It’s only natural to wonder how many years you might have left together. For the first time, a prescription drug built specifically to stretch a dog’s lifespan is within sight of veterinary clinics. What I wouldn’t have given for more time with my beloved dog Marta, so I understand.

Loyal, a San Francisco biotech company, is developing LOY-002, a once-daily tablet for dogs 10 and older who weigh at least 14 pounds. The company has already cleared two of the three major technical sections the FDA requires before it can grant conditional approval, which means the pill could be available as early as late 2026 or 2027.

That’s a big deal for the 44 percent of pets that the American Animal Hospital Association classifies as seniors. It’s also fertile ground for hype and misunderstanding. So I connected with Dr. Rebecca Greenstein, a practicing vet who fields these questions in exam rooms every week, to learn more about what the drug really is, who it’s for, and what she’d want you to know before you ask for a prescription.

About our expert: Dr. Rebecca Greenstein, B.Sc., D.V.M., is the Veterinary Medical Advisor for Rover and the Chief Veterinarian and practice owner at Kleinburg Veterinary Hospital in Ontario, Canada, where she cares for aging dogs every day.

What the dog anti-aging pill actually does

LOY-002 is designed as a once-daily tablet that targets age-related metabolic changes rather than any single disease.

(gostua / Shutterstock.com)

The phrase “anti-aging pill” could easily be misunderstood, so Dr. Greenstein explained, “The term ‘anti-aging pill’ often implies freezing time, turning back the block, or even something cosmetic. LOY-002 specifically targets the metabolic concept that aging is linked to lowered sensitivity to insulin (and other body wide changes) so the thought is that improving sensitivity to insulin, and decreasing high insulin blood levels, could potentially increase longevity. The goal isn’t simply to add months or years to lives marked by frailty and illness but to prolong what’s known as healthspan, the time spent with vitality and less loss of function.”

The science behind that idea isn’t new. In a landmark 14-year Purina study published in 2002, Labrador retrievers fed 25 percent fewer calories than their littermates, stayed leaner, developed chronic disease later, and lived a median of nearly two years longer. Loyal has pointed to that research in its own explainer on why metabolic health is a longevity lever, and LOY-002 is intended to deliver some of the same metabolic benefits without putting a hungry older dog on a lifelong diet.

Why big dogs age faster than small dogs

Large breeds like Great Danes age faster than small dogs like Chihuahuas, a gap Loyal’s large-dog drugs LOY-001 and LOY-003 are designed to narrow.

(pixshots / Shutterstock.com)

Great Danes often don’t make it past eight, while Chihuahuas can trot into their late teens. Researchers who analyzed death records from 74 breeds in The American Naturalist concluded that large dogs die young mainly because they age faster; it’s as though their adult lives run on fast-forward. A separate study in Science found that a single variant of the IGF1 gene, which codes for insulin-like growth factor 1, is common to nearly all small breeds and almost absent in giant ones.

LOY-002 isn’t the drug that targets IGF-1. Loyal’s two large-dog programs do that: LOY-001, a long-acting injection for dogs 7 and older weighing at least 40 pounds, and LOY-003, a daily pill for dogs 5 and older weighing at least 60 pounds, according to the company’s veterinary brief.

“Dogs are an interesting example of how lifespan is negatively associated with body size. There’s not one singular explanation, but multiple complex factors at play, including rate of growth, metabolic factors, and higher activity in growth-related hormone signalling systems, to name a few,” Dr. Greenstein explained. “This higher activity in growth signalling pathways isn’t necessarily ‘bad’ but can accelerate aging, so LOY-001 and LOY-003 works on the premise that less excessive growth signaling as an adult may be linked with less age-related metabolic dysfunction, which may delay biological aging.”

What FDA conditional approval means for a pill your dog takes every day

Loyal announced in a Business Wire release that the FDA’s Center for Veterinary Medicine accepted the drug’s safety package, following its earlier acceptance of the “reasonable expectation of effectiveness” section in February 2025. CEO Celine Halioua told Fast Company she hoped to submit that final section in 2026.

So what does “conditional” mean once the pill is sitting on a pharmacy shelf? Dr. Greenstein said, “Conditional approval doesn’t mean that the FDA has confirmed this drug will make every dog live longer. The FDA believes there’s a reasonable expectation this drug may work but it will likely take years to prove its definitive effectiveness, so the approval allows the drug to reach the market while that gold standard data is being collected.”

According to the FDA, a conditional approval lasts one year and can be renewed annually for up to five years total, during which the company must keep working toward the “substantial evidence” standard required for full approval. Dr. Greenstein told me, “Clients are already asking about them! As with most medications and treatments, it’s really about expectation management and patient selection. A patient who is frail, in poor body condition, and suffering from numerous health conditions, is likely going to experience less benefit that a patient who is in better baseline health. And I think it goes without saying that no pill could possibility be a substitute for a healthy lifestyle and routine medical care.”

That mindset lines up with the AAHA Senior Care Guidelines, which stress that old age is not a disease and that senior care works best when it’s individualized to each dog’s actual health, not just the number on the birthday cake.

Would a vet give her own dog the longevity pill?

So would Dr. Greenstein give the longevity pill to her own dog? “Once the definitive safety data has been collected and we have a clearer idea of possible risks and adverse effects, I would be completely on board with putting my dog on it,” she said. “There’s no magical age that I would start it, but it would likely be in keeping with the manufacturer’s guidelines as well as his overall health. If the risks included significant risk of death or serious illness, the risk:benefit ratio would shift for me and I would be much more hesitant.”

One more year, but what kind of year?

Still enjoying lap time, shorter walks, and favorite routines is one of the clearest signs an older dog’s quality of life is holding up.

(FOUR.STOCK / Shutterstock.com)

Loyal has said it hopes to give senior dogs at least one extra year. For anyone who has nursed an old dog through a long decline, that raises an uncomfortable question. Dr. Greenstein shared, “This raises the key question of whether healthspan is the same as lifespan; and as we know all too well in the human population, it most certainly does not. No reasonable person would want to extend their pet’s life if that extra time was spent with frailty, dysfunction, or pain. The STAY study evaluating LOY-002 specifically looks at quality of life parameters in addition to lifespan, which is why it’s so exciting.”

That trial is enormous by veterinary standards. According to dvm360, STAY is a double-blinded, placebo-controlled study running about four years across 70 veterinary clinics, and Loyal reports more than 1,300 senior dogs enrolled. Because dogs age roughly seven to ten times faster than people, as researchers behind the Dog Aging Project noted in Nature, a four-year dog trial can answer questions that would take decades in humans.

The free things that already help dogs live longer

Keeping a dog lean and active is one of the most powerful, and least expensive, ways to add healthy years, according to Dr. Greenstein.

(nimito / Shutterstock.com)

The longevity pill may not be available yet, but there’s a short list of habits that cost nothing and carry decades of evidence. Dr. Greenstein said, “Just like in people, healthspan is multifactorial. Studies prove that weight management may be one of the single most powerful variables in longevity, so keeping your dog lean is likely one of the best things you can do.”

“Regular exercise, routine preventative care like vaccines and parasiticides, screening for disease with regular wellness exams and bloodwork, as well as addressing changes in your pet’s health as early as possible, would be my recommendations to keep your dog happy and healthy for as long as you can.”

Thank you for reading.
