Ladybridge Surgery, on Broadgate, is operated by Bolton Community Practice CIC, which says it is now exploring alternative premises to continue providing care to patients.
The update comes after local councillors raised concerns about the potential impact on residents if the surgery building were to permanently close.
The organisation said the current building has faced ongoing issues and does not meet modern healthcare requirements.
A spokesperson for Bolton Community Practice CIC said: “Bolton Community Practice CIC has launched a survey for patients registered at Ladybridge Surgery to gather views on how we can improve and modernise the practice.
“The current Ladybridge building is old and we have had many issues with the building over the years.
“An NHS Greater Manchester premises review in 2023 confirmed that the site is no longer suitable and is in need of new, fit-for-purpose premises.
“Our lease has now expired, so we are exploring suitable alternative premises to ensure continuity of care for our patients.”
The surgery building has experienced problems in recent years, including a leak that forced the practice to close for several days in 2024.
The Integrated Care Board (ICB) review carried out in 2023 concluded that the premises would need replacing.
Bolton Community Practice CIC operates four GP surgeries across the borough, including sites in Horwich, Little Lever, Ladybridge and Waters Meeting Health Centre.
The practice has launched a survey asking patients and stakeholders for their views as it considers options for the future of services in the area.
The spokesperson added: “We want to make sure that any future plans for relocating the surgery reflect what matters most to the people who use it.
“We strongly encourage patients and stakeholders to complete the survey so their views can directly inform the development of the new premises and service.”
Patients registered with the practice can attend appointments at any of the organisation’s sites.
The consultation survey will close on Friday, March 27.
