A vulnerable adult patient at a mental health facility in South Carolina was slammed to the floor by an employee inside a seclusion room, according to an arrest warrant. <\/p>\n

The warrant says Thomas Smalls, 27, is charged with abuse of a vulnerable adult. <\/p>\n

According to the warrant, on Aug. 4, Smalls, who was a custody officer at Recovery Solutions in Columbia, took a patient into a seclusion room and slammed them onto the floor. <\/p>\n

The warrant said the patient had a 2- to 3-inch laceration on the head. <\/p>\n

According to the South Carolina Law Enforcement Division, Recovery Solutions is contracted for operation by the South Carolina Department of Behavioral Health and Developmental Disabilities Office of Mental Health.<\/p>\n

SLED reported that Recovery Solutions requested the investigation. <\/p>\n

Smalls was booked into the Alvin S.
