{"id":359085,"date":"2026-04-01T20:00:10","date_gmt":"2026-04-01T20:00:10","guid":{"rendered":"https:\/\/www.newsbeep.com\/nz\/359085\/"},"modified":"2026-04-01T20:00:10","modified_gmt":"2026-04-01T20:00:10","slug":"auckland-woman-offered-strangers-ovary-instead-of-her-own-breast-tissue-at-anatomic-pathology-service","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/nz\/359085\/","title":{"rendered":"Auckland woman offered stranger\u2019s ovary instead of her own breast tissue at Anatomic Pathology Service"},"content":{"rendered":"<p class=\"sYHrSxRJWo\" style=\"display:none\">Both patients caught up in the errors  had the same first name, and surnames weren\u2019t checked &#8211; even though Sarah said she was asked for her driver\u2019s licence three times and provided it at least twice. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cI expressed how what was occurring was not good enough,\u201d Sarah told the Herald. \u201cI\u2019ve put in a complaint and I\u2019m not happy with the response.\u201d<\/p>\n<p>\u2018I wanted to get out of there\u2019<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Sarah\u2019s formal complaint to the Anatomical Pathology Service said, \u201cThis whole incident has had a huge impact on me. I\u2019m not sure I can fully express how distressing it is.\u201d<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">It explained that she had recently been through breast cancer, which had taken a toll on her.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cBeing of M\u0101ori descent, it is really important to me to have my body sample returned so I can bury it with my ancestors,\u201d she said. \u201cCulturally, this is unacceptable.\u201d<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">The letter also said how distressed she was to have had the second woman\u2019s name and private medical details revealed to her.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">She said she was now wondering if an earlier body sample that was returned to her in 2025 and buried on the wh\u0101nau urup\u0101 with her ancestors, was even hers. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cI tried to express how culturally insensitive this incident was but was unable to finish my sentence because of how upset I was,\u201d Sarah wrote.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cI can\u2019t even fully trust the sample returned to me on the date of the incident is mine. It has made me question if an incident like this has occurred previously and gone unnoticed.\u201d <\/p>\n<p class=\"!m-0 !font-sans-italic !text-xl !font-bold !-tracking-[0.0125rem] before:content-open-quote after:content-close-quote\" data-test-ui=\"article__blockquote-quote\">I just wanted what I hoped was my body part and to get out of there<\/p>\n<p>Sarah<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Sarah told the Herald a staff member offered her a M\u0101ori liaison specialist to talk to, but she declined because she no longer trusted APS. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cI tried to express &#8230; how hard it had been for me to even come to APS that day to pick up my body part. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201c[They] offered to call someone or to give me some time. I said I just wanted what I hoped was my body part and to get out of there,\u201d Sarah\u2019s complaint letter said.<\/p>\n<p>\u2018Only first names were checked\u2019<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\"><a href=\"https:\/\/www.nzherald.co.nz\/topic\/health-nz-te-whatu-ora\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.nzherald.co.nz\/topic\/health-nz-te-whatu-ora\/\">Health New Zealand<\/a> told the Herald it accepts the claims in Sarah\u2019s letter. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Director of clinical support services Ian Costello replied to her in February, offering \u201csincere regret and apologies\u201d.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">He confirmed that only first names had been checked, which shouldn\u2019t have occurred, and that new procedures have now been introduced to prevent it from happening again. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cThe tissue management team are mindful that the handling and return of human tissue is of particular cultural importance within te ao M\u0101ori\u201d he wrote. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cI acknowledge the impact this event has had for you and regret that your experience did not align with the respect, care and dignity our service aims to provide. The tissue management team have been trained to recognise human tissue as taonga and staff have been reminded they are required to uphold manaakitanga, kaitiakitanga, and rangatiratanga through correct patient identification, respectful handling of tissue, clear and sensitive communication and by ensuring tissue is returned only to the correct patient.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cWe have no record or knowledge that an incident of this nature has previously occurred at Anatomic Pathology Service,\u201d the letter said. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Costello also confirmed the second patient had been advised of the incident.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Health New Zealand said it was not able to answer follow-up questions from the Herald about what exactly  the second patient was told. <\/p>\n<p>\u2018We\u2019d expect them to notify us\u2019<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Health New Zealand would also not tell the Herald whether it had notified the Privacy Commissioner of its breach &#8211; but said it had \u201ccomplied fully with its obligations under the Privacy Act in respect of this matter\u201d.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">Under the Privacy Act 2020, if an organisation has a privacy breach that has &#8211; or could &#8211; cause anyone serious harm, they must <a href=\"https:\/\/www.nzherald.co.nz\/topic\/privacy-commissioner\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.nzherald.co.nz\/topic\/privacy-commissioner\/\">notify the Privacy Commissioner<\/a> &#8211; and any affected people.<\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">The office of the Privacy Commissioner told the Herald that HNZ had not alerted it to the incident. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cWe would expect an agency that has given someone the wrong tissue sample \u2013 that is identified by name &#8211; to notify us,&#8221; a spokesperson said. <\/p>\n<p class=\"sYHrSxRJWo\" style=\"display:none\">\u201cTissue samples are sensitive health information.
