In rural New Hampshire, a community health center shuttered. And in Iowa, a Des Moines hospital system laid off dozens of employees and closed a clinic.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z00043b6rmjyalt4s@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            All these providers cited President Donald Trump\u2019s sweeping <a href=\"https:\/\/www.cnn.com\/politics\/big-beautiful-bill-text-compare-dg\" rel=\"nofollow noopener\" target=\"_blank\">domestic policy agenda package<\/a>, which slashed more than $1 trillion in federal support for health care, as a factor in their decisions.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z00053b6r44u96k3v@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The legislation, known as the \u201cOne, Big, Beautiful Bill Act,\u201d is expected to leave millions more Americans without coverage and to cut vital financial support for hospitals \u2013 a combination that could force already cash-strapped providers to pull back on services and staffing, leaving patients with fewer places and longer waits for care, particularly in emergency rooms.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z00063b6rk9peg1ny@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            At the legislation\u2019s signing, President Donald Trump touted its tax and border security provisions, downplaying the impact of the health care cuts. The <a href=\"https:\/\/www.cms.gov\/files\/document\/wftca-section-71302-faq-final.pdf\" target=\"_blank\" rel=\"nofollow noopener\">administration<\/a> and GOP <a href=\"https:\/\/emmer.house.gov\/media-center\/videos\/congressman-emmer-highlights-constituents-support-of-the-working-families-tax-cut-act\" target=\"_blank\" rel=\"nofollow noopener\">lawmakers<\/a> often refer to the package as the \u201cWorking Families Tax Cut Act.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z00073b6rglnb879u@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But Republicans hoping to tout the legislation as a point in their favor in November are confronting an escalating political backlash to the emerging consequences of those cuts, as lawmakers who supported it take the blame.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z00093b6rhc3spawa@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Many hospitals, particularly those in rural areas or who treat a lot of underserved patients, have been struggling financially for years, contending with rising expenses, workforce shortages, an aging population and federal reimbursement rates that don\u2019t cover the cost of care. Nearly 200 rural hospitals have closed or stopped providing inpatient care since 2005, according to the Sheps Center for Health Services Research.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000a3b6r49zqm4f1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The \u201cbig, beautiful bill\u201d will increase that fiscal strain, experts say. Its historic cuts to Medicaid, which include adding the first-ever work requirement to the safety net program, are expected to leave 7.5 million more people uninsured over the next decade, according to the Congressional Budget Office. Other provisions <a href=\"https:\/\/www.cnn.com\/2025\/07\/21\/politics\/americans-losing-health-insurance-trump-bill\" rel=\"nofollow noopener\" target=\"_blank\">push up that figure<\/a> to a total of 10 million.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000b3b6r3pdls30d@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But when many of those folks get sick, they will still show up at hospitals, who must provide emergency care even if they won\u2019t get reimbursed.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000c3b6rej919u8h@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Compounding the problem is the <a href=\"https:\/\/www.cnn.com\/2025\/12\/18\/politics\/aca-subsidies-cheap-plans-enrollment\" rel=\"nofollow noopener\" target=\"_blank\">expiration<\/a> of the Affordable Care Act\u2019s enhanced premium subsidies, which Republicans in Congress opted not to extend despite heavy pressure from Democrats. Some 2 million fewer people are expected to be insured this year, according to the CBO.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/nz\/wp-content\/uploads\/2026\/03\/02-gettyimages-2221729460.jpg\" alt=\"Workers from Service Employees International Union protest proposed Medicaid cuts near the US Capitol building last June.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1600\" width=\"2385\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000d3b6rffdyj61o@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In addition, the \u201cbig, beautiful bill\u201d curtailed states\u2019 ability to levy taxes on health care providers, which help states fund their share of Medicaid, and to offer special payments to beef up the reimbursement rates for hospitals and other providers, among other uses. These provider taxes and state-directed payments also boost federal Medicaid funding that states receive.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000e3b6rz8qe2z79@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cOur health care system was already fragile and now the largest cuts in health care history are going to disintegrate it further,\u201d said Vaishu Jawahar, director of policy programs at Protect Our Care, a left-leaning advocacy group.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000f3b6rmb81wukc@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Douglas Holtz-Eakin, president of the conservative-leaning American Action Forum, argued that \u201cthere\u2019s nothing that says Medicaid always has to look like the original program,\u201d and said Republicans want less systemic dependence on it.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000g3b6rafk7t7t8@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cRepublicans have always viewed Medicaid as a targeted, low-income safety net program for health insurance for those who cannot get it on their own,\u201d he said. \u201cDemocrats have taken the view that Medicaid is just another program, another way to get to universal coverage. Expand wherever you can, do what you have to. And so this just represents the collision of those views.