It came after the Honduran government abruptly ended the Cuban medical mission under pressure from the administration of the US president, Donald Trump.<\/p>\n<p>That same day, a \u201csensitive\u201d <a href=\"https:\/\/theconversation.com\/topics\/us-state-department-59324\" rel=\"nofollow noopener\" target=\"_blank\">US State Department<\/a> memo was sent to the secretary of state, Marco Rubio. It discussed <a href=\"https:\/\/www.politico.com\/news\/2026\/03\/13\/cuban-doctors-us-pressure-00827683\" rel=\"nofollow noopener\" target=\"_blank\">the US strategy<\/a> to sabotage Cuba\u2019s medical internationalism, which has been an integral part of the island nation\u2019s foreign policy since 1960. In recent years it has also become a key pillar of its economy.<\/p>\n<p>The US has imposed <a href=\"https:\/\/theconversation.com\/cuba-has-survived-66-years-of-us-led-embargoes-will-trumps-blockade-break-it-now-276065\" rel=\"nofollow noopener\" target=\"_blank\">unilateral sanctions on Cuba<\/a> for more than 60 years. These prevent Cuba from engaging in \u201cnormal\u201d international trade \u2013 for example, third parties cannot sell goods to Cuba if 10% of their components are from US companies or subsidiaries. And Cuba cannot export goods to the US. On top of that, the US blockade <a href=\"https:\/\/www.gov.uk\/government\/publications\/overseas-business-risk-cuba\/overseas-business-risk-cuba#:%7E:text=The%20risk%20of%20US%20sanctions,on%20UK%20Export%20Finance&#039;s%20website.\" rel=\"nofollow noopener\" target=\"_blank\">severely restricts<\/a> Cuba\u2019s access to the international financial system.<\/p>\n<p>In this context, the export of medical professionals has become vital to the Cuban economy. For decades the Cuban government sent medical missions around the world as a donation to developing nations. But over the past two decades, it has developed cooperation agreements under which governments or local authorities pay the Cuban government for the medical services of its healthcare professionals.<\/p>\n<p>Attacking that revenue looks to be a key component of the US push for <a href=\"https:\/\/www.wsj.com\/podcasts\/whats-news\/is-cuba-next-inside-washingtons-push-for-regime-change\/a099a6fe-1fad-4592-a69b-931ea51a373e\" rel=\"nofollow noopener\" target=\"_blank\">regime change<\/a> in Cuba by the end of the year. This is alongside the total oil blockade imposed by Trump\u2019s <a href=\"https:\/\/theconversation.com\/the-us-is-starving-cuba-of-fuel-heres-what-a-deal-between-them-could-look-like-275765\" rel=\"nofollow noopener\" target=\"_blank\">executive order<\/a> on January 29, which has now caused multiple <a href=\"https:\/\/www.theguardian.com\/world\/2026\/mar\/22\/cubas-power-grid-collapses-in-third-nationwide-blackout-amid-us-oil-blockade\" rel=\"nofollow noopener\" target=\"_blank\">national blackouts<\/a> that have left the entire island <a href=\"https:\/\/www.nytimes.com\/interactive\/2026\/03\/19\/world\/americas\/cuba-blackout-electricity.html\" rel=\"nofollow noopener\" target=\"_blank\">in the dark<\/a>.<\/p>\n<p>It is a policy of carrots and sticks. Countries kicking out Cuban medics are offered US support for \u201cinfrastructure modernisation\u201d \u2013 things like telemedicine and virtual training. A year earlier, Rubio had announced <a href=\"https:\/\/www.state.gov\/expansion-of-visa-restrictions-policy-for-individuals-exploiting-cuban-labor\/\" rel=\"nofollow noopener\" target=\"_blank\">visa restrictions<\/a> for current and former officials and their families from anywhere in the world who took part in Cuban programmes.<\/p>\n<p>By mid-March this year, neighbouring governments fell into line. Guatemala, Paraguay, the Bahamas, Guyana and Jamaica terminated Cuban medical missions, ending decades of cooperation. <a href=\"https:\/\/www.aljazeera.com\/news\/2026\/2\/18\/why-is-the-us-targeting-cubas-global-medical-missions\" rel=\"nofollow noopener\" target=\"_blank\">In Guatemala<\/a>, more than 400 Cuban healthcare professionals, most of them doctors, are serving indigenous communities under a three decades-long partnership. The last doctors will leave by the end of the year.<\/p>\n<p>            Cuban doctors leaving Honduras in February 2026.<\/p>\n<p>The US government\u2019s attack on Cuban medical internationalism is not new. It began in 2006, the year after the oil-for-doctors programme between Cuba and Venezuela transformed the export of healthcare professionals into Cuba\u2019s <a href=\"https:\/\/time.com\/5467742\/cuba-doctors-export-brazil\/\" rel=\"nofollow noopener\" target=\"_blank\">greatest revenue source<\/a>. <\/p>\n<p>US policy sought to eliminate this income and undermine the prestige the programmes earned the island. The then US president, George W Bush, set up the <a href=\"https:\/\/www.jstor.org\/stable\/41946012\" rel=\"nofollow noopener\" target=\"_blank\">Cuban Medical Professional Parole (CMPP) Program<\/a>, encouraging Cuban medics abroad to abandon missions and defect to the US. The <a href=\"https:\/\/www.independent.co.uk\/news\/world\/americas\/barack-obama-cuba-immigration-visa-free-wet-foot-dry-foot-latest-a7524986.html\" rel=\"nofollow noopener\" target=\"_blank\">programme was ended<\/a> only in 2017, in Barack Obama\u2019s final days as president.