It\u2019s the focus of the current government shutdown, of course. Democrats are refusing to sign off on a budget that cuts Medicaid benefits for poor Americans in exchange for tax breaks for the rich.<\/p>\n<p>Meanwhile, insurance premiums are set to go sky high next year. They rise every year, but surveys show that the median proposed increase across the country for 2026 is 18 per cent, which is double last year\u2019s rate and triple 2024\u2019s. Federal tax credits for premium spikes may also expire, which means that the total inflation in middle class tax brackets could be as much as 80 per cent, and for the poorest Americans, out of pocket expenditures on premiums could rise by 400 per cent.<\/p>\n<p>Now, businesses will be able to negotiate to reduce some of the proposed premium hikes, but even if you were to get, say, a nine per cent increase that\u2019s a huge impact for most people. The bottom line is that healthcare affordability may end up being a make-or-break political issue in the midterm elections.<\/p>\n<p>It\u2019s ironic that Donald Trump\u2019s first proposal for dealing with the issue is taking a page out of the book of Obamacare (which had in turn taken ideas from what Republican Mitt Romney did in Massachusetts) with his \u201cTrumpRX\u201d proposal, which would create a federal marketplace where individuals could buy cheaper prescription drugs. Of course, he\u2019s also threatened tariffs on European drugmakers (unless they move production to the US) in ways that could potentially drive-up costs on certain drugs. So, it\u2019s tough to figure out where it all nets out.<\/p>\n<p>In my ideal world, we\u2019d both use the purchasing power of the federal government to create a strong demand signal as a way of reducing prices, and we\u2019d not only not cut Medicaid, but we\u2019d have a single payer system for the entire country, with an option for private insurance that you can buy if you want it \u2014 just like the UK and many other countries do. <\/p>\n<p>As it is, the complexity and decentralisation of American healthcare make it rife with rent seeking and inefficiency, a topic which I\u2019ve documented in <a href=\"https:\/\/www.ft.com\/content\/9eaa44ca-082e-11e8-9650-9c0ad2d7c5b5\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">many<\/a> <a href=\"https:\/\/www.ft.com\/content\/a7b8899e-0081-11ea-b7bc-f3fa4e77dd47\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">past<\/a> <a href=\"https:\/\/www.ft.com\/content\/4aed2bc5-6bcb-4938-b565-d69031311f8a\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">columns<\/a>.<\/p>\n<p>Nationalised healthcare won\u2019t happen anytime soon in the US, of course \u2014 unless the hard left gains more steam (that\u2019s an issue I look at in my <a href=\"https:\/\/www.ft.com\/content\/a0ad5408-8abf-42af-9d5a-ec0b0ddf4acc\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">Monday column<\/a>). But there are so many bits of low hanging fruit that could be pulled in the US healthcare system to make things cheaper and more efficient, from creating a national medical records system (so you wouldn\u2019t have to deal with new paperwork every single time providers shift, or you move states), to focusing on prevention rather than after the fact treatment. It just amazes me that we don\u2019t do more.<\/p>\n<p>I\u2019m lucky today to have as my respondent the FT\u2019s US healthcare reporter, Patrick Temple-West. Patrick, my question for you is two-fold. First, what will the impact of the Medicaid cuts be, economically, in your opinion? And what do you think of TrumpRx? Finally, do you agree that healthcare inflation will rise sharply next year? It put me in mind of how the largest technology firms have decoupled from the economic fortunes of the countries in which they reside, even as investors know that there will be two tech stacks \u2014 one American, and one Chinese. So, how long does the divide last?