Photo \/ New Zealand Medical Journal<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">The girl \u201cabsolutely\u201d could have died, he said.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cThe infection itself was advanced and needed IV [intravenous] antibiotics and the site of infection needed surgical drainage \u2026 and then she was also given an extended course of treatment and monitoring for that dural venous sinus thrombosis [a stroke caused by a blood clot in the brain\u2019s venous drainage channels], which is the complications of the infection.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cSo she could\u2019ve died from the infection, and could\u2019ve died from complications from the infection.\u201d <\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">The girl was on intravenous antibiotics for six weeks, the first two in hospital after her discharge was delayed by her parents\u2019 reluctance to provide a home address for continuing outpatient treatment.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">She has since fully recovered.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">The girl\u2019s parents hadn\u2019t indicated they were against the prescribed medicine when she was initially seen for her then-minor infection, Arnold said.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">When her condition worsened, her parents contacted a surgeon their friend knew \u2013 another example of mistrusting the system, he said.<\/p>\n<p><img  alt=\"The girl's parents &quot;didn\u2019t trust doctors, they didn\u2019t trust politicians, they didn\u2019t trust any institutions&quot;, ophthalmology registrar Dr Morgan Arnold says.\" class=\"article-media__image responsively-lazy\" data-test-ui=\"article-media__image\"\/>The girl&#8217;s parents &#8220;didn\u2019t trust doctors, they didn\u2019t trust politicians, they didn\u2019t trust any institutions&#8221;, ophthalmology registrar Dr Morgan Arnold says.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">Once <a href=\"https:\/\/www.nzherald.co.nz\/topic\/health\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.nzherald.co.nz\/topic\/health\/\">back in hospital<\/a>, it still took \u201ca lot to convince them we needed to treat [her]\u201d even though they\u2019d been told their daughter\u2019s condition was life-threatening.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">It was \u201cvery hard\u201d to get an explanation for their reluctance, Arnold said.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cA whole bunch of mistrust of medicine and institutions and modern science as a whole without being able to drill into any reasoning for it. <\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cThey very much didn\u2019t trust doctors, they didn\u2019t trust politicians, they didn\u2019t trust any institutions.\u201d<\/p>\n<p>\u2018Massive\u2019 increase in misinformation since pandemic<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">In New Zealand and globally, medical care was increasingly complicated by rising health <a href=\"https:\/\/www.nzherald.co.nz\/nz\/covid-response-deliberate-disinformation-campaigns-are-a-public-health-risk-but-nz-has-no-effective-strategy-to-deal-with-it\/4YN5P3NDXJAXZIRLN3OTAWCSOY\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.nzherald.co.nz\/nz\/covid-response-deliberate-disinformation-campaigns-are-a-public-health-risk-but-nz-has-no-effective-strategy-to-deal-with-it\/4YN5P3NDXJAXZIRLN3OTAWCSOY\/\">misinformation, which had \u201cmassively increased\u201d after the Covid-19 pandemic<\/a> and in connection with increased social media use, Arnold and Elder wrote in their report.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">In 2022, a baby boy needing life-saving heart surgery was placed under the temporary guardianship of the High Court after his parents opposed him receiving transfused blood from vaccinated donors.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\"><a href=\"https:\/\/www.nzherald.co.nz\/nz\/baby-blood-donor-vaccine-battle-this-isnt-over-baby-w-parents-vow-to-take-action\/O53YJ4QJ4FBM7AAKHJIUKMWGB4\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.nzherald.co.nz\/nz\/baby-blood-donor-vaccine-battle-this-isnt-over-baby-w-parents-vow-to-take-action\/O53YJ4QJ4FBM7AAKHJIUKMWGB4\/\">Critically ill Baby W<\/a> was subsequently taken from his parents by police before the ultimately successful surgery, sparking protests outside the court and Starship children\u2019s hospital by supporters of the parents.<\/p>\n<p><img  alt=\"Baby W and his mother outside the High Court at Auckland in December 2022 with lawyer Sue Gray (left) and anti-vaccine protester Liz Gunn (right). Photo \/ Alex Burton<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">Because they relied on parental adherence to evidence-based medicine, children were particularly vulnerable, Arnold and Elder\u2019s report says.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cIdeally, clinicians should employ proactive advocacy strategies to ensure parental co-operation with medical treatment without relying on evidence of disease progression to sway parental opinion.\u201d<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">Early discussions around the treatment provided and any barriers the patient foresaw could identify where further information was needed, with clinicians needing to be skilled and confident in getting the evidence base of their recommendation across.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cClinicians should approach these conversations with empathy and care to facilitate an open and honest dialogue.\u201d<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">A \u201chigh index of suspicion\u201d and proactive management of health literacy in patients and their families, particularly in unwell children, was needed to avoid complications, they wrote.<\/p>\n<p>\u2018We don\u2019t judge you\u2019<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">Arnold\u2019s message to <a href=\"https:\/\/www.nzherald.co.nz\/topic\/parenting\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.nzherald.co.nz\/topic\/parenting\/\">parents was to be<\/a> upfront, too. <\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cWe lucked out with [this girl] not having any long-term complications \u2026 but the next kid might not [be as fortunate].\u201d<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">If a parent wanted to try an alternative treatment, they should tell their doctor, he said.<\/p>\n<p class=\"bcxzNmxesLZ\" style=\"display:none\">\u201cWe don\u2019t judge you \u2026 if it\u2019s not harmful, we won\u2019t discourage it.