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000h3b6r7uueg4vi@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The package also created a $50 billion Rural Health Transformation Program, which is intended to prompt states and their rural providers to enact lasting changes that will put them on a more sustainable financial path. While some providers say the program will be beneficial, others say it won\u2019t do much to help them contend with the major funding holes the bill will create.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000i3b6rt3bvmsiu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            And the rural fund doesn\u2019t do anything to help urban hospitals. Nearly 100 of these hospitals that are financially distressed and disproportionately provide care to Medicaid enrollees are most at risk of shutting down or limiting services because of the bill, according to a November analysis by Harvard\u2019s TH Chan School of Public Health.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000k3b6r166ak2yc@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Although many of the bill\u2019s main Medicaid provisions don\u2019t start kicking in until the next year or two, some hospitals and other providers are beginning to take steps now to blunt the impact of the coming cuts.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000l3b6ronwvgjlz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            St. Mary\u2019s Sacred Heart Hospital in rural Lavonia, Georgia, closed its labor and delivery unit, as well as its obstetrics and gynecology center, last fall. Expectant mothers now must drive about 30 minutes to the nearest hospital with labor and delivery services or about an hour to St. Mary\u2019s hospital in Athens, which still operates a maternity ward.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000m3b6rv50gcyd9@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            St. Mary\u2019s said in a statement that it spent 18 months trying to recruit more doctors, create partnerships and pursue other funding as it contended with an aging population and a declining number of births. But the bill played a role.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000n3b6rjavt7hrk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cRecent Congressional cuts to Medicaid solidified this decision,\u201d the hospital said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000o3b6rr5mlqurd@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Maternal care is already strained in Georgia, said Whitney Griggs, director of health policy at Georgians for a Healthy Future, an advocacy group. The state has maternal and infant mortality rates that are much higher than the national average, and only 36% of counties had a labor and delivery unit in 2022.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000p3b6r4oawu9eh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Losing a labor and delivery unit is a \u201chuge loss\u201d to the women in the area, she said\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000q3b6r79lxn5iz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Sen. Jon Ossoff, the lone Democrat running for reelection this year in a state Trump won in 2024, has pointed to St. Mary\u2019s decision on the campaign trial.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/nz\/wp-content\/uploads\/2026\/03\/03-gettyimages-2259830302.jpg\" alt=\"Sen. Jon Ossoff arrives on stage during a re-election campaign event in Atlanta, Georgia, in February.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1600\" width=\"2399\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000r3b6ray0lfcaq@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cIn Lavonia, St. Mary\u2019s Hospital, they\u2019ve announced they will no longer provide obstetric services to delivering mothers because of decisions made by politicians,\u201d Ossoff said in Atlanta last September.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000s3b6rfxdx1qj7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In Georgia promoting the \u201cOne Big Beautiful Bill Act\u201d last August, Vice President JD Vance slammed Ossoff\u2019s opposition to the legislation, saying he was \u201callegedly worried about people getting kicked off their health care.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000t3b6rajixydn4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cWell, you know what? The President of the United States made a promise, a sacred promise that the only people who are going to lose access to health care are illegal aliens who shouldn\u2019t be in this country to begin with,\u201d Vance said.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/nz\/wp-content\/uploads\/2026\/03\/04-gettyimages-2230525340.jpg\" alt=\"Vice President JD Vance speaks to supporters of President Donald Trump at an event in Peachtree City, Georgia, last August.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1600\" width=\"2400\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so77z000u3b6rrqp9bhbd@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The GOP package also contributed to MercyOne, which has 21 medical centers and multiple clinics across urban and rural Iowa, deciding to close its clinic in Ottumwa and lay off dozens of staffers.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780000v3b6r2k6g7s1p@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            MercyOne is part of Trinity Health, which said in a statement to CNN that \u201cthe recent and future government funding policy changes\u201d are expected to reduce its annual revenue by $1.5 billion.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780000w3b6rru40o4fk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            That is forcing it to make changes \u201cto ensure long-term sustainability and future growth,\u201d including cutting positions in its billing and collections department by 10.5% across Trinity Health.