\n \u201cThat\u2019s a structural thing.
\n We shackle nurses into healthcare systems and medical practices.\u201d\n <\/p>\n
Smith founded Caci Clinic with her husband in 1994, and grew it to a nationwide network of more than 70 clinics.<\/p>\n
In 2020, after selling a 47% stake to New Zealand\u2019s largest private equity firm, Direct Capital, she stepped back, although she still sits on the board.<\/p>\n
Smith started her career as a nurse, though.<\/p>\n
In fact, she says she has only just lapsed as a registered nurse.<\/p>\n
Smith, who grew up in Cambridge, went to Otago University to study pharmacy.<\/p>\n
\u201cTowards the end of that year I wasn\u2019t really enjoying it,\u201d she says.<\/p>\n
\u201cI saw a TV programme about nursing and thought, \u2018Oh, give that a shot\u2019.\u201d<\/p>\n
She didn\u2019t stay in nursing for long but retains a passion for it.<\/p>\n
That\u2019s what drives her current ambition to restructure many health services around nursing.<\/p>\n
\u201cWhat I\u2019ve seen in our business is that nurses can operate their own practice with absolute safety,\u201d Smith says.<\/p>\n
\u201cThey\u2019re very focused on their customers, and deliver really great service. I think that there are areas of the healthcare system where nurses have a much bigger part to play, and that\u2019s really where I want to spend my time and my energy.\u201d<\/p>\n
There are areas, like dialysis, that are already effectively run by nurses,” Smith says.<\/p>\n
\u201cWhy do we have to have them in hospitals? Why can\u2019t we have those in the community, and why can\u2019t nurses take a much bigger part in running those?\u201d<\/p>\n
It is usually about funding, she says.<\/p>\n
There is a lack of access to capital and a need for a legal framework that allows them to practise independently.<\/p>\n
\u201cIt\u2019s going to take some bold thinking to think about how we restructure our healthcare system for the future,\u201d Smith says.<\/p>\n
\u201cWe all know that we\u2019ve got an ageing population. There\u2019s chronic disease that we are going to have to manage and pay for. We can\u2019t just keep doing things the same way.\u201d<\/p>\n
Smith says she would like to set up a bipartisan working group that looks at how we might do things differently.<\/p>\n
She acknowledges that there might be some pushback applying a commercial model to public health.<\/p>\n
\u201cI don\u2019t think you can translate business straight into government,\u201d she says.<\/p>\n
\u201cBut I do think that there are lessons that you can take from business.<\/p>\n
\u201cI think we\u2019re bureaucracy-heavy, but maybe it is less about that and more about how you enable people to really make a difference.\u201d<\/p>\n
The Caci Clinic franchise model grew organically. But only after Smith and her husband David took a big leap of faith.<\/p>\n
In the late 1980s, the couple were travelling and found themselves living on Jersey in the Channel Islands.<\/p>\n
\u201cDavid and I [he was born in the UK] in the late \u201980s we kind of went exploring, as everybody does.<\/p>\n
\u201cI fell into the business world when we were in the Channel Islands and started working in a computer technology company,\u201d Smith says.<\/p>\n
Her husband had more of an entrepreneurial bent, she says.<\/p>\n

\u201cHe\u2019s probably the one that\u2019s more focused on money itself. He was quite ambitious. He was the one that wanted to be a millionaire when he was 30 and all that stuff.\u201d<\/p>\n
It was one piece of technology that changed the course of their lives.<\/p>\n
\u201cWe had been living in the Channel Islands for six or seven years, we had a 3-year-old son, and it was time to come back to New Zealand, really,\u201d Smith says.<\/p>\n
\u201cBut not long before that, David\u2019s aunt had pointed out this piece of technology, and we flew to London, and that was the original Caci machine, which was a facial toning machine.\u201d<\/p>\n
It was a piece of technology based on microcurrent.<\/p>\n
\u201cQuite quickly after that we opened our first clinic, though, we realised that we needed more than that in the business.\u201d<\/p>\n
The couple had a second child by that point.<\/p>\n
\u201cDavid was up in the middle of the night watching Tomorrow\u2019s World … the British science programme. Flying cars and all the rest. There was a programme about this laser that was being built in Wales that was treating unwanted body hair.\u201d<\/p>\n
\u201cDavid\u2019s a bit like a terrier, I suppose. He doesn\u2019t let things go. So off he went and tracked that down, and we got into laser hair removal fairly quickly after that.\u201d<\/p>\n
It was a big leap in terms of the investment required.<\/p>\n
\u201cThat was definitely kill or cure,\u201d Smith says. \u201cProbably we were a bit naive at the time.\u201d<\/p>\n
It wasn\u2019t all luck, of course.<\/p>\n
David had done an MBA in the UK, so he could see how to put a business together.<\/p>\n
\u201cI had the clinical aspect of it,\u201d Smith says.<\/p>\n
\u201cI think that was one of our core skills really, being able to take a piece of technology, which on its own it\u2019s, you need to wrap it in a system to be able to deliver something to a consumer or customer.\u201d<\/p>\n
They never intended to build a franchise, she says.<\/p>\n
\u201cIn those days there weren\u2019t all the business books and all of that kind of things that you can follow today. So, we weren\u2019t really thinking about our strategy and \u2018what are our goals\u2019 and all of that kind of thing.\u201d<\/p>\n
In fact, the goal was just to support the family, she says.<\/p>\n
\u201cBut quite quickly we had people kind of knocking on our door saying, \u2018We wanna do what you\u2019re doing, and we think that\u2019s great, and sounds like a great idea, so how can we do that?\u2019<\/p>\n
\u201cThat\u2019s when we had to decide on the pathway that we went down and ended up franchising.\u201d<\/p>\n
So what does money mean to Smith now that the Direct Capital deal has given her some real financial freedom? Is it still a goal?<\/p>\n
\u201cMoney is this kind of the housekeeping bit,\u201d she says.<\/p>\n
\u201cYou have to work and everybody says that they\u2019re focused on the customer. But most people don\u2019t really understand what it is to be focused on designing a business that delivers exactly what that customer needs.\u201d<\/p>\n
\u201cIf you do that well, and you get your structure right, then you make money. But you have to get those other things right first.\u201d<\/p>\n
As for her attitude to money over the years, Smith was always focused on growing the business.<\/p>\n
\u201cI think probably until we sold part of the business to Direct Capital, up until then, I think we still lived a pretty frugal life. We kept reinvesting in the business.<\/p>\n
\u201cYou\u2019re also time poor as well. And we paid ourselves a lot less than if we went out and got a job.\u201d<\/p>\n
But they still enjoyed life.<\/p>\n
\u201cI mean, we had some good holidays,\u201d Smith says. \u201cWe had good opportunities to travel.\u201d<\/p>\n
They were often able to link family holidays to work trips, she says.<\/p>\n
\u201cWe would go to conferences and that kind of thing, and perhaps have a holiday at the end of that.\u201d<\/p>\n
Nowadays Smith has the freedom to travel much more on her own terms.<\/p>\n
