Portsmouth Hospitals University NHS Trust (PHU) confirmed the incidents took place at Queen Alexandra Hospital between April 2025 and May 2026.
Of the 10 recorded incidents, five involved surgery being carried out in the wrong place, and less than five were related to the use of a wrong implant or prosthesis, retained foreign objects following a procedure, and the transfusion or transplantation of blood components or organs.

Steve Thomas, group chief nurse for PHU and IWT, said: “Our teams work hard to provide safe, compassionate care for our patients and, when things don’t go as planned, it is vital that we take steps to learn from them.
“In response to these incidents, thorough investigations were launched in line with national NHS standards.
“All reported never events are carefully reviewed to understand how they occurred, but also what actions are needed to prevent them happening again.
“Patients and families are invited to contribute to these reviews to ensure their voices are heard.
“As a Trust, we continue to strengthen our approach to incident reporting, embed best practice and share learning across departments.
“We have also welcomed external experts from the wider healthcare system to visit us and offer independent feedback.
“Following the completion of a national review into never events that is currently taking place, we will ensure that our processes align with any updated guidance.”
A breakdown by department shows that incidents occurred in areas including musculoskeletal, ophthalmology, and dermatology.
Other involved areas were theatres, pathology, orthopaedics, and maxillofacial outpatients.
The trust was not able to reveal how much was paid out in compensation for never events as it does not centrally hold information about compensation related to never events, as clinical negligence claims are managed externally through NHS Resolution.