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780000x3b6rupw05zct@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Retired Dr. Peter Reiter, who spent four decades serving patients in Ottumwa, reacted to the closure of the MercyOne clinic there, saying that \u201cthis is not the way that\u2019s best for patients and not the best for providers.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780000y3b6r2lx0thcv@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe number of people who were former patients or just acquaintances who said, I go to MercyOne. What am I supposed to do? Where do I go? How do I get a doctor? And I don\u2019t have an answer,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780000z3b6r9pb1ixd0@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Julie Lawrence, an Ottumwa resident and a patient impacted by the MercyOne closure, shared her frustration with the disruptions.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000103b6r40sxgxj2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cYou know, I\u2019m in the elderly category,\u201d she said. \u201cI\u2019m 68, and I would just like to stay in my hometown. So, you know, if you don\u2019t feel good, you don\u2019t want to have to drive half an hour. I mean, I can, but I don\u2019t really want to.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000113b6r1ak3iept@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Like St. Mary\u2019s in Georgia, the MercyOne closures in Iowa are playing out in competitive midterms contests, with affected locations in the battleground 3rd district.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000123b6rntbg8d30@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            GOP Rep. Zach Nunn is seeking reelection there after winning in 2024 by just under 4 points, and his leading Democratic challenger, Sarah Trone Garriott, has focused her criticism on his vote for the \u201cOne, Big Beautiful Bill Act\u201d and its changes to Medicaid.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000133b6r9tmkcz0f@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cIt\u2019s a bad time for health care here in Iowa\u2019s 3rd congressional district, and it\u2019s Zach Nunn\u2019s fault,\u201d Trone Garriott <a href=\"https:\/\/www.instagram.com\/reels\/DUJgzmukZRF\/\" target=\"_blank\" rel=\"nofollow noopener\">said in a social media video<\/a>. \u201cFirst MercyOne announced they were closing their Ottumwa clinic because of federal Medicaid cuts. Now MercyOne Des Moines is announcing layoffs this March, because again, federal Medicaid cuts.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000143b6r4qh7tlu1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            This month, Nunn wrote on <a href=\"https:\/\/nunn.house.gov\/2026\/02\/23\/%F0%9F%8F%A5-protecting-iowas-health-care\/\" target=\"_blank\" rel=\"nofollow noopener\">his House blog<\/a> that he met with patients and providers at Ottumwa Regional Health Center \u201cto hear directly from those on the front lines,\u201d touting $209 million allocated to his state through the OBBBA\u2019s $50 billion \u201cRural Health Transformation Fund.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000153b6rexjq9tsr@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe message was clear and consistent: workforce shortages are stretching staff thin, operating costs continue to climb, and rural hospitals can\u2019t keep carrying the burden alone,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000163b6rxwpdg37z@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            And amid the backlash to his vote for the One Big Beautiful Bill Act, Nunn broke with his party last month on a vote to extend subsidies for the Affordable Care Act despite his vocal criticism of the program.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000183b6rwooj8e6x@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Ed Shanshala, CEO of Ammonoosuc Community Health Services in New Hampshire, had to make the painful decision to shut down its location in Franconia, which served nearly 1,600 patients in the rural White Mountains, last fall.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so78000193b6rxd9s7rc9@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Ammonoosuc, which has four remaining centers that provide primary care, behavioral health and other medical services to 26 towns, is facing a $500,000 annual loss in revenue from the bill, mainly because fewer of its patients are expected to have Medicaid coverage due to the new work requirements. The move provides about $250,000 in one-time savings since Ammonoosuc shifted the eight staffers to other locations.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001b3b6r6cizugo4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cIt buys time for us to look at other revenue opportunities consistent with our mission, but it won\u2019t come close to addressing the impact of the bill\u2019s disinvestment in our friends, family and neighbors through Medicaid,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001c3b6rieayunoz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            After states began expanding Medicaid to more low-income adults under the Affordable Care Act in 2014, the uninsured rate fell, which led to a decline in hospitals\u2019 uncompensated care costs in those states, said Cindy Mann, a partner at Manatt Health and former Obama administration official.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001d3b6rx6gfkp4c@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cWhen the uninsured rate goes up, you\u2019ll have more uncompensated care and you\u2019ll see it pretty quickly,\u201d she said. \u201cIf you have a patient base which is mostly Medicaid, Medicare and the uninsured \u2014 which is really a lot of the safety net hospitals \u2014 there\u2019s nowhere to go to, there\u2019s no cushion, there\u2019s no cost shift that can make that up.