For this study, we examined IC among elderly patients\u2009\u2265\u200960 years at Beijing Hospital in China from November 2020 to December 2023. Initially, 745 participants were recruited. However, 284 participants were excluded based on the following criteria: (1) being under 60 years of age; (2) having incomplete intrinsic capacity data; and (3) lacking osteosarcopenia data. The final analysis included a total of 461 participants (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4#Fig1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a>). This study was conducted with the approval of the Medical Ethics Committee of Beijing Hospital (Approval Number: 2024BJYYEC-KY083-02) and was performed in accordance with the Declaration of Helsinki. Given the retrospective nature of this study and the maintenance of patient anonymity, a waiver for informed consent was granted.<\/p>\n<p>Fig. 1<a class=\"c-article-section__figure-link\" data-test=\"img-link\" data-track=\"click\" data-track-label=\"image\" data-track-action=\"view figure\" href=\"https:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4\/figures\/1\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" aria-describedby=\"Fig1\" src=\"https:\/\/www.newsbeep.com\/nz\/wp-content\/uploads\/2025\/09\/12877_2025_6424_Fig1_HTML.png\" alt=\"figure 1\" loading=\"lazy\" width=\"685\" height=\"422\"\/><\/a><\/p>\n<p>Flowchart of Participant Selection<\/p>\n<p>Assessment of osteosarcopenia<\/p>\n<p>Sarcopenia was defined as the presence of both low appendicular skeletal muscle mass index (&lt;\u20097.0\u00a0kg\/m\u00b2 for men and &lt;\u20095.7\u00a0kg\/m\u00b2 for women) and low handgrip strength (&lt;\u200928\u00a0kg for men and &lt;\u200918\u00a0kg for women), and\/or slow gait speed (&lt;\u20091.0\u00a0m\/s)[<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 11\" title=\"Chen LK, Woo J, Assantachai P, et al. Asian working group for sarcopenia: 2019 consensus update on sarcopenia diagnosis and treatment. J Am Med Dir Assoc. 2020;21(3):300\u2013e3072.\" href=\"http:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4#ref-CR11\" id=\"ref-link-section-d33242063e523\" rel=\"nofollow noopener\" target=\"_blank\">11<\/a>]. These criteria were recommended by the Asian Working Group for Sarcopenia in 2019. Additionally, individuals with a self-reported history of sarcopenia were also included. Skeletal muscle mass was evaluated using bioelectrical impedance analysis with the Inbody S10 device (Korea). Grip strength was measured using the Leaping HealthTM WL-1000 dynamometer, with two trials conducted for each hand to determine the maximum grip strength. Gait speed was assessed by timing participants as they walked 6\u00a0m at their usual pace.<\/p>\n<p>Osteosarcopenia was defined as the coexistence of osteoporosis and sarcopenia. Bone mineral density of the lumbar spine and bilateral femoral neck was measured using Dual-Energy X-ray Absorptiometry (GE Healthcare, Lunar iDXA). Osteoporosis was diagnosed based on a T-score below \u2212\u20092.5, in accordance with World Health Organization criteria [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 12\" title=\"S\u00f6zen T, \u00d6z\u0131\u015f\u0131k L, Ba\u015faran N\u00c7. Eur J Rheumatol. 2017;4(1):46\u201356.\" href=\"http:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4#ref-CR12\" id=\"ref-link-section-d33242063e529\" rel=\"nofollow noopener\" target=\"_blank\">12<\/a>]or through self-reported history of osteoporosis.<\/p>\n<p>Participants were categorized into four groups according to their osteoporosis and sarcopenia status. The normal group included individuals without either osteoporosis or sarcopenia. The sarcopenia group consisted of those with sarcopenia but without osteoporosis. The osteoporosis group included participants with osteoporosis but not sarcopenia. Finally, the osteosarcopenia group comprised individuals with both sarcopenia and osteoporosis.<\/p>\n<p>Measurements of intrinsic capacity<\/p>\n<p>IC framework was originally proposed by the World Health Organization [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 7\" title=\"World Health Organization. Integrated Care for Older People: Guidelines on Community-level Interventions to Manage Declines in Intrinsic Capacity. (2017). Available online at: &#010;                  https:\/\/www.who.int\/publications\/i\/item\/9789241550109&#010;                  &#010;                 (accessed August 20, 2022).