<\/p>\n<p>Despite this, and reflecting the deficit in healthcare globally, Cuba\u2019s earnings from the export of healthcare services rose. Revenues in 2018 (the first year Cuba published <a href=\"https:\/\/www.onei.gob.cu\/anuario-estadistico-de-cuba-ano-2018\" rel=\"nofollow noopener\" target=\"_blank\">separate data<\/a> for health services) were <a href=\"https:\/\/www.scienceopen.com\/hosted-document?doi=10.13169\/intejcubastud.15.2.0203#:%7E:text=The%20Cuban%20programs%20referenced%20below,(Morales%202018:%2020).\" rel=\"nofollow noopener\" target=\"_blank\">US$6.4 billion<\/a> (\u00a34.8 billion). Trump\u2019s first administration developed policies, <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/10714839.2020.1809103\" rel=\"nofollow noopener\" target=\"_blank\">and funding<\/a>, to sabotage these programmes.<\/p>\n<p>Cuba\u2019s bill of health<\/p>\n<p>It also devised a new justification for doing this. The US government could not openly demand that countries sacrifice the health and wellbeing of their populations just to deny Cuba revenue. So instead, <a href=\"https:\/\/2017-2021.state.gov\/a-call-to-action-first-hand-accounts-of-abuses-in-cubas-overseas-medical-missions\/\" rel=\"nofollow noopener\" target=\"_blank\">it accused Cuba<\/a> of human trafficking and equated its healthcare professionals to slaves.<\/p>\n<p>Anyone who has spoken to Cuban participants \u2013 as I have \u2013 knows the overseas service contracts they sign provide them with their regular Cuban salary, plus extra remuneration from the host country. They are guaranteed holidays and contact with families. <\/p>\n<p>Even with <a href=\"https:\/\/www.jstor.org\/stable\/26690938\" rel=\"nofollow noopener\" target=\"_blank\">tens of thousands<\/a> of medical workers overseas, the state\u2019s investment in healthcare and medical training means that the Cuban population has the <a href=\"https:\/\/worldpopulationreview.com\/country-rankings\/doctors-per-capita-by-country\" rel=\"nofollow noopener\" target=\"_blank\">highest ratio<\/a> of doctors per person in the world. In 2022, it was said to have <a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o123\/rr-5#:%7E:text=Emphasis%20is%20placed%20on%20preventive,2.6%20doctors%20per%201%2C000%20inhabitants.&amp;text=Dr%20Yamila%20Guevara%20Amarales%2C%20Dr,Alvarez%2C%20MSc%20Mirya%20Valdivia%20Hernandez\" rel=\"nofollow noopener\" target=\"_blank\">nine doctors and nine nurses<\/a> for every 1,000 citizens. In the US, there are 2.6 doctors per 1,000 citizens and in the <a href=\"https:\/\/www.bma.org.uk\/advice-and-support\/nhs-delivery-and-workforce\/workforce\/medical-staffing-in-the-nhs\" rel=\"nofollow noopener\" target=\"_blank\">UK the figure is 3.2<\/a>.<\/p>\n<p>For many Cuban healthcare professionals, it signifies the fulfilment of an internationalist duty; for others a way to travel or increase their income. The Cuban government takes the lion\u2019s share of revenues and <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5184795\/#:%7E:text=Providing%20health%20professionals%20is%20now,full%20diplomatic%20relations%20with%20Cuba.\" rel=\"nofollow noopener\" target=\"_blank\">puts them back into<\/a> Cuba\u2019s universal free public healthcare provision and medical training.<\/p>\n<p>But under Trump\u2019s second administration, Rubio, the son of Cuban migrants who left the island during the Batista dictatorship, has spearheaded a renewed attack on the island\u2019s international medical programmes. The recent State Department memo stated that Cuban medical brigades were a key source of <a href=\"https:\/\/www.politico.com\/news\/2026\/03\/13\/cuban-doctors-us-pressure-00827683\" rel=\"nofollow noopener\" target=\"_blank\">\u201chard cash\u201d<\/a> for the regime.<\/p>\n<p>The <a href=\"https:\/\/www.jstor.org\/stable\/48810859\" rel=\"nofollow noopener\" target=\"_blank\">four forms<\/a> of Cuban medical internationalism practices established in the 1960s are:<\/p>\n<p>emergency medical brigades overseas<br \/>\ntreatment of foreign patients in Cuba<br \/>\ntraining foreign students as healthcare professionals, and,<br \/>\nestablishment of public healthcare facilities overseas.<\/p>\n<p>This contribution to developing nations has often been ignored or censored. But it translates into millions of lives saved and improved globally every year. Sabotaging medical internationalism would devastate Cuba.