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001e3b6r68uo1ql7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In Maine, Northern Light Health\u2019s executives knew they had to factor into their multi-year plan the increased financial pressures that the \u201cbig, beautiful bill\u201d would bring. The health system, which operates nine hospitals, a medical group, nursing homes and several dozen primary care locations in the state, is projecting a $27 million annual drop in revenue from the package, said chief financial officer James Rohrbaugh. That\u2019s largely because 34,000 Mainers are expected to lose Medicaid coverage later this year and next.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001f3b6rayv49q9h@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The looming revenue decline led Northern Light Health to make some major moves last year, including significantly cutting staff, improving its payment collection and consolidating doctors\u2019 offices. It also closed one of its money-losing hospitals, though Rohrbaugh said that decision predated the legislation.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001g3b6rwn98elz2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Now, the health system is looking for other ways to become more fiscally sound \u2013 and it hopes to use some of the $190 million that Maine is receiving from the Rural Transformation Health Program this year to help it do so. Among its plans are to increase its use of telehealth to expand its ability to care for rural residents and to hire more financial counselors to more proactively aid patients with applying for coverage \u2013 including navigating Medicaid\u2019s new work requirements \u2014 or finding alternative payment assistance, Rohrbaugh said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001h3b6rg8130272@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cI see that as a big opportunity,\u201d he said of the program. \u201cThe plan for the rural transformation funds is to help figure out how to make sure health care is sustainable in rural areas.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001i3b6rtxnbkq3a@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Going forward, more hospitals and providers will have to contend with shrinking reimbursements and higher uncompensated care costs as the provisions of the \u201cbig, beautiful bill\u201d start to take effect.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001j3b6r9d3d1mq3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThey will have to determine whether they can continue offering all their current services or have to shutter some or shift them to other providers,\u201d said Lucas Swanepoel, vice president for public policy at the Catholic Health Association, which represents Catholic hospitals, long-term care facilities and other providers.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001k3b6rfvzofkoe@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe coming year or two will be really telling,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001l3b6rkevflxnk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Jennifer Mendrzycki is the CEO of TMC Health, which based out of Tucson oversees four hospitals located throughout southern Arizona, in addition to dozens of other smaller locations covering the state\u2019s large rural expanse.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001m3b6rbmgp90f4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThis is a time like we have not seen in health care before, where we\u2019ve got competing pressures,\u201d she said, discussing the impacts of the sweeping legislation. \u201cWe\u2019ve always had competing pressures, but this is more acute than I have seen it in my career.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001n3b6r38ccekpk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Mendrzycki emphasized the difficulty of simultaneously contending with Medicaid cuts and reductions in provider taxes. \u201cIt\u2019s totally untenable, and it\u2019s \u2013 we\u2019ve never, I don\u2019t think, had a time like this, in at least the time that I have been in healthcare, where both of those things are coming. It\u2019s like the perfect storm,\u201d she said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001p3b6ruvqzdix1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            It\u2019s an issue for a series of high-stakes races across all these states that could help decide control of Congress this fall.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001q3b6rtft3ct32@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In New Hampshire, the parties are vying for the open seat left by retiring Sen. Jeanne Shaheen in a state that Democratic presidential nominee Kamala Harris carried by less than 3 full points in 2024. And in Maine, Republican Sen. Susan Collins is seeking a sixth term in what\u2019s expected to be one of the most competitive 2026 races, already drawing tens of millions of dollars in ad spending reservations.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/nz\/wp-content\/uploads\/2026\/03\/06-gettyimages-2245405122.jpg\" alt=\"Sen. Holtz-Eakin said Republicans, and Collins especially, should point to the Rural Transformation Fund, though he acknowledged its limitations.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001s3b6rm89d9kpm@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe Rural Health Transformation Fund is there because of her,\u201d he said, touting her legislative efforts. \u201cSo certainly she\u2019s going to talk about that.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmm3so780001t3b6rq2sepahi@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThere\u2019s an enormous amount of discretion by (Centers for Medicare &amp; Medicaid Services Administrator Mehmet Oz) in allocating \u2013 so there\u2019s like $10 billion you have that goes out from the total of $50 billion. And about $5 billion of that is greatly at the discretion of the CMS administrator.
