\/p>\n<p>In conducting their study, the researchers examined data from nearly 200,000 participants across three large studies. Data gathered included more than 30 years of follow-up with each participant.<\/p>\n<p>During that time, 20,033 cases of coronary heart disease were noted.<\/p>\n<p>Participants\u2019 dietary patterns were assessed using food frequency questionnaires.<\/p>\n<p>The role of diet quality in low carb and low fat diets has until now remained unknown. The study is regarded as one of the most comprehensive analyses to date related to diet quality, metabolomic profiles, and coronary heart disease risk.<\/p>\n<p>The researchers found that unhealthy versions of low fat and low carb diets that were high in <a href=\"https:\/\/www.healthline.com\/health\/heart-disease\/good-fats-vs-bad-fats\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">animal-based fat<\/a> and protein and refined carbohydrates were associated with a higher risk of coronary heart disease.<\/p>\n<p><a href=\"https:\/\/urldefense.com\/v3\/__https:\/www.keckmedicine.org\/provider\/oana-maria-penciu\/__;!!JkGBRS3n8cDS!h3WyBrPilIl5uN9m_90_0EvdaxJMKwphwqptTenqvVcVaK5_NjW_5qT-oPduzUff9MaPANw0jmnM3NSPx2nJTrUj5Ow%24\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">Oana Penciu<\/a>, MD, a cardiologist with Keck Medicine of USC, who was not part of the study, said the findings are important.<\/p>\n<p>\u201cIt\u2019s not a surprise, but it\u2019s the verdict we\u2019ve been waiting for. For years, the debate over \u2018low carb\u2019 versus \u2018low fat\u2019 diets has been muddled by inconsistent research results. This study resolves years of conflicting data by showing that quality, not just quantity, is the missing variable,\u201d Penciu told Healthline.<\/p>\n<p>\u201cThis aligns with prior trials such as <a href=\"https:\/\/clinicaltrials.gov\/study\/NCT00924937\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">CORDIOPREV<\/a> and <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5274550\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">DIETFITS<\/a>, which showed that healthy versions of both diets \u2014 emphasizing <a href=\"https:\/\/www.healthline.com\/health\/nutritionists-guide-to-plant-based-protein\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">plant-based proteins<\/a>, unsaturated fats, and whole grains \u2014 provide similar protective benefits. The takeaway is clear: the source of your food matters far more than the label attached to it,\u201d she added.<\/p>\n<p>In the United States, both low carb and low fat diets are popular. But Penciu said that many people are missing the fundamentals in following these diets in a healthy way.<\/p>\n<p>\u201cThere remains a significant gap in public awareness. While <a href=\"https:\/\/www.healthline.com\/nutrition\/23-studies-on-low-carb-and-low-fat-diets\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">low carb and low fat diets<\/a> are widely practiced, unhealthy versions \u2014 high in refined carbohydrates or animal fats \u2014 are still common,\u201d Penciu said. <\/p>\n<p>\u201cA low fat diet high in sugar can be just as harmful as a low carb diet high in <a href=\"https:\/\/www.healthline.com\/health\/food-nutrition\/saturated-and-unsaturated-fat\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">saturated fat<\/a>. A low fat cookie is still a cookie. Past inconsistencies in research likely contributed to confusion, as studies often defined diets very differently,\u201d she added. <\/p>\n<p>In the United States, heart disease is the <a href=\"https:\/\/www.cdc.gov\/heart-disease\/data-research\/facts-stats\/index.html\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">leading cause<\/a> of death for both males and females, and for most racial and ethnic groups.<\/p>\n<p>Every 34 seconds, someone in the United States dies from cardiovascular disease.<\/p>\n<p>Coronary heart disease is the most common form of heart disease and, in 2022, was responsible for 371,506 deaths.<\/p>\n<p>Maintaining a healthy diet is a key factor in reducing the risk of <a href=\"https:\/\/www.healthline.com\/health\/heart-disease\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">heart disease.