\" href=\"http:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4#ref-CR7\" id=\"ref-link-section-d33242063e543\" rel=\"nofollow noopener\" target=\"_blank\">7<\/a>]. Two experienced and rigorously trained nurses administered the IC assessment to all participants using a face-to-face questionnaire. The IC score was calculated based on the following method: each domain is assigned a maximum of 2 points. The total IC score is then obtained by summing the scores across the five domains (maximum score\u2009=\u200910), with higher scores reflecting stronger intrinsic capacity in older adults [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 13\" title=\"L\u00f3pez-Ortiz S, Lista S, Pe\u00f1\u00edn-Grandes S, et al. We used the Mini-Mental State Examination (MMSE) to evaluate cognitive status, with scores\u2009\u2265\u200927 assigned 2 points, \u2265 10 assigned 1 point, otherwise 0 points [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 14\" title=\"Li H, Jia J, Yang Z. Mini-mental state examination in elderly Chinese: a population-based normative study. J Alzheimers Dis. 2016;53(2):487\u201396.\" href=\"http:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4#ref-CR14\" id=\"ref-link-section-d33242063e549\" rel=\"nofollow noopener\" target=\"_blank\">14<\/a>]. For evaluating vitality, the Mini Nutritional Assessment Short-Form (MNA-SF) was employed, with scores\u2009\u2265\u200912 indicating good nutritional health and earning 2 points, \u2265 8 assigned 1 points, otherwise 0 points [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 15\" title=\"Kaiser MJ, Bauer JM, Ramsch C, et al. Locomotion was assessed using the Short Physical Performance Battery (SPPB) [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 16\" title=\"G\u00f3mez JF, Curcio CL, Alvarado B, et al. Scores of \u2265\u200910 were awarded 2 points, while scores below 3 received 0 points, otherwise 1 point. Depressive symptoms were identified using the Geriatric Depression Scale-15 (GDS-15), with scores\u2009\u2264\u20094 indicating good psychological status and earning 2 points, \u2265 9 assigned 0 points, otherwise 1 point [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 17\" title=\"Yesavage JA. Geriatric depression scale. Psychopharmacol Bull. 1988;24:709\u201311.\" href=\"http:\/\/bmcgeriatr.biomedcentral.com\/articles\/10.1186\/s12877-025-06424-4#ref-CR17\" id=\"ref-link-section-d33242063e559\" rel=\"nofollow noopener\" target=\"_blank\">17<\/a>]. Sensory function was assessed based on auditory and visual impairments: 0 points for both impaired, 1 point for either vision or hearing impaired, and 2 points for both normal.<\/p>\n<p>Data collection<\/p>\n<p>Data were collected for the following variables: age, sex, education, body mass index (BMI), number of medications, Clinical Frailty Scale (CFS), blood pressure. Smoking status was assessed based on whether they currently smoke. Blood pressure was measured three times using a digital sphygmomanometer (Omron, Japan). Laboratory data were extracted from electronic medical records, including fasting plasma glucose (FPG), HbA1c, total cholesterol, high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), albumin, 25-OH-VD3, and creatinine.<\/p>\n<p>Statistical analysis<\/p>\n<p>The statistical presentation of variables was conducted as follows: Normally distributed continuous data were summarized as mean values with standard deviations, whereas non-normally distributed continuous measures were reported using median and interquartile range values. Categorical data were presented as frequency percentages. Analytical comparisons employed different approaches based on variable types &#8211; continuous variables were analyzed through one-way ANOVA while categorical differences were examined using Pearson\u2019s Chi-square tests. To address the issue of multiple comparisons, we employed the Bonferroni correction method. To investigate potential associations between IC domains and osteoporosis, sarcopenia, osteosarcopenia, multivariate logistic regression models were developed to assess relationships.<\/p>\n<p>The analytical framework incorporated two hierarchical regression models to address potential confounding variables. The base model (model 1) maintained unadjusted estimates, with successive adjustments introduced in progressive specifications: Model 2 incorporated sex, education, smoking, and number of medications. Subpopulation analyses were conducted through stratification by seven key characteristics: age tertiles, biological sex, smoking status, drinking status, body mass index (BMI) categories, stroke event history, and diabetic status. Between-strata heterogeneity was evaluated using Breslow-Day tests for interaction, with effect estimates expressed as adjusted odds ratios (ORs) with 95% confidence intervals.