<\/p>\n<p>A good <a href=\"https:\/\/www.wsj.com\/business\/tylenol-autism-kirk-perry-acetaminophen-trump-7dc652e3?gaa_at=eafs&amp;gaa_n=ASWzDAiol-mDzMQW3ah4vDQQsKojmDxMLkdBei_E6BiV3VxHnYM4XazVu4AHIrQI0fI%3D&amp;gaa_ts=68e00677&amp;gaa_sig=cGPDka_CryjCg67CpXOiv5DkOKSpRIskggp9sb2MpMjrR0sHk_SxvhQZCzVa8RPfZi2bxF8kjy6tcqNlTCwj-w%3D%3D\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">feature<\/a> on Tylenol\u2019s crisis following Trump\u2019s comments linking the medication to autism (with no good medical data). It made me think about how foolish it is for individual CEOs to try and \u201cmanage\u201d the president \u2014 they\u2019d do better to band together against his attacks. <\/p>\n<p>Two great opinion pieces in the FT to read, one from <a href=\"https:\/\/www.ft.com\/content\/f836b4e2-38d7-4ffb-b526-2a777988b0ff\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">Martin Wolf <\/a>on a world in which we now have not one but two predatory superpowers and another from FT alumna and former Canadian deputy prime minister and finance minister <a href=\"https:\/\/www.ft.com\/content\/8d5907de-50f3-43fa-bea0-5d9667641330\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">Chrystia Freeland<\/a> on how Ukraine is actually innovating in defence, and how that could be a huge advantage for Europe. This echoes a bit of what I <a href=\"https:\/\/www.ft.com\/content\/11a6b844-fe57-4e39-86ba-bb04e839bf2f\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">wrote<\/a> a few months ago on how Ukraine heralds the biggest shift in warfare since Genghis Khan.<\/p>\n<p>And just for fun, I\u2019ve been watching and loving <a href=\"https:\/\/www.hbomax.com\/shows\/100-foot-wave\/34e489ad-2eca-4078-adb6-6d2b521b407e\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">this HBO Max series<\/a> on big wave surfers. As someone who finds it difficult to stay up on a board for more than five seconds, I can\u2019t believe what these guys (and a few gals) do.\u00a0<\/p>\n<p>Patrick Temple-West replies<\/p>\n<p>Hi Rana. Medicaid coverage dramatically increased during and after the Covid-19 pandemic. Republican\u00a0deficit hawks say Medicaid has got too big and is rife with fraudulent\u00a0claims. The budget reconciliation bill from earlier this year slashed Medicaid, prompting the Congressional Budget Office to estimate 7.8mn Americans would lose health insurance.<\/p>\n<p>Republicans argued people who might lose Medicaid coverage are too often <a href=\"https:\/\/budget.house.gov\/press-release\/wall-street-journal-editorial-board-about-those-millions-losing-medicaid\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">sleeping, watching television and playing video games<\/a>. These people would be better off getting jobs and earning their health insurance.<\/p>\n<p>On the other hand, Medicaid covers a whopping <a href=\"https:\/\/www.aha.org\/fact-sheets\/2025-02-07-fact-sheet-medicaid\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">40 per cent of all childbirths in the US<\/a> and nearly 50 per cent of births in rural communities, according\u00a0the American Hospital Association.\u00a0<\/p>\n<p>People are inevitably going to lose Medicaid coverage. If you think that instability is bad for the economy, then Medicaid cuts are harmful. These cuts also inevitably add financial stress to hospital systems. These hospitals\u00a0issue debt to municipal bond investors. All these stress fractures aren\u2019t great for the economy.<\/p>\n<p>On TrumpRx, the White House took great pains to say it isn\u2019t getting into the drug distribution business. This website sounds like it will work as a search\u00a0engine to help people find cheaper drugs. During Obama\u2019s second term, the troublesome rollout of <a href=\"http:\/\/healthcare.gov\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">healthcare.gov<\/a> hurt him politically. TrumpRx isn\u2019t as complicated, but it also isn\u2019t as consequential.<\/p>\n<p>Finally, healthcare inflation is real and will be shooting higher next year. It isn\u2019t a question, it is just a matter of how much it will eat into people\u2019s disposable spending. Will the inflation be high enough to sully Trump\u2019s halo in the eyes of his voters?