<\/a><\/p>\n<p>\u201cDiet is an incredibly important component of cardiometabolic health. The food that you eat will influence cardiovascular risk factors such as <a href=\"https:\/\/www.healthline.com\/health\/high-blood-pressure-hypertension\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">blood pressure<\/a>, cholesterol, and weight,\u201d said <a href=\"https:\/\/www.memorialcare.org\/providers\/cheng-han-chen\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">Cheng-Han Chen<\/a>, MD, board certified interventional cardiologist and medical director of the Structural Heart Program at MemorialCare Saddleback Medical Center in Laguna Hills, CA. Chen wasn\u2019t involved in the study.<\/p>\n<p>\u201cIn addition, the quality of your diet will affect the degree of inflammation in your body, further influencing your risk of heart disease,\u201d he told Healthline.<\/p>\n<p>Chen recommended eating a heart-healthy diet rich in plants, nuts, whole grains, and lean protein, and low in <a href=\"https:\/\/www.healthline.com\/nutrition\/sodium-per-day\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">sodium<\/a>, saturated fats, and processed carbohydrates.<\/p>\n<p>\u201cFor cardiovascular health, we generally recommend a low fat diet. This study clarifies that the macronutrient components of the diet are also important and should be low in refined carbohydrates as well,\u201d he said.<\/p>\n<p>When it comes to choosing between a low fat or low carb diet, experts say both dietary patterns have benefits. <\/p>\n<p>\u201cBoth low carb and low fat diets have merit for cardiometabolic health. For a patient of relatively normal weight with <a href=\"https:\/\/www.healthline.com\/health\/high-cholesterol\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">high cholesterol<\/a>, a low fat diet may be preferable, said <a href=\"https:\/\/www.memorialcare.org\/providers\/mir-b-ali\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-1c2ok3c\">Mir Ali<\/a>, MD, bariatric surgeon and medical director of MemorialCare Surgical Weight Loss Center at Orange Coast Medical Center in Fountain Valley, CA. Ali wasn\u2019t involved in the study.<\/p>\n<p>\u201cConversely, if weight loss is a primary objective, lowering carbohydrate intake is often beneficial. Recommendations must be tailored to the individual after a close clinical evaluation,\u201d he told Healthline.<\/p>\n<p>Ali recommended starting with small changes, such as replacing one unhealthy food at a time, to allow for a gradual transition. Drastic dietary overhauls are difficult to implement and even harder to sustain long-term.<\/p>\n<p>Penciu said that instead of sticking to a specific diet, it can be helpful to focus on balance and whole foods.<\/p>\n<p>\u201cDon\u2019t stress the math; just eat real food. Build a balanced plate with vegetables, lean protein such as <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/nutrition\/is-chicken-good-for-you\/\" rel=\"nofollow noopener\" target=\"_blank\">chicken<\/a>, fish, <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/nutrition\/proven-health-benefits-of-eggs\/\" rel=\"nofollow noopener\" target=\"_blank\">eggs<\/a>, <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/nutrition\/beans-101\/\" rel=\"nofollow noopener\" target=\"_blank\">beans<\/a>, or <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/health\/video\/what-is-tofu\/\" rel=\"nofollow noopener\" target=\"_blank\">tofu<\/a>, and whole grains or <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/nutrition\/starchy-vs-non-starchy-vegetables\/\" rel=\"nofollow noopener\" target=\"_blank\">starchy vegetables<\/a>, like <a href=\"https:\/\/www.healthline.com\/nutrition\/8-health-benefits-quinoa\" class=\"content-link css-1c2ok3c\" rel=\"nofollow noopener\" target=\"_blank\">quinoa<\/a>, <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/health\/food-nutrition\/brown-rice-vs-white-rice\/\" rel=\"nofollow noopener\" target=\"_blank\">brown rice<\/a>, or <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/nutrition\/foods\/sweet-potatoes\/\" rel=\"nofollow noopener\" target=\"_blank\">sweet potatoes<\/a>. Sprinkle in a little <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/health\/food-nutrition\/healthy-fats-guidelines\/\" rel=\"nofollow noopener\" target=\"_blank\">healthy fat<\/a> (i.e., olive oil, <a class=\"content-link css-1c2ok3c sl\" href=\"https:\/\/www.healthline.com\/nutrition\/avocado-nutrition\/\" rel=\"nofollow noopener\" target=\"_blank\">avocado<\/a>, nuts, or seeds) to make meals satisfying,\u201d Penciu said.<\/p>\n<p>\u201cKeep flavors simple with herbs, spices, or a squeeze of lemon instead of sugary sauces, skip heavily processed foods when possible, and swap sugary drinks for water or tea. Focus on consistency and quality, not perfection.
